Use of Continuous Glucose Monitors by People Without Diabetes: An Idea Whose Time Has Come?

BACKGROUND: Continuous glucose monitor (CGM) systems were originally intended only for people with diabetes. Recently, there has been interest in monitoring glucose concentrations in a variety of other situations. As data accumulate to support the use of CGM systems in additional states unrelated to diabetes, the use of CGM systems is likely to increase accordingly. METHODS: PubMed and Google Scholar were searched for articles about the use of CGM in individuals without diabetes. Relevant articles that included sufficient details were queried to identify what cohorts of individuals were adopt

Advanced Technology in the Management of Diabetes: Which Comes First-Continuous Glucose Monitor or Insulin Pump?

PURPOSE OF REVIEW: In this article, we consider advanced technologies for the management of diabetes. RECENT FINDINGS: Specifically, we pose the question of which should come first: an insulin pump (CSII) or a continuous glucose monitor (CGM)? Historical perspective on both insulin delivery and glucose measurement is provided. Recently published clinical trials are reviewed. Practical issues including quality of life, patient education, and out-of-pocket cost are discussed. Based on available evidence and clinical experience, we favor CGM as a first-line technology recommendation for the trea

A standard approach to continuous glucose monitor data in pregnancy for the study of fetal growth and infant outcomes.

The power of continuous glucose monitoring system (CGMS) technology to profile glycemic patterns throughout a 24-h period has benefited the care of individuals with diabetes mellitus for over 10 years. Recently, this technology has been utilized to better understand glucose patterns in pregnancy, especially as they relate to abnormal fetal growth given that adiposity at birth is associated with increased risks for childhood obesity and metabolic syndrome. However, the lack of a standardized approach to defining glucose measures associated with maternal outcomes and fetal growth has greatly lim…

Optimizing Continuous Glucose Monitoring for Older Adults with Diabetes: From Clinical Evidence to a Practical Implementation Pathway.

Older adults represent a growing proportion of people with diabetes and often experience hypoglycemia, glycemic variability (GV), multimorbidity, frailty, and treatment-related vulnerabilities that are not fully captured by glycated hemoglobin A1c (HbA1c) alone. Continuous glucose monitoring (CGM) provides time-resolved glucose profiles, trend information, alerts, and remote data sharing, making it well suited to safety-focused geriatric diabetes care.In this narrative review, we synthesize evidence on CGM features, randomized and real-world outcomes, patient and caregiver experience, barriers…

Continuous Glucose Monitoring Before and After Simultaneous Pancreas-Kidney Transplantation: Insights from a Real-World Clinical Setting.

INTRODUCTION: Simultaneous pancreas-kidney (SPK) transplantation normalizes glycemia in patients with diabetes and end-stage kidney disease, yet data on continuous glucose monitoring (CGM) remain scarce. METHODS: We retrospectively analyzed CGM metrics in SPK recipients using FreeStyle Libre 2®, comparing pre- and post-transplant profiles. RESULTS: Among n = 19 recipients, only n = 14 continued CGM use after transplantation (n = 9 continuously during the entire study period). Within 1 month, all CGM metrics improved significantly, including increased time-in-range (54.21 vs. 83.85%) and time…

A Novel Method to Evaluate Insulin Sensitivity by Combining Continuous Glucose Monitoring and Urinary C-Peptide Excretion.

AIM: Assessing insulin sensitivity is a central component of clinical research in metabolism. Although considered the gold standard, the euglycemic-hyperinsulinemic clamp evaluates insulin sensitivity under non-physiological conditions and is highly burdensome and costly, thereby prompting the development of alternative methods. The 24-h urinary C-peptide excretion rate adjusted for energy intake is a surrogate marker of insulin sensitivity in non-diabetic adults. We hypothesized that the combination of 24-h continuous glucose monitoring (CGM) and 24-h urinary C-peptide excretion (24 h-glycemi…

Real-Time Psychological Factors Using Ecological Momentary Assessment With Continuous Glucose Monitoring in Diabetes Mellitus: Scoping Review.

BACKGROUND: Ecological momentary assessment (EMA) is a tool that captures emotional states, experiences, and behaviors in real or near-real time. Concurrent EMA and continuous glucose monitoring (CGM) data can provide an in-depth understanding of the impacts of psychosocial factors on momentary glucose levels. However, study methodology varies widely for EMA and CGM data collection and analysis. OBJECTIVE: This scoping review aims to summarize the objectives, methodologies, and outcomes of studies analyzing concurrent biopsychosocial EMA and CGM data in diabetes. METHODS: This study was cond…

Associations of Circadian-Disruptive Behaviours With Plasma Glycemic Parameters and Continuous Glucose Monitoring Measures Among Pregnant Women.

AIMS: Circadian-disruptive behaviours (CDBs) may impair glucose regulation via central and peripheral clocks. This study investigated the association between CDB burden and glycemic outcomes during pregnancy. MATERIALS AND METHODS: In this prospective cohort study, 246 pregnant women were recruited at 18-24 weeks' gestation. Four CDBs were assessed using dietary records and questionnaires: eating jetlag, predominant night-eating, social jetlag, and electronic media use before bedtime. Outcomes included oral-glucose-tolerance-test (OGTT) and insulin parameters (24-28 weeks' gestation) and cont

Continuous Glucose Monitoring and Hypoglycaemia Risk During Paediatric DKA Treatment in the PICU: A Retrospective Cohort Study.

BACKGROUND: Hypoglycaemia is a frequent complication during diabetic ketoacidosis (DKA) treatment. Continuous glucose monitoring (CGM) provides real-time glucose trend information; however, its clinical utility during paediatric DKA management in the intensive care unit (ICU) remains uncertain. We investigated whether CGM insertion during DKA treatment in the paediatric ICU (PICU) is associated with reduced hypoglycaemia and a lower burden of point-of-care (POC) glucose measurements. MATERIALS AND METHODS: We conducted a retrospective single-centre cohort study of paediatric patients (< 20 ye…
