A Rare Case of a Newborn with Blocked RhD Antigen Caused by Low-Titer Antibody.

BACKGROUND: Hemolytic disease of the fetus and newborn (HDFN) is a condition characterized by severe hemolysis and anemia in fetuses or neonates. To our knowledge, this is the first reported case of a newborn with blocked RhD antigen caused by low-titer antibody. METHODS: Peripheral blood samples were collected from the newborn upon admission. ABO and RhD blood typing was performed using a microcolumn gel agglutination test. Direct antiglobulin test (DAT) was performed using polyspecific anti-human globulin. Indirect antiglobulin test (IAT) was used to screen for irregular antibodies in the s

Favorable outcome without corticosteroids during post-artesunate delayed hemolysis with positive direct antiglobulin test in severe imported Plasmodium falciparum malaria, France.

OBJECTIVES: Positive direct antiglobulin tests (DATs) have been reported in cases of post-artesunate delayed hemolysis (PADH), but the causal role of auto-immune hemolysis remains unclear. We aimed to analyze a cohort of patients with PADH and DAT during severe malaria. METHODS: We describe PADH and DAT results in a 7-year multi-center retrospective cohort of patients receiving artesunate for severe imported malaria. RESULTS: Of 337 patients treated with artesunate, 46 (13.6%) had at least one DAT result within 30 days of treatment initiation, and 25/46 (54.3%) had at least one positive DAT.

Patient ABO blood type is a major predictor of a positive DAT following a transfusion reaction.

BACKGROUND: A direct antiglobulin test (DAT) checks for antibody or complement on the surface of RBCs and is often done following a transfusion reaction. While passive anti-A and anti-B antibodies are known to cause positive DATs, the extent this occurs following transfusion is unknown. STUDY DESIGN AND METHODS: DAT results, ABO type, eluate information, and blood product information were recorded on 1097 transfusion reactions at a large academic hospital over 8 years. The effect of patient blood type, product type, and plasma compatibility of blood product transfused on DAT results were dete

[Analysis of Screening Results in 501 Newborns with Hemolytic Disease].

OBJECTIVE: To explore the relationship between the serological detection of neonatal hemolytic disease (HDN) and related factors, and to observe the detection rate and specificity of the antibodies against the blood group in the newborn hemolytic disease. METHODS: Maternal-neonatal blood type was detected firstly, and then the direct antiglobulin test(DAT), the free antibody test and the antibody release test were used to detect the occurrence of HDN; For those suspected hemolytic disease except ABO or direct DAT result over 2+, the indirect antiglobulin test with irregular antibody were used

A case series of pediatric patients with direct antiglobulin test negative autoimmune hemolytic anemia.

BACKGROUND: The diagnosis of autoimmune hemolytic anemia (AIHA) can be challenging since the direct antiglobulin test (DAT) has been reported to be falsely negative in 3%-11% of cases. In children with anemia, laboratory and/or clinical evidence of hemolysis and a negative DAT, clinicians should consider further specialized testing to confirm AIHA to accurately diagnose and treat this uncommon pediatric entity. STUDY DESIGN AND METHODS: A retrospective chart review was undertaken at a large tertiary care academic pediatric hematology practice to describe our experience with DAT-negative AIHA.

Indirect antiglobulin test-crossmatch using low-ionic-strength saline-albumin enhancement medium and reduced incubation time: effectiveness in the detection of most clinically significant antibodies and impact on blood utilization.

Indirect antiglobulin test-crossmatch (IAT-XM) using enhancement media such as low-ionic-strength saline (LISS) and polyethylene glycol (PEG) usually requires 15 minutes of incubation. These methods are necessary when testing samples from blood recipients who have a higher risk of alloimmunization. In emergency situations, IAT-XM can be time-consuming and can influence presurgery routine, resulting in more red blood cell (RBC) units being tested and stored to avoid the transfusion of uncrossmatched ones. The objective of this study was to evaluate the performance of a LISS-albumin enhancer to

Warm-reactive autoantibodies in pediatric patients: clinical and serologic correlations.

