A CT-Based Deep Learning Radiomics Nomogram for Early Recurrence Prediction in Pancreatic Cancer: A Multicenter Study.

BACKGROUND: Early recurrence (ER) following curative-intent surgery remains a major obstacle to improving long-term outcomes in patients with pancreatic cancer (PC). The accurate preoperative prediction of ER could significantly aid clinical decision-making and guide postoperative management. PATIENTS AND METHODS: A retrospective cohort of 493 patients with histologically confirmed PC who underwent resection was analyzed. Contrast-enhanced computed tomography (CT) images were used for tumor segmentation, followed by radiomics and deep learning feature extraction. In total, four distinct featu

From options to decisions: an innovative model for treatment sequencing in relapsing-remitting multiple sclerosis.

AIMS: With the expanding range of disease-modifying therapies (DMTs) for relapsing-remitting multiple sclerosis (RRMS), clinicians face increasing complexity in defining optimal treatment sequences and timing of therapy switches. In this study, we adapted an earlier computer-assisted model to optimize therapeutic decisions and identify preferred treatment pathways, based on expert opinion and observations from clinical practice in Spain. MATERIALS AND METHODS: The original model was updated to integrate magnetic resonance imaging (MRI) activity data and define reaching an Expanded Disability

Editor's Choice-Biomarkers of acute cardiovascular and pulmonary diseases.

Acute cardiothoracic and respiratory diseases frequently remain a challenge to diagnose and differentiate in the emergency setting. The main diseases that manifest with chest pain include ischaemic heart disease, myocarditis, acute pericarditis, aortic dissection/rupture and pulmonary embolism (PE). Diseases that primarily present with dyspnoea include heart failure (HF), acute respiratory distress syndrome (ARDS), pneumonia, asthma exacerbations and chronic obstructive pulmonary disease. Pre-test probabilities of clinical findings play a vital part in diagnostic decisions, and the use of a Ba

Deformable image registration to assist clinical decision for radiotherapy treatment adaptation for head and neck cancer patients.

Head and neck (H&N) cancer patients often present anatomical and geometrical changes in tumors and organs at risk (OARs) during radiotherapy treatment. These changes may result in the need to adapt the existing treatment planning, using an expert's subjective opinion, for offline adaptive radiotherapy and a new treatment planning before each treatment, for online adaptive radiotherapy. In the present study, a fast methodology is proposed to assist in planning adaptation clinical decision using tumor and parotid glands percentage volume changes during treatment. The proposed approach was applie

Fast personalized electrophysiological models from computed tomography images for ventricular tachycardia ablation planning.

AIMS: Clinical application of patient-specific cardiac computer models requires fast and robust processing pipelines that can be seamlessly integrated into clinical workflows. We aim at building such a pipeline from computed tomography (CT) images to personalized cardiac electrophysiology (EP) model. The simulation output could be useful in the context of post-infarct ventricular tachycardia (VT) radiofrequency ablation (RFA) planning for pre-operative targets prediction. METHODS AND RESULTS: The support for model personalization is a patient-specific virtual three-dimensional heart obtained

Multifactor consciousness level assessment of participants with acquired brain injuries employing human-computer interfaces.

BACKGROUND: A lack of communication with people suffering from acquired brain injuries may lead to drawing erroneous conclusions regarding the diagnosis or therapy of patients. Information technology and neuroscience make it possible to enhance the diagnostic and rehabilitation process of patients with traumatic brain injury or post-hypoxia. In this paper, we present a new method for evaluation possibility of communication and the assessment of such patients' state employing future generation computers extended with advanced human-machine interfaces. METHODS: First, the hearing abilities of 3

Video-assisted thoracic surgery for bronchogenic cysts: is this the surgical approach of choice?

OBJECTIVES: Although there is no consensus on the management of bronchogenic cyst, most surgeons advocate early removal, even in asymptomatic patients. To evaluate the feasibility and safety of video-assisted thoracic surgery (VATS) in the management of bronchogenic cysts and long-term follow-up, a retrospective analysis was performed. METHODS: From January 1995 to April 2013, we retrospectively reviewed the charts of 113 patients who underwent VATS resection of bronchogenic cysts in our institution. Resection of the bronchogenic cysts by VATS was initially performed in patients who had a cys

Residual number processing in dyscalculia.

Developmental dyscalculia - a congenital learning disability in understanding numerical concepts - is typically associated with parietal lobe abnormality. However, people with dyscalculia often retain some residual numerical abilities, reported in studies that otherwise focused on abnormalities in the dyscalculic brain. Here we took a different perspective by focusing on brain regions that support residual number processing in dyscalculia. All participants accurately performed semantic and categorical colour-decision tasks with numerical and non-numerical stimuli, with adults with dyscalculia

Remission from mania is associated with a decrease in amygdala activation during motor response inhibition.

