Pitfalls in the management of monaural deafness.

OBJECTIVE: We report a patient who underwent cochlear implantation in an ear with long-term deafness, after an acoustic neuroma had been removed surgically from the other, hitherto good ear and the cochlear nerve had subsequently been resected to relieve severe tinnitus. METHOD: Case report. RESULTS: The patient could not tolerate the cochlear implant, because of a moderate headache due to the stimulation level necessary for environmental sound discrimination. CONCLUSION: Cochlear implantation in patients with long-term deafness should be considered carefully, even if deafness is monaural.

Cochlear implantation in deaf infants.

OBJECTIVES: With the application of universal newborn hearing screening programs, a large pool of newly identified deaf infants has been identified. The benefits of early intervention with cochlear implants (CI) is being explored. Mounting evidence suggests that age at implantation is a strong predictor of language outcomes. However, new behavioral procedures are needed to measure speech and language skills during infancy. Also, procedures are needed to analyze the speech input to young CI recipients. STUDY DESIGN: Cohort-sequential. METHODS: Thirteen infants with profound hearing loss who w

A case of cortical deafness and anarthria.

Generally, cortical deafness is not complicated by anarthria and cortical anarthria does not affect auditory perception. We report a case of simultaneous progressive cortical deafness and anarthria. At the age of 70 years, the patient, a woman, noticed hearing problems when using the telephone, which worsened rapidly over the next 2 years. She was then referred to our hospital for further examinations of her hearing problems. Auditory tests revealed threshold elevation in the low and middle frequencies on pure-tone audiometry, a maximum speech discrimination of 25% and normal otoacoustic emiss…

Cochlear implantation in the deaf-blind.

OBJECTIVE: This study aimed to examine the outcome and rehabilitation of cochlear implantation in a select group of individuals: the deaf-blind. STUDY DESIGN: The study design was a retrospective and prospective case evaluation of the deaf-blind implantees. SETTING: The study was conducted at a national adult and pediatric cochlear implant center. PATIENTS: A total of eight adult and two pediatric blind or visually impaired individuals undergoing cochlear implantation for severe or profound deafness participated. Evaluation of the history, etiology of blindness and deafness, and outcome of …

[The role of cytomegaloviruses in childhood deafness].

The relationship between cytomegalovirus (CMV) and the auditory apparatus is still unclear. The authors examined 120 children from nursery as well as elementary schools for children with hearing defects and, using the complement-fixation method, they determined the CMV antibodies. The result showed a higher quantity of CMV than that found in a group of healthy children. A detailed examination of the auditory functions (of what remained of them) in this group did not eliminate the possibility of CMV effect on the cochlea, acoustic nerve or even on the higher levels of the hearing passage.

Ocular deficiencies in deaf children.

In a sample of 150 deaf children, 45.3% were found to present ocular anomalies interfering with good vision. This high frequency of visual impairment contributes to the low-level functioning and lack of normal developmental drives characteristic of deaf children. Early identification, competent diagnosis and appropriate intervention by a team of specialists are required in the treatment of such cases.

A study of deafness in West Africa.

Some 800 profoundly deaf children from 4 Yoruba states in Nigeria were assessed and examined in an attempt to identify the causes of deafness in this part of West Africa. It was found that measles, meningitis and rubella were the three major preventible diseases causing deafness. Genetic abnormalities, in contradistinction to other tropical countries, do not seem to be very important and chronic ear infections show different manifestations in this area to the situation reported in East Africa. The overwhelming problems that face the deaf and any possible extension of services for the deaf are …

Patterns of drinking among the deaf.

Thirty-nine White deaf persons functioning normally within the general hearing community were surveyed on a variety of factors concerning their use of alcohol, and compared to the data from two comparable hearing samples reported previously in the literature. No significant differences were found between the deaf and hearing samples on patterns of drinking or other parameters of alcohol use. Heavier alcohol use among the deaf correlated significantly with reported frequency of driving after having drunk too much, age of having had the first drink, ever having been drunk, feeling guilt over dri

Deafness in Osteodysplasty of Melnick and Needles.

A 12-year-old boy had osteodysplasty of Melnick and Needles and a mixed conductive and perceptive deafness of both ears. Deafness in this rare, inherited skeletal disorder is exceptional. Structurally, the pinnae and external auditory canals were small; thin, serous fluid was found in both middle ears, and on surgical exploration of the right ear, no round window could be found.

Audiologic evaluation of deaf children.

This paper describes an audiologic test battery for hearing-impaired children which includes otoscopic examination, tympanometry, pure-tone audiometry, speech perception testing, and hearing aid evaluation. Several of the procedures and modifications to apparatus have been developed specifically for use with deaf children. Clinical data are presented from 160 hearing-impaired children (age range three-16 years). Eighteen percent of their ears were found to contain excessive cerumen and to require ear canal irrigation. The incidence of abnormal tympanograms was high for young children but decre
