The authors report on a patient with postoperative CSF (cerebrospinal fluid) leakage subsequent to transsphenoidal surgery where cisterno SPECT (single photon emission computed tomography) demonstrated the precise location of a CSF fistula. Opioid compounds injected intracisternally in rats induce a centrally-mediated increase in blood pressure. In the present study, the intracisternal injection of an antagonist of opioid compounds, diprenorphine was examined. Diprenorphine caused a similar blood pressure decrease in spontaneously hypertensive rats and normotensive control Wistar Kyoto rats both in young and adult animals. A 6-year-old girl was admitted to our emergency room because of a gunshot wound in the posterior craniocervical junction. On admission, she was alert, but left hemiplegia and right hemiparesis were noted. Cranial CT scanning showed a retained bullet in the cerebellomedullary cistern without bone destruction.
