Thoracic outlet syndrome in children.

Thoracic outlet syndrome, although uncommon in children, may be precipitated during rapid growth in adolescence, especially in association with a cervical rib. Four children exhibited the aching, tiredness in the limb, and occasional paresthesia in adolescence. Two had a cervical rib requiring removal, after which the symptoms disappeared. The other two were treated nonoperatively with shoulder-strengthening exercises and avoidance of posturing that elicited the symptoms. It is proposed that in a child, the anatomy of the thoracic outlet is constantly changing with growth and development of th

Unusual clinical presentation of cervical extradural meningioma detected with neuromuscular ultrasound: A case report.

Extradural meningiomas are rare in the cervical region. A total of 70-77% of reported cases have occurred in the thoracic region. Tumors that occur in the cervical region may invade the adjacent nerve root and brachial plexus. Typically, diagnoses of extradural meningioma are made after patients present with signs of myelopathy, such as progressive paresis and numbness. In the current study, a 64-year-old male patient presented with neck pain, numbness and mild weakness in the left hand over a 6-month period. The general neurological examination was unremarkable, except for mild grasping weakn

Resolving Inflammation: The Impact of Antiretroviral Therapy on Macrophage Traffic In and Out of the CNS.

The effect of antiretroviral therapy (ART) and therapy interruption on myeloid cell traffic out of the central nervous system (CNS) with human immunodeficiency virus (HIV) and simian immunodeficiency virus (SIV) infection are understudied. Using intracisternal (i.c.) injection of dual-colored fluorescent superparamagnetic iron oxide nanoparticles (SPION) in SIV-infected macaques early (12-14 dpi) and late (30 days before sacrifice) we studied CNS macrophage viral infection, turnover, and traffic out. SPION are preferentially taken up by perivascular, meningeal, and choroid plexus macrophages.

[The debridement of submandibular abscesses under locoregional block].

The efficiency of analgesic block of the inferior alveolar, auriculotemporal, lingual, buccal and superficial cervical plexus in patients requires debridement of unilateral submandibular abscess has been investigated. The technique was significantly more useful (p < 0.001) when compared with other analgesic block techniques utilized in these cases. This treatment can be performed in any medical office with minimal resources and be of use as an alternative for general anesthesia.

Dural arteriovenous fistula on the brain stem and upper cervical spinal cord - a case report -.

A 53-year-old man abruptly developed headache and unconsciousness. Brain computed tomography (CT) and CT angiography showed subarachnoid hemorrhage, intraventricular hemorrhage, and multiple tortuous vascular structures on the brain stem and upper cervical spinal cord. Four-vessel angiography displayed intradural ventral arteriovenous fistula, supplied by the left vertebral and occipital arteries. Drainage was via both sigmoid sinus and cervical venous plexus. He had been treated with transarterial coil embolization of the left vertebral artery. Subsequently, he suffered from left hemiplegia a

Cervical rib fracture: an unusual etiology of thoracic outlet syndrome in a child.

Thoracic outlet syndrome in children is an extremely rare disorder. Only 5 previous reports were presented in the literature with a description of thoracic outlet syndrome in 8 children. In these cases, the diagnosis of a cervical rib was common. We describe a case of neurogenic thoracic outlet syndrome in a 9-year-old patient where the initial symptoms occurred 2 months after a fall. Clinically, the patient developed progressive pain, numbness and tingling along the inner surface of the left forearm and in the palmar surface of the fourth finger and lateral aspect of the fifth finger. A plain

Pain and sensory complaints were almost universal and most probably due to concomitant injury of cervical plexus cutaneous branches, which are in direct relation to the spinal accessory nerve in the posterior cervical triangle. Lesion type is usually axonotmesis, but varied degrees of compression by cicatricial spurs are observed in some patients. Surgical app …

Innervation of the heart: Anatomical study with application to better understanding pathologies of the cardiac autonomics.

Current advances in management of the cardiac neuroaxis in different cardiovascular diseases require a deeper knowledge of cardiac neuroanatomy. The aim of the study was to increase knowledge of the human fetal extrinsic cardiac nervous system. We achieved this by systematizing the origin and formation of the cardiac nerves, branches, and ganglia and their sympathetic/parasympathetic connections. Thirty human fetuses (60 sides) were subjected to detailed sub-macroscopic dissection of the cervical and thoracic regions. Cardiac accessory ganglia lying on a cardiac nerve or in conjunction with tw

Relationship between neuropathy proximal to the suprascapular nerve and rotator cuff tear in a rodent model.

BACKGROUND: Rotator cuff tears are believed to coexist with cervical spine lesions. In cases of preexisting neuropathy, such as cervical spine lesions, fatty degeneration has likely already occurred due to the neuropathy. In these cases, rotator cuff tear is thought to occur easily because of preexisting extensive fatty degeneration and degeneration of the tendons due to neuropathy. This study aimed to evaluate the effects of paralysis due to neuropathy proximal to the suprascapular nerve on the supraspinatus and infraspinatus tendons using a rat model of brachial plexus paralysis. METHODS: T

Enigmatic osteomyelitis and bilateral upper limb palsy in a neonate.

