[Design and implementation of an epidemiological surveillance system for mental health].

OBJECTIVE: To design and test a surveillance system to detect and rank the most significant mental health problems in a community and to establish their association with the living conditions in that community. METHODS: The system was designed and tested at a primary health care center in the community of La Florida, which is in the metropolitan region of Santiago, the capital of Chile. The health problems that were under surveillance, with monthly monitoring, were: addiction (to alcohol, tobacco, and benzodiazepines or other drugs), violence (domestic violence and sexual abuse, real or suspe

Application of Skills Acquired During My Học Mãi Fellowship.

This reflection from a geriatric and palliative care clinician in Vietnam considers how care for older adults can move beyond disease management to support function, quality of life and informed decision-making, informed by a 6-week Học Mai fellowship in Australia. It provided insight into how core geriatric principles are embedded across the continuum of care, from acute to community settings. Key lessons included the role of allied health in multidisciplinary care, the routine assessment of frailty, mobility, cognition, nutrition and sensory function, early integration of advance care planni

Revisiting community-based distribution programs: are they still needed?

Community-based distribution (CBD) programs are the optimum way of reaching people in rural areas of developing countries where conventional methods of delivery do not exist or fail. This paper reviews findings and experiences from over 30 years of efforts to implement CBD of family planning methods around the world. Although research suggests that community-based service delivery can contribute to contraceptive use, the magnitude of impact is often in doubt or its existence is questionable when compared to alternative family planning delivery services. After the review of more than 30 years o

Reproduction, risk and reality: family planning and reproductive health in northern Vietnam.

In collaboration with the National Committee for Population and Family Planning, a study was conducted in 1994 in two Vietnamese communes to provide community level information on women's reproductive health and behaviors. A survey of 504 rural and 523 urban women collected five-year histories of reproduction, contraception, abortion and symptoms of Reproductive Tract Infections (RTI). This analysis focuses on the relationships between women's individual characteristics, use of family planning and abortion, and reported RTI symptoms. The findings reveal that IUDs do not raise women's likelihoo

Addition of mHealth (mobile health) for family planning support in Kenya: disparities in access to mobile phones and associations with contraceptive knowledge and use.

BACKGROUND: Recently mobile health (mHealth) has been implemented in Kenya to support family planning. Our objectives were to investigate disparities in mobile phone ownership and to examine the associations between exposure to family planning messages through mHealth (stand-alone or combined with other channels such as public forums, informational materials, health workers, social media and political/religious/community leaders' advocacy) and contraceptive knowledge and use. METHODS: Logistic and Poisson regression models were used to analyze the 2014 Kenya Demographic and Health Survey. RE

Nurse-managed free clinic fosters care connection for homeless population.

The goal of this article is to demystify the process that healthcare providers must follow when working with homeless patients who sustain injuries or exhibit illnesses that necessitate rehabilitation care. Observations made over a period of more than 12 years at an inner-city medical/psychiatric nurse-managed free clinic that delivers cutting-edge services and educates multidisciplinary students to care for disenfranchised populations led the author to several conclusions: homeless people frequently lose their identity as individuals when facing healthcare providers; previous negative percept

A community-based participatory research study of multifaceted in-home environmental interventions for pediatric asthmatics in public housing.

Pest infestation is a major problem in urban, low-income housing and may contribute to elevated asthma prevalence and exacerbation rates in such communities. However, there is poor understanding of the effectiveness of integrated pest management (IPM) efforts in controlling pediatric asthma, or of the interactions among various interventions and risk factors in these settings. As part of the Boston-based Healthy Public Housing Initiative, we conducted a longitudinal, single-cohort community-based participatory research intervention study. Fifty asthmatic children aged 4-17 from three public ho

Measuring progress in pregnancy planning and preconception health.

As efforts to support pregnancy planning and improve preconception health are increasing at scale, appropriate systems to monitor progress are required. Despite developments in a few countries, no surveillance systems currently in operation are using a comprehensive set of indicators for monitoring preconception health. This Review describes relevant indicators, reflecting both system-level and individual-level factors, that can be drawn from routine data sources to form the basis for developing new surveillance systems. We present a new framework for national and international surveillance th

From emergency adaptation to institutional resilience: a policy and practice review of Somalia's health system, 1991-2026.

Health-system resilience is a major policy concern in fragile and conflict-affected settings, where recurrent shocks intersect with chronic weaknesses in governance, financing, workforce, service delivery, information systems, and supply chains. This policy and practice review synthesizes 51 peer-reviewed and institutional sources published or issued between 1990 and 3 July 2026 to examine how Somalia's health system has responded to three decades of conflict, climatic emergencies, displacement, outbreaks, and humanitarian intervention. Sources were identified through targeted searches of PubM
