Management of thyrotoxicosis and pregnancy: Review of the current literature and an update of the care pathway.

Pregnancy can be complicated by hyperthyroidism or thyrotoxicosis. Diagnosis is founded on an increase in free thyroid hormones and low TSH. The most frequent etiologies are Graves' disease, an autoimmune disease linked to stimulatory anti-TSH receptor antibodies, and non-autoimmune gestational hyperthyroidism linked to the TSH-like activity of the chorionic growth hormone (hCG). During pregnancy, thyrotoxicosis can entail maternal, obstetrical and fetal or neonatal complications. Graves' hyperthyroidism may be responsible for fetal and neonatal hyperthyroidism due to placental transfer of sti

Impact of a clinical pathway on cardiovascular risk in patients with diabetes.

OBJECTIVES: Clinical pathways (CPs) are interventions that target the way clinical practice guidelines are applied. They can be implemented in different diseases, including diabetes. In this study we evaluated the impact of the implementation of a CP in the control of cardiovascular risk factors and the occurrence of new events in patients with type 2 diabetes. METHODS: A pre- and post-intervention population-based study in a Spanish region, conducted in 2014-2016. Variables before and after the intervention were: screening; good control of diabetes, dyslipidemia and hypertension; hypoglycemi

Lack of Association Between Perioperative Medication and Postoperative Delirium in Hip Fracture Patients in an Orthogeriatric Care Pathway.

OBJECTIVES: Units for perioperative geriatric care are playing a growing role in the care of older patients after hip fracture surgery. Postoperative delirium is one of the most common complications after hip fracture, but no study has assessed the impact of therapeutics received during a dedicated orthogeriatric care pathway on its incidence. Our main objective was to assess the association between drugs used in emergency, operating, and recovery departments and postoperative delirium during the acute stay. DESIGN: Retrospective cohort study. SETTING AND PARTICIPANTS: All patients ≥70 years

Diagnosis and management of shock in the emergency department.

Shock is a state of acute circulatory failure leading to decreased organ perfusion, with inadequate delivery of oxygenated blood to tissues and resultant end-organ dysfunction. The mechanisms that can result in shock are divided into 4 categories: (1) hypovolemic, (2) distributive, (3) cardiogenic, and (4) obstructive. While much is known regarding treatment of patients in shock, several controversies continue in the literature. Assessment begins with identifying the need for critical interventions such as intubation, mechanical ventilation, or obtaining vascular access. Prompt workup should b

Screening and diagnosis of primary aldosteronism.

Primary aldosteronism (PA) is the commonest form of secondary hypertension in South Africa with an approximate prevalence of 7.5% in the primary-care setting. Hypokalaemia is a poor screening test with 70% of proven cases having normal serum potassium levels. The aldosterone/rennin ratio is a robust screening test with a high sensitivity and specificity, but in South Africa this should be combined with an absolute aldosterone level because of the high incidence of low-renin hypertension in Blacks. The limitations of the ratio, especially with regard to concomitant drug therapy will be discusse

Radiofrequency ablation for accessory pathways: success and variants.

INTRODUCTION: Radiofrequency ablation (RFA) is currently being advocated as the first line of therapy for symptomatic and drug refractory arrhythmias mediated by accessory pathways (APs). However, a large database on RFA for APs from India is still lacking. METHODS: Four hundred thirty-four patients (298 males, (69%)), with APs who underwent RF ablation between January, 1998 and May, 2001 were included. The success and variants were evaluated retrospectively from the data. RESULTS: The mean age was 36 +/- 11 years. There were 314 (73%) patients with WPW syndrome while the other 120 (27%) had

[Dyspnea as a presenting symptom in the emergency department - From triage through rational diagnosis to evidence-based patient pathways].

Accounting for 4-8% of all patients in emergency departments, dyspnea is one of the most common presenting symptoms 11 22. Its clinical significance is reflected in the high rate of admission to the intensive care unit (16-18%) 11 and an in-hospital mortality rate of approximately 9% 22. The concurrent presence of multiple organ pathologies often poses a diagnostic challenge 33. This article translates the current evidence from 2025/2026 into action-oriented strategies: from pathophysiology and AI-supported triage to critical airway management and the discussion of specialized dyspnea units 44

Optimising the breast radiotherapy planning pathway: a quality improvement project at a regional cancer centre.

At the South West Wales Cancer Centre, breast radiotherapy accounts for 24% of the radiotherapy workload. In 2021, only 16% of patients started treatment within 14 days compared with the Welsh time-to-radiotherapy quality metric target of 80%. Delays risk poorer outcomes for patients and create inefficiencies as tasks are often batched and staff alternate between idle and peak workload periods.Earlier analysis showed that only 1%-2% of the overall breast radiotherapy pathway time was value-adding activity. Key delays occurred at several stages of the pathway, where the total duration of each s

Economic impact of TAVI pathway optimisation: the experience of the University Hospital of Parma.

BACKGROUND: Transcatheter Aortic Valve Implantation (TAVI) has emerged as a standard of care for patients suffering from symptomatic severe aortic stenosis. However, organisational analyses and investigations of this intervention's economic impact are lacking. This study aims to assess the potential impact of implementing a process designed to optimise the TAVI pathway regarding organisational, human, material, and economic resources. METHODS: The research is conducted through a case study. The methodology consisted of three stages: (1) mapping of the current TAVI pathway; (2) identification

The MyPath Pain Pathway: Developing a Digital Patient-Centred Care Pathway for Cancer Pain.

CONTEXT: Cancer pain affects up to 45% of patients, with more than 30% experiencing moderate to severe pain. Despite established guidelines, over 40% of patients receive inadequate pain management, severely impacting their quality of life. While effective pain assessment is essential, a standardized approach is lacking in both research and clinical practice. OBJECTIVES: MyPath, a European Union-funded project, aims to implement a patient-centered care (PCC) approach across nine European cancer centers by systematically assessing and managing common symptoms and psychosocial issues to improve

Effect of combined clinical pathway and scenario simulation teaching on psychological stress and anxiety among tuberculosis interns: A retrospective cohort study.

In the realm of medical education, clinical internship represents a pivotal phase where theoretical knowledge must be practically applied amidst real-world patient care, often leading to heightened stress and anxiety among interns. This transitional challenge is particularly acute for tuberculosis (TB) clinical interns, requiring innovative teaching methodologies. Therefore, we aim to evaluate the efficacy of integrating clinical pathway instruction with scenario-based simulation teaching in alleviating psychological stress and anxiety among TB clinical trainees. Data extraction from electroni

STIMULATE-ICP-Delphi (Symptoms, Trajectory, Inequalities and Management: Understanding Long-COVID to Address and Transform Existing Integrated Care Pathways Delphi): Study protocol.

INTRODUCTION: As mortality rates from COVID-19 disease fall, the high prevalence of long-term sequelae (Long COVID) is becoming increasingly widespread, challenging healthcare systems globally. Traditional pathways of care for Long Term Conditions (LTCs) have tended to be managed by disease-specific specialties, an approach that has been ineffective in delivering care for patients with multi-morbidity. The multi-system nature of Long COVID and its impact on physical and psychological health demands a more effective model of holistic, integrated care. The evolution of integrated care systems (I
