[True neurogenic outlet syndrome: report of 2 cases].

True neurogenic thoracic outlet syndrome is caused by compression of the lower trunk of the brachial plexus usually by a cervical rib, fibrous band or an elongated transverse process of C7. We describe two cases of female patients (23 and 19 years old) with pain in the right superior limb and progressive muscular weakness and atrophy of the intrinsic muscles of hand. Electrodiagnostic studies showed reduced amplitude of compound muscle action potential of median nerve and decreased amplitude of ulnar sensory nerve action potential. Motor and sensory nerve conduction velocities were normal in b…

Blunt trauma precipitating arterial thoracic outlet syndrome: a case report.

A 39-year-old male who sustained polytraumatic injuries in a motor vehicle collision, including a neck seatbelt sign and acute thrombosis of the left subclavian artery adjacent to an undiagnosed hypoplastic cervical rib, presented with a threatened left upper extremity. He underwent emergent surgical exploration through a supraclavicular incision, with cervical rib resection, resection of the contused subclavian artery segment, and end-to-end anastomosis. Through an infraclavicular incision, he had a thrombectomy of the axillary and brachial arteries. Postoperatively, he developed progressive …

Generating hand dysaesthesiae: the "GHD phenomenon" - straight to the diagnosis.

Two Irish women presented with difficulty in completion of hair straightening, limited by upper limb dysaesthesiae due to claudication or brachial plexus entrapment induced by sustained shoulder abduction beyond 90 degrees. The first described arm pain precipitated by elevation and sustained abduction above shoulder level, particularly while using her GHD hair-straightener. Elevated arm stress test was positive and a left cervical rib was seen on chest x ray. Neurogenic thoracic outlet syndrome (TOS) was diagnosed and the cervical rib was resected. The second described arm tingling and weaknes…

[Phlebitis of effort and venous edema of the upper limbs. Compressions of the subclavian vein].

The authors present a series of 18 cases (13 patients) of a venous syndrome of the arm. Three were cases of phlebitis due to effort and 15 were cases of intermittent or permanent oedema of the arms. Four of the patient also presented homo- or contralateral arterial disorders. Dynamics and multiposition phlebography gave evidence on 18 occasions of a branchial compression syndrome. Twelve patients were operated upon. On 16 occasions resection of the first rib was carried out by the axillary route (in 1 case this was combined with the ablation of one cervical rib). All the patients were cured wi…

[Clinical aspects, diagnosis and surgical treatment of developmental anomalies of the cervical spine].

The examination of 20 patients with abnormal cervical spine has identified Klippel-Feil disease in 12 of them, isolated vertebral synostoses of the cervical spine in 8. In 2 patients with Klippel-Feil anomaly neurovascular syndrome was reported which resulted from compression of the neurovascular bundle by extra cervical ribs. Indications to surgical treatment in Klippel-Feil disease are specified. The authors performed operations to correct the cosmetic defect and relieve compression of the neurovascular bundle in neurovascular syndrome.

Treatment of thoracic outlet syndrome with combined scalenectomy and transaxillary first rib resection.

Of a total of 225 patients with suspected thoracic outlet syndrome, 37 (16.4%) underwent surgery. Some eight patients required bilateral operations. One patient had a cervical rib and one a prominent C7 transverse process. A total of 45 limbs were operated on. Thirty-nine procedures were performed as combined scalenectomy and transaxillary first rib resection, four as two-stage scalenectomy and transaxillary first rib resection and two as simple scalenectomy alone. Follow-up from 6 to 60 (mean 17) months was available for 39 operations. A two-tier assessment method was used to improve the accu…

Focal hand dystonia in a patient with thoracic outlet syndrome.

A patient affected by thoracic outlet syndrome, with an involvement of the left lower primary trunk due to a rudimentary cervical rib, developed a severe hand dystonia on the same side. The dystonic posture was characterised by a flexion of the wrist with the fingers curled into the palm. Polygraphic recordings performed on the left flexor digitorum superficialis (FDS4) and extensor digitorum superficialis (EDC4) muscles, during a repetitive tapping task of the fourth digit, showed a loss of well formed bursts without a clear silent period along with long duration bursts of cocontraction in an…

Why do almost all mammals have seven cervical vertebrae? Developmental constraints, Hox genes, and cancer.

