OBJECTIVE: To investigate changes in tissue oxygenation in the arm and leg during the cold pressor test in humans with spinal cord injury (SCI).
METHODS: Subjects with SCI at cervical 6 (n=7) and subjects with SCI at thoracic 5 or thoracic 6 (n=5) experienced 3-min cold water immersion of the foot and subsequent 10-min recovery. OBJECTIVE: To explore how non-freezing cold nerve injury (NFCNI) is affected by fluctuating ambient temperatures.
METHODS: The sciatic nerves of Wistar rats were exposed to either intermittent cooling and rewarming or continuous cooling. The pathology was assessed neurophysiologically and morphologically.
RESULTS: More severe injury was observed when nerve was intermittently cooled, despite an identical duration and degree of cooling.
CONCLUSION: The observed pathology is a result of repeated reperfusion injury to nerves that are cooled intermittently.

Upper extremity vascular injuries are common in trauma. The mortality rate from these injuries is quite low; however, the morbidity rate is quite significant. Prompt diagnosis and treatment can reduce the amputation rate for these injuries to minimal. Furthermore, morbidity from late complications of chronic ischemia, restenosis, and cold intolerance can be decreased as well. Fasciotomy, although less frequently required than in lower extremity injuries, should be used in all cases of suspected compartment syndrome.

Acetaminophen, a commonly used medication, is present in many over-the-counter remedies. In recent years, its potential to cause severe liver injury has been increasingly appreciated. Chronic abusers of alcohol may be particularly susceptible to hepatotoxicity from acetaminophen. We report two cases of unintentional liver injury associated with ingestion of Nyquil, a liquid cold remedy containing acetaminophen and 25% alcohol.

INTRODUCTION: A common consideration for replantation success is the ischemia time following injury and the preservation temperature. A classic principle within the hand surgery community describes 12 hours of warm ischemia and 24 hours of cold ischemia as the upper limits for digit replantation; however, these limits are largely anecdotal and based on older studies. PURPOSE: IR injury is an unavoidable consequence in deceased donor liver transplantation. Cold preservation and warm reperfusion may change the expression and function of drug transporters in the liver due to vasoconstriction, infiltration of neutrophils and release of cytokines.
