Traumatic vertical atlantoaxial instability in a case of atlanto-occipital coalition.

STUDY DESIGN: This is a report of a woman with an atlanto-occipital coalition who experienced a traumatic vertical atlantoaxial facet dislocation and severe associated neurologic injury. OBJECTIVE: To describe the radiographic and pathoanatomic characteristics of the injury process. SUMMARY OF BACKGROUND DATA: Severe injuries of the occipito-atlantoaxial complex are uncommon and only rarely are associated with patient survival. This is the first report of this particular injury. METHODS: Plain radiography and tomography demonstrated the facet dislocation and instability of the injury. Surgi

Indications and technique for the operative treatment of hypoplastic deformities of the odontoid process.

On the basis of four typical cases of congenital hypoplastic deformities of the odontoid process the clinical significance, symptoms, diagnosis and the indications for operative treatment of the malformations are described. It is further shown that these malformations lead to clinical manifestations only when associated with a primarily existing, or a later acquired inadequacy of the ligaments with movement-induced changes in the position of the atlas. The operative treatment recommended involves a special technique for a bilateral dorsal fixation with tibial grafts. By this operative techniqu

Acute Spinal Cord Contusion in a Patient with Multiple Upper Cervical Fractures, Parkinson's Disease, and Torticollis: Surgical Management.

Case Report. Spine surgery in patients with Parkinson's disease (PD) involves increased risk. We describe a case of cervical myelopathy in a patient with PD, multiple fractures involving the atlas and axis vertebrae, and spasmodic torticollis. The patient was successfully treated with an upper cervical decompression and occipital-cervical (OC) fusion surgery. Strategies for torticollis reduction and successful surgical outcome are discussed. Risks and benefits must be carefully weighed when considering occipital cervical fusion in PD patients. Conclusion. Intraoperative manual reduction of lat

Spontaneous Anterior Arch Fracture of the Atlas Following C1 Laminectomy in a Patient With Osteopetrosis: A Case With Five Years of Follow-Up.

Osteopetrosis is a rare group of genetic disorders characterized by excessive bone density due to impaired osteoclast function and can lead to various complications, including fractures and immune dysfunction. We describe the case of a 63-year-old man with osteopetrosis who presented with cervical discomfort and was diagnosed with an anterior arch fracture of the atlas, a rare spontaneous fracture following C1 laminectomy. Initially, no neurological abnormalities were observed, and imaging confirmed the continuity of the transverse ligament. Thus, the decision was made to follow the natural co

Injuries of the atlas and axis. A follow-up study of 85 axis and 10 atlas fractures.

In a follow-up study of ten atlas and 85 axis fractures, 12% had residual symptoms in the form of local and radiating cervical pain. The residual symptoms were interpreted as a sign of a mild demyelinating process initiated by the trauma of the medulla. At least 22% of the accidents occurred while the person was under the influence of drugs or alcohol. Therefore, a wide range of residual symptoms (progressive or nonprogressive) could be attributed in part to abuse of drugs and alcohol. A permanent measurable loss of motion occurred following injury to the atlas or axis, irrespective of the mod

Examination of the deviation of cervical spine before and after orthognathic surgery.

PURPOSE: This retrospective study aimed to quantitatively evaluate the effects of improved facial asymmetry on cervical spine deviation in patients who had undergone orthognathic surgery. MATERIALS AND METHODS: This study enrolled 51 female patients who underwent orthognathic surgery at the University of Yamanashi Hospital. Preoperative and 1-year postoperative cephalometric analysis and three-dimensional computed tomography (3D-CT) image analysis were performed. Facial asymmetry was evaluated using cephalometric analysis. In 3D-CT analysis, cervical spine deviation was assessed by first plott

Osteoid Osteoma of the Atlas in a Boy: Clinical and Imaging Features-A Case Report and Review of the Literature.

Osteoid osteoma is a benign osteoblastic tumor, quite uncommon in the spine. We report a case of an osteoid osteoma involving the atlas in a 6-year-old boy, who presented with suboccipital pain and torticollis. Initial radiological findings were ambiguous as magnetic resonance imaging showed mainly edema of upper cervical soft tissues. The subsequent computed tomography depicted a lesion of left lamina of C1. As conservative treatment failed, the lesion was surgically resected and the patient became pain free. To our knowledge, this is the first case of osteoid osteoma involving the atlas asso

The demographic, clinical, and management differences between traumatic dens fracture patients with and without simultaneous atlas fractures.

INTRODUCTION: Atlas fractures often accompany traumatic dens fractures, but existing literature on the management of simultaneous atlantoaxial fractures is limited. METHODS: We examined all patients with traumatic dens fractures at our institution between 2008 and 2018. We used multivariable logistic regression and ordinal logistic regression to identify factors independently associated with presentation with a simultaneous atlas fracture, as well myelopathy severity, fracture nonunion, and selection for surgery. RESULTS: Two hundred and eighty-two patients with traumatic dens fractures with

Human Wings Apart-Like Protein as a Serum Diagnostic Biomarker in Cervical Cancer: An Integrative Bioinformatics Analysis with Serum Validation.

Cervical cancer remains a major threat to women's health worldwide, and reliable serum biomarkers for early detection and therapeutic stratification remain limited. Human wings-apart-like (hWAPL) protein has been implicated in cervical carcinogenesis, but its diagnostic and clinical value has not been fully elucidated. To address this gap, this study integrated public multi-omics datasets, including The Cancer Genome Atlas, GEPIA2, the Human Protein Atlas, and single-cell transcriptomic data, to characterize hWAPL expression, clinicopathological associations, immune infiltration, co-expression

Transoral Closed Reduction of Fixed Atlanto-Axial Rotatory-Subluxation (AARS) in Childhood and Adolescence.

Atlanto-axial rotatory-subluxation (AARS) is the most common pediatric cervical spine injury. Patients usually present with contralateral rotation and inclination of the upper cervical spine after minor trauma, or associated with an infection of the upper respiratory tract. According to the authors, initial management of patients with acute and chronic AARS type I-II should comprise closed reduction and immobilization with a cervical collar or a Halo-Body-Jacket. Surgical options of open reduction or C1/2 fusion should be restricted to irreducible or recurrent subluxations. This paper reviews

Congenital axis dysmorphism in a medieval skeleton : …secunda a vertendo epistropheus….

PURPOSE: We describe here the axis dysmorphism that we observed in the skeletal remains of a human child dug up from a fifteenth century cemetery located in north-eastern Italy. This bone defect is discussed in the light of pertinent literature. METHODS: We performed macroscopical examination and CT scan analysis of the axis. RESULTS: Axis structure was remarkably asymmetric. Whilst the left half exhibited normal morphology, the right one was smaller than normal, and its lateral articular surface showed horizontal orientation. In addition, the odontoid process appeared leftward deviated and

Axial loading with hyperflexion injury to the atlas resulting in crushed lateral masses.

A 40-year-old man was involved in an ATV accident, in which he landed on the top of his head. There was no neurological deficit. A plain radiograph showed prevertebral soft tissue swelling at the atlas and axis level. Computed tomography (CT) demonstrated vertical fractures of the anterior aspects of both lateral masses of the atlas, extending to the junction of the lateral mass with the anterior arch bilaterally. There was no lateral offset of the lateral masses. The mechanism of injury is believed to be axial loading along with hyperflexion.
