Electroclinical signs of benign neonatal familial convulsions.

Benign neonatal familial convulsions comprise a distinct epileptic syndrome with an autosomal mode of transmission. The electroclinical signs of seizures in this syndrome are not yet well defined. In 3 children from two families presenting with benign neonatal familial convulsions, 14 seizures were recorded during electroencephalographic (EEG)-video sessions. All seizures occurred during sleep, after a short arousal reaction. Seizures started with bilateral, symmetrical flattening of the EEG for 5 to 19 seconds; simultaneously there was apnea and tonic motor activity. The EEG flattening was fo

Convulsions with malaria: febrile or indicative of cerebral involvement?

Evaluation of 446 infants and young children (6 months to 5 years olds) with malaria parasitaemia showed a significant relationship (P < 0.05- < 0.001) (a) between coma and age, pattern of convulsions, haematocrit, and blood glucose, and (b) between the severity of parasitaemia and risk of convulsions, prevalence of hepatosplenomegaly, and severe anaemia. No significant relationship was observed between convulsions and temperature or haematocrit. Comatose children were older and had a higher prevalence of repeated convulsions, severe anaemia, and hypoglycaemia than non-comatose children. Convu

[Retrospective study of 160 children with febrile convulsions].

In a retrospective study of 411 children with cerebral convulsions over a period of 4 years, 160 patients with febrile seizures were found. This group consisted of 94 boys and 66 girls. The main purpose of this study was to establish the age of the first convulsive fit in each child. Febrile convulsions started in the first half year, increased in the second half year and culminated in the second year of life. This age dependent appearance was explained with passive immunization by maternal antibodies so that febrile convulsions appear when these antibodies decrease. The first occurrence of fe

[Intermittent prophylaxis in febrile convulsions with oral diazepam].

Intermittent prophylaxis with oral diazepam is presented as an optional treatment for febrile seizures. This proposition is justified by the severe side effects of the currently used chronic anticonvulsant drug therapy in febrile seizures (phenobarbital and valproate). Nineteen patients aged between 3 months and 5 years were treated. They had either simple or complex febrile seizures. Sixteen patients had at least one prognostic factor for recurrence of febrile seizures: first febrile seizure before 15 months of age, positive family history for epilepsy or febrile seizures, occurrence of a com

Recurrent seizures in children with Shigella-associated convulsions.

Fifty-five children with Shigella-associated convulsions were followed prospectively to investigate their risk of subsequent febrile or nonfebrile seizures. The duration of the follow-up period was between 6.9 and 14.1 years (9.7 +/- 3.1 years). No case of nonfebrile seizures and only 2 cases (3%) of subsequent febrile seizures were observed during this period. We conclude that although febrile and Shigella-associated convulsions share many clinical features, the natural history of these two conditions seems to be distinctly different. Shigella-related convulsions are not associated with an in

Lidocaine treatment of neonatal convulsions, a therapeutic dilemma.

Three infants with neonatal convulsions were given lidocaine infusions for three days, three weeks and three months, respectively, and the plasma concentrations of lidocaine and its metabolites were analyzed by HPLC. After a prolonged infusion there was considerable accumulation of the metabolites. This may account for the difficulty of stopping the infusion without relapse of the seizures.

Effect of fever on recurrence rate of febrile convulsions.

We studied 154 children admitted consecutively with their first febrile convulsion to assess the influence of the temperature on the recurrence rate of convulsions. Those with temperatures of 40 degrees C or more were nine times less likely to have subsequent convulsions than those with temperatures of 38-38.9 degrees C.

A twin study of febrile convulsions in the general population.

Seven monozygotic (MZ) and six dizygotic (DZ) twin pairs with febrile convulsions (FC) in the general population were studied. The pairwise concordance rate for FC in MZ 85.7% (6/7) was higher than that in DZ 16.7% (1/6). In a discordant MZ pair, the unaffected co-twin was attacked by epileptic seizures later. Between the concordant DZ twins, the clinical symptoms and EEGs differed in quality. According to the ratio of concordance rate in MZ to that in DZ 5.1, a multifactorial mode of inheritance for FC was suspected.

Frequent febrile episodes and recurrent febrile convulsions.

The relationship between the number of febrile episodes and recurrent febrile convulsions was studied prospectively in 289 children after their first febrile seizure. They were randomized to either short-term diazepam prophylaxis (n = 152) or to no prophylaxis (n = 137), and followed for 18 months. Among untreated children with many subsequent febrile episodes (greater than or equal to 4 per year) 29 of 37 (78%) had a recurrence vs. 17 of 100 (17%) with only few (less than 4 per year) feverish illnesses. The former group had a 4:1 chance of developing further febrile fits, compared with a 1:4

Can drug effects on anxiety and convulsions be separated?

The effects of benzodiazepines, barbiturates, a series of novel putative anxiolytic compounds and anxiogenic compounds are reviewed in animal tests of anxiety and on experimentally-induced seizures. It is clear from the data that drug effects on anxiety and convulsions are not always in the same direction; certain compounds are apparently both anxiolytic and proconvulsant, others are anxiogenic and anticonvulsant, others have varied effects depending on the test situation. It is suggested that this work necessitates considerable revision of our traditional concepts of an "anticonvulsant." The

[Phenobarbital in febrile convulsions of children (author's transl)].

Phenobarbital (Luminal) was given to 41 children (aged 11 months to 4 1/2 years) with uncomplicated febrile convulsions. The drug was administered orally or intramuscularly at an initial dose of 10 mg/kg or 15 mg/kg, respectively. Twelve hours later and then every 24 hours until the fever had subsided the dosage was 5 mg/kg orally or intramuscularly. The effective serum phenobarbital level of 15 micrograms/ml was reached with an oral dose of 10 and 15 mg/kg after 25 hours or 90 minutes, respectively, with an intramuscular injection at the same dose after 12 hours and 40 minutes, respectively.