Mammals have seven cervical vertebrae, a number that remains remarkably constant. I propose that the lack of variation is caused by developm … ties in Hox gene expression and congenital anomalies. In these embryonal cancers the incidence of a cervical rib (a rib on the seventh cervical vertebra, a homeotic transformation of a cervical vertebra towards a thoracic-type vertebra) appears to be increased. The minimal estimate of the selection coefficient acting against these mutations is about 12%. In bi …

Sprengel's Deformity: A Paediatric Case Report.

Sprengel's deformity is a conspicuous anomaly, affecting one or both scapulas. The congenital elevation of the scapula is frequently accompanied by additional anomalies, such as rib, vertebral, or muscular deformities, among which are rib fusion or vertebral deformity. Defects in the cervical vertebrae are most likely to result in Klippel-Feil syndrome, which is characterised by a short neck, restrictions on head mobility, and low-growing neck hair. Fewer than half of the instances had scoliosis, which is compensatory due to efforts to straighten the spine. The present case report was the case

[The choice of approach in the surgical therapy of the superior thoracic outlet syndrome].

The symptoms of thoracic outlet syndrome (TOS) may be improved or cured either by physiotherapy or by a surgical operation. The choice of patients to be submitted to surgery must be performed on the basis of clinical picture and of non invasive and invasive assessment. Moreover the surgeon must choose the best procedure to relieve symptoms. The Authors on the basis of their experience and of a literature review refer to the various surgical approaches used in the treatment of TOS.

A dose-related increase in cervical ribs or ossification sites lateral to the seventh cervical vertebra was significant at 2,000 ppm and above. Thus, the NOAEL for the developmental toxicity in this study was 1,000 ppm. A log-logistic dose response model was applied to the incidence data …

Regional anaesthesia for major orthopaedic trauma surgery in patients with conservatively managed pneumothoraces.

The anaesthetic management of multiple traumatic injuries poses numerous challenges. In this report, we present the cases of two patients with polytrauma including pneumothoraces and multiple rib fractures. The first patient, a 39-year-old man, presented with multiple left upper limb fractures, multiple bilateral rib fractures, bilateral pneumothoraces and fractures of multiple facial and cranial bones. The second patient, a 39-year-old woman, presented with right-sided radial and ulnar fractures, a right-sided pelvic fracture, and multiple right-sided rib fractures with an associated pneumoth

Vascular manifestations of the thoracic outlet syndrome. A surgical urgency.

Although the vascular manifestations of the thoracic outlet syndrome are infrequent, their presence is an ominous portent for the affected limb. The cases of two recent patients indicate the importance of prompt recognition, urgent angiography, and definitive surgery. Regarding the surgical procedure, we used a two-incision approach-supraclavicular and intraclavicular-combining scalenotomy, resections of the cervical rib if present, the first thoracic rib, and the subclavian artery with retroclavicular interposition woven Dacron graft reconstruction. Preceding graft replacement, a Fogarty cath…

Association between structural rib autograft and the rate of arthrodesis in children undergoing occiput-C2 instrumentation and fusion.

OBJECTIVE: The purpose of this study was to identify factors associated with fusion success among pediatric patients undergoing occiput-C2 rigid instrumentation and fusion. METHODS: The Pediatric Spine Study Group registry was queried to identify patients ≤ 21 years of age who underwent occiput-C2 posterior spinal rigid instrumentation and fusion and had a 2-year minimum clinical and radiographic (postoperative lateral cervical radiograph or CT scan) follow-up. Fusion failure was defined clinically if a patient underwent hardware revision surgery > 30 days after the index procedure or radiogr

[Anatomy of the lateral cervical region with emphasis on thoracic outlet syndrome].

The anatomy of the lateral cervical region is described and revisited. The three topographical regions where the neurovascular bundle can be easily compressed by surrounding structures are highlighted. Furthermore, the paper focuses on the anatomical variations which are able to produce a thoracic outlet syndrome (TOS).

Historical perspectives and anatomic considerations. Thoracic outlet syndrome.

The major developments in the evolution of understanding the thoracic outlet syndromes is presented in a chronologically historical record beginning with the first mention of a cervical rib by Galen in the second century A.D. Appreciation of the vascular and neurologic types of TOS progressed slowly through the centuries until detailed clinical studies were presented in the early 1900s. Interest in these syndromes increased steadily since then with description of the effectiveness of first rib resection by Murphy in 1910, and scalenotomy without cervical rib resection favored by Adson in 1927.…