OBJECTIVES: Neuroimaging studies of bipolar disorder (BD) have provided evidence of brain functional abnormalities during both the states of mania and remission. However, the differences in brain function between these two states are still poorly known. In the current study, we aimed to use a longitudinal design to examine the functional changes associated with symptomatic remission from mania within the brain network underlying motor response inhibition. METHODS: Using event-related functional magnetic resonance imaging (fMRI), 10 BD patients and 10 healthy subjects were imaged twice while p

Brain maps of Iowa gambling task.

BACKGROUND: Somatic Marker Hypothesis (SMH), based on clinical observations, delineates neuronal networks for interpreting consciousness generation and decision-making. The Iowa gambling task (IGT) was designed to verify the SMH. However, more and more behavioral and brain imaging studies had reported incongruent results that pinpointed a need to re-evaluate the central representations of SMH. The current study used event-related fMRI (functional Magnetic Resonance Imaging) to examine neural correlates of anticipation vs. outcome, wins vs. losses, and differential decks' contingencies of IGT.

Development of a vibrotactile tasking device for use in vestibular assessment.

The purpose of this study was to design an automated mental alerting task that could be utilized when performing vestibular testing on a broad range of patient populations, including certain difficult-to-test populations, such as the hard-of-hearing. A device was developed that utilized vibrotactile stimuli output to two vibrators placed on the subject's left leg, and responded to activation of two momentary pushbuttons controlled by the subjects. Fourteen normal-hearing subjects without history or symptoms of vestibular involvement participated. Each participant underwent three mental-alertin

Description and preliminary evaluation of a Multiagent Intelligent Dosing System (MAIDS) to manage combination insulin-oral agent therapy in type 2 diabetes.

BACKGROUND: Computer decision support systems are potentially effective methods for adjusting insulin, but current models do not take into account simultaneous changes of more than one agent. We describe the development of the Multiagent Intelligent Dosing System (MAIDS, Dimensional Dosing Systems, Wexford, PA) for predicting glycemic outcome in response to concurrent dose adjustments in oral hypoglycemic agents and insulin. METHODS: Retrospective data from a patient cohort with type 2 diabetes who had simultaneous changes in insulin and metformin were analyzed. Glycemic markers (fasting gluc

Coherence-Based Beamforming Increases the Diagnostic Certainty of Distinguishing Fluid from Solid Masses in Breast Ultrasound Exams.

Ultrasound is often used as a supplement for mammography to detect breast cancer. However, one known limitation is the high false-positive rates associated with breast ultrasound. We investigated the use of coherence-based beamforming (which directly displays spatial coherence) as a supplement to standard ultrasound B-mode images in 25 patients recommended for biopsy (26 masses in total), with the eventual goal of decreasing false-positive rates. Because of the coherent signal present within solid masses, coherence-based beamforming methods allow solid and fluid-filled masses to appear signifi

Hash Transformation and Machine Learning-Based Decision-Making Classifier Improved the Accuracy Rate of Automated Parkinson's Disease Screening.

Digitalized hand-drawn pattern is a noninvasive and reproducible assistive manner to obtain hand actions and motions for evaluating functional tremors and upper-limb movement disorders. In this study, spirals and straight lines in polar coordinates are used to extract polar expression features such as the key parameters deviation (cm) and accumulation angle (rad). These parameters are quantitative manner to scale the variations of functional tremors in normal control subjects and patients with Parkinson's disease (PD) and essential tremor (ET). However, difficulty arises in using nonlinear pol

Neural correlates of moral judgment in pedophilia.

Pedophilia is a sexual preference that is often associated with child sex offending (CSO). Sexual urges towards prepubescent children and specifically acting upon those urges are universally regarded as immoral. However, up until now, it is completely unknown whether moral processing of sexual offenses is altered in pedophiles. A total of 31 pedophilic men and 19 healthy controls were assessed by using functional magnetic resonance imaging (fMRI) in combination with a moral judgment paradigm consisting of 36 scenarios describing different types of offenses.Scenarios depicting sexual offenses a

The neural circuitry of a broken promise.

Promises are one of the oldest human-specific psychological mechanisms fostering cooperation and trust. Here, we study the neural underpinnings of promise keeping and promise breaking. Subjects first make a promise decision (promise stage), then they anticipate whether the promise affects the interaction partner's decision (anticipation stage) and are subsequently free to keep or break the promise (decision stage). Findings revealed that the breaking of the promise is associated with increased activation in the DLPFC, ACC, and amygdala, suggesting that the dishonest act involves an emotional c

Dopaminergic Radiopharmaceutical Imaging in Parkinsonian Syndromes: From Molecular Targets to Clinical Decision-Making.

Parkinsonian syndromes comprise overlapping neurodegenerative and non-degenerative disorders, making aetiological diagnosis difficult, while existing procedural guidance does not fully integrate tracer-specific biology with clinical decision-making, multimodal strategies, and emerging quantitative and pathology-directed biomarkers. This review synthesises evidence on the molecular targets, radiopharmaceutical characteristics, biological interpretation, and clinical applications of dopaminergic single-photon emission computed tomography (SPECT) and positron emission tomography (PET), focusing o