The significance of warm-reactive autoantibodies in pediatric patients has not been a subject of thorough evaluation. This study was undertaken to correlate the clinical and serologic features of these antibodies to identify predictors of clinical significance. Forty-two consecutive patients with serologically detectable warm-reactive autoantibodies were studied. These patients (21 male, 21 female) had a mean age of 9 years (range: 2 mo to 21 y). Primary diagnoses included autoimmune disorders (14), sickle cell disease (14), viral infection (4), idiopathic autoimmune hemolytic anemia (2), leuk

Evaluation of a modified IAT-gel with polyethylenglycol (PEG) addition method for red blood cell antibody identification.

We aimed to evaluate whether a PEG-IAT method, modified for gel technique, could replace the two-stage enzyme method for routine identification of red blood cell (RBC) antibodies.A total of 161 selected patient samples were analyzed with the two-stage enzyme tube technique and the PEG-IAT in gel techniques. In 65 samples, identical antibody results were obtained by the two methods. An additional 28 antibodies were identified by either the PEG-IAT gel method or the enzyme method. Twenty antibodies were identified by the PEG-IAT gel method only (12 Rh antibodies, four anti-Kell, two anti-Fya, tw

Comparison of a modified manual hexadimethrine bromide (Polybrene) and a low-ionic-strength solution antibody detection technique.

Manual hexadimethrine bromide (Polybrene) tests (Polybrene in low-ionic medium) were used in parallel with manual low-ionic-strength solution (LISS) procedures for the routine testing of patient samples referred to a general hospital blood bank. Of 5646 consecutive sera tested, 5167 (91.5%) did not react with either technique; 320 sera (5.7%) reacted in both methods. The Polybrene technique detected 63 antibodies which did not react in the LISS methods. One hundred sera did not react in the Polybrene test, but did react in the LISS methods. Sera showing discrepant results between the two metho

Autoimmune neutropenia in Hodgkin's disease.

A patient was profoundly neutropenic at the time of diagnosis of stage IIIB Hodgkin's disease. The neutropenia was not due to infection or bone marrow involvement by tumor. It did not respond to discontinuation of medication or to splenectomy, done for pathologic staging of Hodgkin's disease. The patient's serum contained abnormally increased granulocyte-binding antibody, which reacted with his own cells. The neutropenia resolved with high-dose prednisone therapy, and has not recurred after chemotherapy. Thus, immune neutropenia--as well as autoimmune hemolytic anemia and immune thrombocytopen

[Aplastic crisis due to parvovirus B19 and Epstein-Barr virus in a patient with hereditary spherocytosis].

Anemic syndrome in childhood requires a diagnosis and urgent treatment guided by systematic protocols that can avoid unnecessary additional testing. The case of a 4 year-old girl with fatigue and intermittent fever of 7 days duration, accompanied by abdominal pain is presented. She had regular general health status, with mucocutaneous jaundice, a grade III/VI/iv murmur, and painful abdomen with hepatosplenomegaly. The blood analysis showed a hypo-regenerative anemia with increased LDH and indirect bilirubin. The Coombs Test was negative, with spherocytes being observed in the peripheral blood …

Immunologic aspects of leprosy with special reference to the circulating antispermatozoal antibodies.

Macroscopic sperm agglutination in gelatin, sperm immobilization and tanned red cell hemagglutination tests could detect antispermatozoal antibodies respectively in 41%, 37%, and 23% sera of 35 leprosy patients, including 5 female cases. Interestingly, all of the above tests were positive in one serum from a female patient with borderline leprosy. Sperm antibodies were detected in both lepromatous and tuberculoid forms of leprosy by the above three technics and no significant difference was observed in their incidences among the two groups of patients. A three dimensional correlation was obser

A case of systemic lupus erythematosus having concurrent Evans syndrome and acquired thrombotic thrombocytopenic purpura.

An 18-year-old Japanese woman with systemic lupus erythematosus experienced dyspnoea, headache, tinnitus, and purpura for 2 weeks and was admitted to our hospital. The patient had been diagnosed with systemic lupus erythematosus and secondary immune thrombocytopenia 8 years before and treated with high-dose prednisolone and mycophenolate mofetil. Since the blood test on admission showed haemolytic anaemia with a positive direct Coombs test and anti-glycoprotein IIb/IIIa antibodies, the patient was initially diagnosed with Evans syndrome (ES). The patient was treated with pulse intravenous meth…
