Depression: an exploratory parallel-group randomised controlled trial of Antenatal guided self help for WomeN (DAWN): study protocol for a randomised controlled trial.

BACKGROUND: Depression is a common antenatal mental disorder and is associated with an increased risk of adverse effects on the fetus and significant morbidity for the mother; if untreated it can also continue into the post-natal period and affect mother-infant interactions. There has been little research evaluating the effectiveness or cost-effectiveness of antenatal psychological interventions for antenatal depression, particularly for mild to moderate disorders. International guidelines recommend a stepped care approach starting with Guided Self Help, and the aim of this exploratory trial i

Helicobacter pylori in patients with intestinal metaplasia and in controls: a serological and biopsy study in four UK centres. UK Sub-Group of the ECP-EURONUT-Intestinal Metaplasia Study Group.

The incidence of Helicobacter pylori was compared in high and low risk gastric cancer areas. Complete data were available on 116 age and sex matched sets of cases--patients with histologically proven intestinal metaplasia (IM)--and two control groups, one non-IM endoscoped controls (EC) and one non-endoscoped controls (NC) recruited from out-patient clinics. H. pylori status was determined by both serology and histology in individuals biopsied from three separate gastric sites. Our data show an increased incidence of H. pylori and intestinal metaplasia in areas at high risk for gastric cancer,…

A controlled trial of fluconazole or amphotericin B to prevent relapse of cryptococcal meningitis in patients with the acquired immunodeficiency syndrome. The NIAID AIDS Clinical Trials Group and Mycoses Study Group.

BACKGROUND: After primary treatment for cryptococcal meningitis, patients with the acquired immunodeficiency syndrome (AIDS) require some form of continued suppressive therapy to prevent relapse. METHODS: We conducted a multicenter, randomized trial that compared fluconazole (200 mg per day given orally) with amphotericin B (1 mg per kilogram of body weight per week given intravenously) in patients with AIDS who had completed primary therapy for cryptococcal meningitis with amphotericin B (greater than or equal to 15 mg per kilogram). To be eligible, patients had to have at least two negative

Study protocol for a multi-center RCT testing a group-based parenting intervention tailored to mothers with borderline personality disorder against a waiting control group (ProChild*-SP1).

BACKGROUND/AIMS: Borderline personality disorder (BPD) is a severe mental disorder characterized by an unstable sense of self, intense and rapidly changing affect, as well as impulsive and self-destructive behaviors. Interpersonal relationships of individuals with BPD are characterized by marked instability, a lack of dependability, and quick changes between love and hate. For children of individuals with BPD, this can lead to permanent stress and attachment insecurity and an increased risk of adverse physical and mental health development. To reduce dysfunctional parenting and improve positiv

A community based study on haemoglobinopathies and G6PD deficiency among particularly vulnerable tribal groups in hard-to-reach malaria endemic areas of Odisha, India: implications on malaria control.

BACKGROUND: Haemoglobinopathies and G6PD deficiency are inherited disorders found mostly in malaria-endemic areas among different tribal groups of India. However, epidemiological data specific to Particularly Vulnerable Tribal Groups (PVTGs), important for planning and implementing malaria programmes, is limited. Therefore, the present community-based study aimed to assess the prevalence of haemoglobinopathies and G6PD deficiency among the 13 PVTGs found in the state of Odisha, reporting the maximum malaria cases in the country. METHODS: This cross-sectional study was conducted from July 2018

[Modification of pertussis vaccination schedule in Chile, immunization of special groups and control strategies: Commentary from the Consultive Committee of Immunizations of The Chilean Society of Infectious Diseases].

In Chile, an increased number of notifications of cases of whooping cough was detected at the beginning of October 2010, and maintained through 2012. Accumulated cases during 2011 were 2,581 (15.0 per 100,000), which is greater than the number of cases registered during the period 2008-2010 (2,460 cases). On the other hand, the local sanitary authority introduced a modification of pertussis vaccination schedule (starting 2012), which consists in the replacement of the second booster of pertussis vaccine (DTwP, administered to 4-year-old children) as well as diphtheria-tetanus toxoid (dT, admin

A Highly Powered Large Outcomes Randomised Controlled Trial Will Usually, but Not Always, Have Balanced Groups at Baseline.

OBJECTIVE: The importance of randomised trials for discerning an intervention's effect rests on randomisation's ability to create groups with (on average) equal hazards of future outcomes. The frequency with which this does not occur in well-designed large trials with a high event rate was studied. RESEARCH DESIGN AND METHODS: Data from 8215 participants in the Outcomes Reduction with an Initial Glargine Intervention (ORIGIN) trial's biomarker study were used to simulate multiple trials. Participants were repeatedly randomised to two groups. After each randomisation, the hazard of death in pe

Effectiveness of Multidisciplinary Rehabilitation in Chronic Low Back Pain: A Pragmatic Study With a Waiting List Control Group.

INTRODUCTION: Small-scale care providers often encounter challenges in demonstrating the effectiveness of treatments through randomised controlled trials (RCTs) because of the high costs and organisational demands of RCTs. The current study examines the feasibility of evaluating a multidisciplinary treatment for chronic low back pain using a waiting list as a control group. MATERIAL AND METHODS: Of 344 chronic pain patients referred to a Dutch outpatient rehabilitation clinic, 123 received treatment, while 88 were on a waiting list. The main effect parameters were pain (visual analogue scale)

A Randomized Controlled Trial of a Telehealth Group Intervention to Reduce Perinatal Depressive Symptoms: A Mixed Methods Analysis.

INTRODUCTION: Perinatal depression affects approximately 20% of childbearing individuals and is associated with adverse perinatal outcomes. Nonpharmacological therapies are effective for mild to moderate depression, but multiple access barriers exist, including financial constraints and the inconvenience of in-person appointments. Remote access (ie, telehealth) to services is a promising option, but few studies have evaluated the effectiveness of this approach. The objective of this study was to use a mixed methods approach in a randomized controlled trial to evaluate the effectiveness of a gr

The impact of peer support groups on the psychological well-being of patients with rheumatoid arthritis: a randomized controlled trial.

Patients with rheumatoid arthritis have higher rates of mental health problems and lower levels of psychological well-being compared to the general population. This study examines the impact of these factors on the psychological well-being of patients with rheumatoid arthritis. In the trial conducted in 2024, a total of 78 patients were enrolled. They first completed a demographic characteristics form and Ryff’s Psychological Well-being Scale. Subsequently, the patients were randomly assigned to either the intervention group or the control group. The control group received routine care, while

Baricitinib in early polymyalgia rheumatica (BACHELOR): a randomised, double-blind, placebo-controlled, parallel-group trial.

BACKGROUND: Moderate doses of glucocorticoids result in improvements in nearly all patients with polymyalgia rheumatica, but related adverse events are common in older individuals. We aimed to evaluate whether treatment with baricitinib (a Janus kinase 1/2 inhibitor) results in disease control without the use of oral glucocorticoids in people with recent-onset polymyalgia rheumatica. METHODS: We conducted a randomised, double-blind, placebo-controlled, parallel-group trial at six expert centres in France. Participants with recent (<6 months) polymyalgia rheumatica naive to glucocorticoids and
