Generating Advancements in Longitudinal Analysis in X and Y Variations: Rationale, Methods, and Diagnostic Characteristics for the GALAXY Registry.

Sex chromosome aneuploidies (SCAs) are a family of genetic disorders that result from an atypical number of X and/or Y chromosomes. SCAs are the most common chromosomal abnormality, affecting ~1/400 live births, yet are often underdiagnosed, leading to over-representation of more severely impacted individuals in many clinical studies. In addition to this ascertainment bias, existing work in SCAs has also been limited by low geographic and demographic diversity. To address these limitations, we have created the Generating Advancements with Longitudinal Analysis in X and Y variations (GALAXY) Re

The end of medical confidentiality? Patients, physicians and the state in history.

Medical confidentiality has come under attack in the public sphere. In recent disasters both journalists and politicians have questioned medical confidentiality and claimed that in specific contexts physicians should be compelled to communicate data on their patients' health. The murders of innocent individuals by a suicidal pilot and a Swiss convicted criminal have generated polemical debates on the topic. In this article, historical data on medical confidentiality is used to show that medical practices of secrecy were regularly attacked in the past, and that the nature of medical confidentia

Preserving Fertility in People With Rheumatic Diseases.

Rheumatology patients historically were told they "can't" or "shouldn't" become pregnant. Improved rheumatic diagnostics and treatments have led to decreased morbidity and mortality and increased quality of life resulting in an opportunity to focus on fertility and its preservation. In the same vein as rheumatic disease care, assisted reproductive technology (ART), which includes freezing of egg and sperm as well as in vitro fertilization, has made considerable strides in the recent past. ART is safe for those with rheumatic diseases and has comparable outcomes to the general public, but may t

Enhancing Healthy Longevity: Scoping Review of Practices and Interventions.

BACKGROUND: Over the past 2 decades, concerns have arisen about the distinction between health span and lifespan, highlighting that longevity does not necessarily equate to good health, a concept often referred to as "healthy longevity." While various strategies have been explored to promote healthy aging and achieve healthy longevity, it remains uncertain which practices are most effective. OBJECTIVE: This scoping review identifies the existing and emerging practices and interventions that promote healthy longevity, identifies the key components of these practices and interventions, and cons

Knowledge of Neonatal Danger Signs Among Mothers Attending Vaccination Centers in Central West Bank: A Cross-Sectional Study.

BACKGROUND AND AIMS: One core goal of the 2030 Sustainable Development Goals is embracing good health and well-being among all age groups including the vulnerable neonatal age group. Aside from the contribution of preconception, antenatal, and intrapartum factors, neonatal health is a harmony between the mother's health and neonatal care. In Palestine, specifically in the Central West Bank, there is a scarcity of studies investigating the determinants of neonatal deaths. Through this study, we investigated the level of mothers' knowledge and awareness in recognizing neonatal danger signs (NDS)

Let's Talk About Sex: The Social Determinants of Sexual and Reproductive Health for Second-Year Medical Students.

INTRODUCTION: Sexual health is influenced by a myriad of social factors including health care access, social and cultural norms, insurance status, educational level and health literacy, economic status, sex, gender identity, and sexual orientation and behavior. It is pivotal to educate future physicians about these social determinants so that they can work to mitigate the resulting disparities and thereby improve the health of patients and their communities. METHODS: This 2-hour, large-group session for second-year medical students was first given in the fall of 2017. It included a 1-hour, ca

Strengths and limitations of healthcare transition programs for patients with spina bifida: a systematic review.

BACKGROUND: Due to the complex needs of patients with spina bifida, many pediatric clinics have adopted a multidisciplinary approach, which allows the patient to see multiple specialists in one setting. However, patients with spina bifida require ongoing care throughout their lifetime, and many patients experience interruptions in care when they transition to adulthood; therefore, many centers have also begun to develop programs to streamline the transition process. The goal of this review is to summarize the existing healthcare transition programs for patients with spina bifida that have been

Resource utilization and health care charges associated with the most frequent ED users.

STUDY OBJECTIVE: Emergency department (ED) visits have continued to rise, and frequent ED users account for up to 8% of all ED visits. Reducing visits by frequent ED users may be one way to help reduce health care costs. We hypothesize that frequent users have unique ED utilization patterns resulting in differences in health care charges. METHODS: We conducted a retrospective review of electronic medical records from an urban community teaching hospital for the year 2012 comparing the top 108 frequent ED users (>12 visits/year) to a randomly selected group of 108 nonfrequent users (<4 visits/

Nonlinear modeling of health checkup factors and future claims-based healthcare costs in Japan: a retrospective cohort study.

BACKGROUND: Population health checkups support disease prevention and health management. Clarifying how routinely collected checkup factors relate to subsequent healthcare costs may inform population-level risk assessment and preventive planning. We aimed to characterize nonlinear and sex-specific associations between routine checkup factors and near-term claims-based healthcare costs in Japan and to explore whether composite metabolic indices provide additional information beyond routine measures. METHODS: We linked Japan's Specific Health Checkups (FY2012-FY2022) to subsequent administrativ

Women's Information Needs and Educational Preferences Regarding Lung Cancer Screening.

Background: Physicians are less likely to discuss lung cancer screening (LCS) with women, and women have lower awareness of LCS availability. The objective of this qualitative study was to determine information needs, patient-provider communication barriers, and preferences for LCS education among women. Materials and Methods: Eight semistructured qualitative focus groups were conducted with 28 self-identified women meeting LCS eligibility criteria. Participants were recruited through a large health system, from a community-based LCS program, and through a national online database between Octo

Evaluation of social determinants of health and emotional symptoms among rural family caregivers providing palliative and end-of-life care: An explanatory sequential mixed methods study protocol.

INTRODUCTION: Approximately 53 million family caregivers (FCGs) support and provide home-based care for patients with chronic and life-limiting conditions. Caregiving at the end of life is an emotionally, cognitively and physically demanding role, and FCGs are often unpaid and receive little training. FCGs in remote, rural areas experience adverse social determinants of health (SDOHs), such as limited access to healthcare services and are often financially vulnerable, affecting their healthcare coverage, placing them at higher risk for poor emotional health. Adequate health literacy, a key SDO

Incorporating Systems-Level Stakeholder Perspectives into the Design of Mobile Integrated Health Programs.

OBJECTIVES: Despite early evidence of effectiveness, cost-savings, and resource optimization, mobile integrated health (MIH) programs have not been widely implemented in the United States. System, community, and organizational-level barriers often hinder evidence-based public health interventions, such as MIH programs, from being broadly adopted into real-world clinical practice. The objective of this study is to identify solutions to the barriers impeding the implementation of MIH through interviews with multilevel stakeholders. METHODS: Using the CENTERing multi-level partner voices in Impl

Overcoming the HIV Pre-Exposure Prophylaxis Uptake Barrier With A Health Action Process Approach and Conditional Economic Incentives Among Men Who Have Sex With Men in 3 Chinese Cities: Protocol for a Randomized Controlled Trial.

BACKGROUND: Despite high pre-exposure prophylaxis (PrEP) awareness and willingness among Chinese men who have sex with men (MSM), actual uptake remains critically low. The health action process approach (HAPA) is a 2-phase theory that addresses both the motivational and volitional phases of health behavior change, making it suitable for tackling the intention-behavior gap in PrEP use. In China, where PrEP is not covered by national health insurance, financial barriers further hinder uptake. Group-based conditional economic incentives (GCEIs) represent an innovative strategy to address cost-rel
