In vitro megakaryocyte colony formation was studied in 25 patients with chronic idiopathic thrombocytopenic purpura (ITP), and compared to 16 controls. A significantly lower number of megakaryocyte progenitors (CFU-MK) was observed in patients (10.8 +/- 12.0 CFU-MK per 10(5) bone-marrow non-adherent mononuclear cells (NAMC), compared to controls (22.5 +/- 15.4 CFU-MK); P < 0.002. Only six patients had a normal number of CFU-MK. The number of CFU-MK was not correlated to initial platelet count (P = 0.35). An outbreak of chronic cryptosporidiosis resulting in hypertrophic gastritis occurred in a captive colony of Australian elapid snakes. Two species of the genus Notechis were involved: Notechis ater (Black Tiger Snake) and Notechis scutatus (Eastern or Mainland Tiger Snake). We report the use of granulocyte-macrophage colony stimulating factor (GM-CSF) in a case of rheumatoid arthritis with sulfasalazine induced agranulocytosis, leading to a rapid bone marrow recovery within 7 days. This case and 2 others reported in the literature emphasize the need for further research into the possible role of GM-CSF in the treatment of drug induced agranulocytosis.

GM-CSF decreases the hematopoietic toxicity of both nonablative and marrow ablative intensive chemotherapy regimens. Data are limited at this time, particularly in regard to optimal schedules and GM-CSF dosing regimens with particular chemotherapy programs. The importance of GM-CSF scheduling in relationship to chemotherapy is becoming more clear. The dose-intensive regimens supported by GM-CSF produce a high complete remission rate in several types of malignancy. Evidence that these remissions will be durable is still lacking. In a retrospective study sex ratio and mortality were analysed in a captive colony of common marmosets (Callithrix jacchus). Seven hundred and thirty-five infants in 294 litters (20 singletons, 119 twins, 140 triplets, 14 quadruplets) out of 57 breeding females were evaluated. The hematopoietic growth factors are under investigation for the treatment of patients with chemotherapy-induced bone marrow suppression. One such trial at the University of California, Los Angeles involves chemotherapy with or without granulocyte colony-stimulating factor (G-CSF) in patients with small cell lung cancer. Eight infants with chronic neutropenia ranging in age from 1 to 13 months were studied for serum granulocyte colony-stimulating factor (G-CSF) levels. Serum G-CSF levels were elevated, especially when peripheral blood absolute neutrophil counts (ANC) were under 500/microliter in seven patients with autoimmune neutropenia. On the other hand, in a patient with congenital agranulocytosis (Kostmann type), G-CSF levels were below the sensitivity of the assay (less than 50 pg/ml) despite severe neutropenia (less than 100/microliter).

A 55-year-old man was admitted to our hospital for the evaluation of neutropenia. On physical examination, he had apthae and splenomegaly. CBC showed 1,000/microliter WBC with 5% neutrophils, and microcytic anemia consistent with iron deficiency. Bone marrow examination revealed a marked decrease of mature neutrophils, but normal percentage of immature myeloid cells. There was no morphological abnormality in the hemopoietic cells. Factors contributing to the development of cytopenias in patients with advanced human immunodeficiency virus (HIV) disease include primary HIV-related suppression of blood cell production, opportunistic infections and neoplasms that directly involve the marrow cavity, and the toxicity of antiviral, antiinfective, and antineoplastic therapy. Indeed, bone marrow toxicity is often the complication limiting delivery of effective therapy in such patients. Recombinant granulocyte-macrophage colony-stimulating factor (GM-CSF) has been shown to increase the leukocyte count in this patient population.

Vitamin E deficiency was found to be the cause of reproductive failure and nutritional myodegeneration in a rabbit breeding colony. The diagnosis was based on the low vitamin E concentration of doe liver (5.73 micrograms/g) associated with myodegeneration of tongue, skeletal muscle, and heart and the absence of other abnormal microbiologic and parasitologic findings. Supplementation of the diet with wheat germ oil returned the does' fertility to normal, and abortion, stillbirth, and neonatal death ceased.
