More active surgical tactics is recommended in acute cholecystitis, which consists in a tendency to solve the question of an emergency operative intervention in the first 24 hours after admittance to the clinic and the performance of postponed operations on the 3rd-5th day. This leads to increase of surgical activity, decrease of the frequency of complicated forms of acute cholecystitis and total and postoperative mortality and postoperative complications, and a shorter period of hospital stay.

The syndrome of abdominal fullness and nausea, diaphoresis, chest pain, and ECG changes long has been associated with impending myocardial infarction. For a few patients, however, a working diagnosis of coronary ischemia is seen to be inaccurate on further testing, including stress testing and cardiac catheterization. Acute cholecystitis may cause a clinical picture similar to that of cardiac ischemia. The ECG changes that may occur in acute cholecystitis, the possible basis for these changes, and their clinical implications are discussed in this article.

The active surgical approach obtains more and more importance in the therapy of acute cholecystitis. On the basis of an own retrospective examination over 10 years (1978 to 1987) with 342 cases of treatment and of the literature the various possibilities of the surgical therapy are described and assessed. On account of the possibilities of complication the treatment of the acute cholecystitis should on principle be performed at a hospital. According to the treatment of acute cholecystitis is differed between the immediate and early operation in the acute phase and the interval operation 2-3 mo…

A total of 418 gallbladders removed surgically as chronic cholecystitis or carcinoma gallbladder were studied retrospectively for the presence of xanthogranulomas which were found in 45 (10.8%). The age of patients with xanthogranulomatous cholecystitis ranged from 15 to 80 yr with a mean of 45.3 indicating an early presentation of the disease in our population. The male to female ratio was 1:8. The gallbladder was invariably thickened and calculi of cholesterol or mixed type were present in all cases. Xanthogranulomatous foci appeared as yellowish nodules 2-15 mm in size, on the mucosal surfa…

A case of the HELLP syndrome in a pregnant woman, which was diagnosed initially as obstructive cholecystitis, is presented. The diagnostic difficulties and some management problems are discussed. Practitioners should be aware of the HELLP syndrome and related conditions when caring for pregnant patients.

Xanthogranulomatous cholecystitis is rare, and information on its radiologic diagnosis is limited. Radiologic findings of seven patients with histologically proven xanthogranulomatous cholecystitis were reviewed retrospectively. Sonography showed a calculus or calculi of the gallbladder with moderate thickening of the wall in all. IV cholangiography was performed in six cases. Findings were nonspecific. Four had nonopacified gallbladder, and one had stenosis of the proximal part of the common bile duct. Marked thickening of the gallbladder wall was seen in six of seven cases on CT. In one case…

The development of peritonitis in hospitalized patients, especially those with significant associated illness, can be a difficult and delayed diagnosis. To ascertain the clinical presentation of acute cholecystitis in this group, a retrospective analysis was performed. Over a 10-year period 18 patients were identified who developed either posttraumatic or postoperative acute cholecystitis. The condition occurred in 12 patients admitted for some form of trauma and in six patients after elective surgery. Fever and right upper quadrant pain and tenderness were present in most. These physical find…

Reported are 10 patients with surgically verified acute cholecystitis and its complications. Of these 10 patients, 6 (60%) had atypical clinical presentation so that cholecystitis was not the primary diagnosis being considered before computed tomography. Common computed tomography findings included gallbladder wall thickening (100%), pericholecystic fluid (80%), gallstones (50%), and air in the gallbladder lumen and wall (20%). An awareness of the computed tomography appearances can be helpful in the diagnosis of complicated cholecystitis even when the clinical presentation is atypical.

Eleven patients were examined by ultrasound before undergoing cholecystectomy (n = 9) or cholecystostomy (n = 2) for acalculous cholecystitis after abdominal surgery. The ultrasound images were analyzed retrospectively and compared with the surgical and histologic findings. The results indicate several established ultrasound criteria of cholecystitis to be less reliable than usual. Although 10 of 11 patients were on parenteral hyperalimentation, gross distention of the gallbladder was observed in only 3. In 4 of 7 patients, in whom pericholecystic fluid was observed, no gallbladder perforation…

The article presents results of treatment of 300 patients with acute cholecystitis, 285 of them (95%) being operated upon. Hepatic insufficiency was found to develop in 173 patients (57,6%). The operation of choice was cholecystectomy (97,1%). Cholecystostomy was fulfilled in 7 patients (2,4%), in 2 patients (0,5%) choledocholithotomy was made following a previous cholecystectomy. Lethality in patients with hepatic insufficiency was 4 times as high as in patients without it. In 34,1% of the operated patients hepatic insufficiency was the main cause of death.

This paper discusses ten patients who developed acute cholecystitis during the postoperative period following surgery which did not involve the gall bladder. All were examined with ultrasound during the first two weeks after surgery and were diagnosed as acute cholecystitis. Six (60%) had complications such as empyema, gangrene, perforation, pericholecystic abscess or localized peritonitis; four (40%) had no evidence of complications. The authors highlight the importance of ultrasound (US) as the technique of choice for a first screening if this entity is suspected because of its speed and hig…

A case report of chronic cholecystitis due to Enterobacter agglomerans, occurred in a 54-year old female with homozygous beta-thalassemia. The patient responded successfully to cholecystectomy and sulfamethoxazole + trimetroprim therapy. The source of the infection was not known, however, cystic duct obstruction and immune deficit were thought to be the predisposing causes.