This report describes a male infant who developed right upper limb palsy 5 days after birth and contralateral paralysis at 14 days. Abnormal in utero posture of the right arm had resulted in a difficult cephalic delivery. Right shoulder osteomyelitis was diagnosed at age 16 days from clinical, hematologic, and radiologic findings. Antibiotics were administered, followed by complete resolution of the symptoms in 2 weeks. Electromyographic and nerve conduction studies demonstrated direct involvement of the right brachial plexus, secondary to the osteomyelitis, explaining the unilateral onset and

A Case of Horner's Syndrome following Ultrasound-Guided Infraclavicular Brachial Plexus Block.

Horner's syndrome results from paralysis of the ipsilateral sympathetic cervical chain (stellate ganglion) caused by surgery, drugs (mainly high concentrations of local anesthetics), local compression (hematoma or tumor), or inadequate perioperative positioning of the patient. It occurs in 100% of the patients with an interscalene block of the brachial plexus and can also occur in patients with other types of supraclavicular blocks. In this case report, we presented a case of Horner's syndrome after performing an ultrasound-guided infraclavicular brachial plexus block with 15 mL of bupivacaine

Cross-chest C7 nerve grafting followed by free muscle transplantations for the treatment of total avulsed brachial plexus injuries: a preliminary report.

The number of donor nerves available for nerve transfer in the reconstruction of total root avulsion injuries of the brachial plexus is always insufficient. Use of the contralateral normal C7 cervical nerve as a donor nerve is a new approach to obtain more nerve fibers but also is a controversial procedure. Fifteen patients with total root avulsion of the brachial plexus received cross-chest C7 nerve grafting as the first stage of reconstruction. Eight of these patients, after an interval of 11 to 20 months, had free muscle transplantations (one to three muscles transferred per individual) to

Mechanisms of brachial plexus lesions.

The main causes of brachial plexus palsies are traction, due to extreme movements, and heavy impact. In downward traction of the arm and forcible widening of the shoulder-neck angle the lesion will occur in the upper roots and trunk. Forcible upward traction will cause avulsion of T1 and C8. The most violent trauma will result in a lesion at all levels. Rupture of the cords and/or individual infraclavicular nerves will be produced by traction and/or forcible widening of the scapulohumeral angle. Vascular structures are subjected to the same mechanism and injuries of these structures give infor

Arm pain in the patient with breast cancer.

The causes of ipsilateral arm pain were analyzed in a consecutive series of 38 patients with breast cancer. A lesion of the brachial plexus was diagnosed in 17 patients, of whom 8 had tumor involvement, 5 had radiation fibrosis, 1 had lymphedema entrapment, and 3 had a probable transient neuritis of the plexus. In four patients, a cervical radiculopathy was found; two of these patients had a Horner's syndrome. A carpal tunnel syndrome was seen in four patients and could possibly be attributed to lymphedema in two patients. In five patients, a pericapsulitis of the shoulder joint was seen. Seve

Cervical spine injuries in the pediatric and adolescent athlete.

Injuries of the cervical spine in the pediatric and adolescent athlete are less common than other musculoskeletal injuries. Although many of these injuries are relatively minor, serious and potentially unstable or progressive spinal injury must be excluded. Important anatomic differences between the child younger than 10 years and older children and adolescents influence the types of injuries sustained and make assessment of the child's cervical spine sometimes difficult for practitioners accustomed to treating adolescent and adult athletes. Stable soft-tissue injuries of the cervical spine ar

Low-output lymphatic leakage after contralateral seventh cervical nerve cross transfer: a clinical and anatomical observation.

BACKGROUND: Contralateral seventh cervical nerve (CC7) cross transfer restores upper limb function in hemiplegia after brain injury. As a cervical procedure involving dissection near major lymphatic structures, CC7 transfer carries a potential risk of lymphatic leakage, a complication well described in other neck surgeries but not systematically studied in this context. METHODS: This study aims to evaluate lymphatic leakage after CC7 transfer through a combined clinical analysis and cadaveric anatomical study. We retrospectively reviewed 507 patients who underwent CC7 transfer from May 2023 to

Shoulder Surgery Under Regional Anesthesia With Sedation Without General Anesthesia: A Case Series.

INTRODUCTION: In Brazil, shoulder surgeries are traditionally performed under general anesthesia combined with regional blocks. This study aimed to describe the feasibility and safety profile of regional anesthesia with sedation, without general anesthesia, for shoulder surgery. MATERIALS AND METHODS: Following approval by the institutional ethics committee, 10 consecutive patients undergoing elective shoulder surgery (arthroscopy, n = 6; arthroplasty, n = 4) received an ultrasound-guided superior trunk block of the brachial plexus combined with an intermediate superficial cervical plexus blo…

Great auricular nerve preservation during parotidectomy for benign tumors.

The great auricular nerve, the largest sensory branch of the cervical plexus, arises from the third cervical nerve (C3) with irregular contribution from the C2. The first part of its course is deep to the sternocleidomastoid muscle. In few years, many experiences by different authors concerning the issue of great auricular nerve integrity during parotidectomy were published in the literature. The aims of our article were to report our experience with 78 consecutive patients who underwent standard superficial, subtotal, or total parotidectomy for benign tumors and to illustrate postsurgical fin…
