The article presents results of using the extracorporal connection with the donor (porcine) spleen (ECCDS) for the treatment of 14 patients with critical mechanical traumas and their complications. An experience with 30 ECCDS-s has shown them to possess a pronounced detoxicating and immunomodulatory effect followed by a positive clinical result. In 6 cases of ECCDS a temporary chill was noted. There were no other complications.

We present a 67-year-old man with complaints of confusion, chills, night sweats, and several days of poor oral intake. He had severe plasmacytosis on lab work, which initially raised concern for plasma cell leukemia. However, further workup led to the diagnosis of angioimmunoblastic T-cell lymphoma. His initial hospital course was complicated by spontaneous tumor lysis syndrome. Early recognition and prompt interventions are pivotal to improve survival outcomes in such patients.

A 54-year-old man presented with 6 months' history of dry cough and dyspnea on exertion. He also reported intermittent joint pain and orthopnea. He denied fevers, chills, and rashes. His medical history was significant for rheumatoid arthritis, for which he was taking 20 mg of prednisone daily. He had not been receiving adalimumab or methotrexate for several months. Infection is an infrequently reported complication following septoplasty and septorhinoplasty. Among the recognized but rare infections are toxic shock syndrome, spinal osteomyelitis, meningitis, septic cavernous sinus thrombosis and endocarditis. A high index of suspicion is required to diagnose these infections early and thereby minimize morbidity and mortality. We present a case of endocarditis following septoplasty in a patient who had no identifiable preoperative risk factors but who experienced recurrent fever and chills postoperatively.

Noncaseating granulomatous hepatitis may be caused by a variety of drugs, but we have not found, by computer search of the literature, a previous describe of granulomatous hepatitis associated with pyrazinamide. We describe a 52-year-old man with hectic fever, chills, extreme fatigue, liver damage, and hyperuricemia about 4 weeks after commencing pyrazinamide therapy. A liver biopsy specimen showed noncaseating epithelioid granulomas. The patient recovered soon after the interruption of tuberculostatic treatment.

Five instances of urethro-venous intravasation during retrograde urethrography are reported. Four cases were of urethral strictures and one case was of urethral hemangioma. All patients had post procedural bleeding while one patient got allergic reactions, another showed chills and rigors. Anatomy of the drainage veins is described. Factors responsible for this complication and its clinical implications are discussed. It is suggested that urethro-venous intravasation should be considered a diagnostic sign of urethral inflammation.

A girl with Thalassemia major reacted to a transfusion of packed red blood cells with increasing respiratory distress until death 12 1/2 hours later. Chills and fever were followed by dry cough, dyspnea, and pulmonary edema. The recipient had lymphocytotoxic antibodies specific for donor leukocyte antigens HL-A11 and possibly W14. At autopsy, the lungs showed pulmonary edema with extensive nonspecific acute alveolar injury. Similar cases in the literature are reviewed.

This study proposes a method to determine tylosin A in livestock products and tylosin A and B in honey using LC-MS/MS. A case of acute hepato-renal failure which developed after the oral intake of rifampicin is reported. Allergic reaction on the drug was accompanied by chill, weakness, paraesthesia, skin itch and facial swelling. The case described in the article appears to be all the more interesting due to the fact that severe lethal complication has developed in patient who had a history of allergic reactions on rifampicin.
