Liberation Playbook: Pain, Sedation, Sleep, and Mobility Led by Advanced Practice Registered Nurses.

Mechanical ventilation is lifesaving yet harmful when paired with deep sedation and immobility. The ABCDEF, or intensive care unit liberation bundle (assessment and treatment of pain, spontaneous awakening and breathing trials, judicious choice of analgesia/sedation, delirium prevention/management, early mobility, and family engagement) mitigates these risks, improving survival, reducing delirium and coma, shortening time receiving ventilation, and promoting recovery. This article synthesizes evidence for the awake-and-walking intensive care unit, in which wakefulness and mobility are the defa…

Modern sedative agents and techniques used in dentistry for patients with special needs: A review.

According to the World Health Organisation, approximately 1.3 billion people worldwide experience substantial disability due to physical, mental or sensory impairment. People with special needs require special consideration and more time or altered delivery methods when receiving dental treatments. Various factors, such as patients' lack of cooperation, cognitive impairment and complex medical status, may lead dental practitioners to recommend conscious sedation. Several pharmacological agents and administrative routes are available, which achieve varying levels of sedation ranging from minima

Etomidate-ketamine versus dexmedetomidine-ketamine for entropy-guided procedural sedation during endoscopic retrograde cholangiopancreatography procedures: A randomized single blind study.

BACKGROUND AND AIMS: The major challenge for the anesthetist in endoscopic retrograde cholangiopancreatography (ERCP) procedures is to provide moderate to deep levels of sedation in prone position with preservation of spontaneous respiratory efforts in shared airway scenario with an endoscopist. These patients have other comorbidities, making them vulnerable to complications during the routinely used sedation with propofol. We compared the entropy-guided efficacy of combination of etomidate-ketamine to dexmedetomidine-ketamine in patients undergoing ERCP. METHODS: This prospective single blin

Deep intrauterine insemination and embryo transfer in pigs.

A new method for non-surgical deep intrauterine catheterization of pigs, without sedation of the sow, is described. Insemination results obtained with this method using fresh spermatozoa demonstrate that, in comparison to conventional artificial insemination (AI) (3 x 10(9) spermatozoa in 80-100 ml), a 20-60-fold reduction in the number of spermatozoa inseminated and at least a 8-10-fold reduction in the dose volume can be used without affecting fertility if spermatozoa are deposited deep (middle or upper) into one of the uterine horns. Results from deep intrauterine insemination with frozen-t

Managing Persistent Radial Artery Spasm During Coronary Catheterization: A Complex Case Study.

This case report details the management of persistent radial artery spasm during coronary catheterization, unresponsive to standard pharmacotherapy. After conventional methods failed, deep sedation, neuromuscular blockade, and orotracheal intubation proved essential. We propose a structured step-wise approach to radial artery spasm, emphasizing adaptable strategies for treatment-resistant cases.

A comparison of the use of propofol alone and propofol with midazolam for pediatric magnetic resonance imaging sedation - a retrospective cohort study.

BACKGROUND: Pediatric MRI sedation performed by a variety of specialists such as sedationists and anesthesiologists commonly uses propofol, which has similar effects to an ideal sedative agent for maintaining deep sedation. However, when propofol is used, adverse airway events are relatively more common than when using other sedative agents. The concomitant administration of midazolam and propofol can reduce the dose of propofol needed for adequate sedation and might also reduce the frequency of airway obstruction without affecting the patient's recovery profile. METHODS: We reviewed the our …

Combined Use of Remimazolam and Ciprofol Reduces Hypoxemia and Shortens Recovery Time During Sedated Gastrointestinal Endoscopy.

BACKGROUND: Hypoxemia is the most common adverse event during painless gastrointestinal endoscopy (PGIE). This study aimed to evaluate whether the combined use of remimazolam besylate and ciprofol (group RC) reduces the incidence of hypoxemia compared to ciprofol alone (group C) in patients undergoing PGIE under deep sedation. METHODS: A total of 246 patients scheduled for PGIE were recruited from the Second Xiangya Hospital of Central South University and randomly assigned to group C or RC. The primary outcome measured was the incidence of intraoperative hypoxemia. Secondary outcomes include…

[Potential effect of the stimulus threshold level of the nociceptive flexion reflex (NFRT) on mortality and delirium incidence in the critically ill patient: a retrospective cohort analysis].

BACKGROUND: Mortality and delirium in critically ill patients are affected by the provided analgesics and sedatives. The deeper the sedation and the higher the dose of analgesics applied, the more difficult it is to assess pain and the depth of sedation. Therefore, instrumental measurement methods, such as the measurement of the stimulus threshold of the nociceptive flexion reflex (NFRT), are becoming increasingly more important. OBJECTIVE: The aim of the present study is to investigate a potential association between the level of the nociceptive flexion reflex, mortality, and the occurrence

Sedoanalgesia modality during laser photocoagulation for retinopathy of prematurity: Intraoperative complications and early postoperative follow-up.

BACKGROUND: Laser photocoagulation (LPC) is a surgical procedure used in the treatment of premature retinopathy that may cause retinal detachment and blindness if not diagnosed and treated early. The anesthesia method used in LPC varies from sedoanalgesia to general anesthesia and airway management varies from spontaneous ventilation to endotracheal intubation. In this study, we aimed to evaluate the effectiveness of sedoanalgesia applications and this anesthesia procedure concerning intraoperative and postoperative complications by avoiding intubation and mechanical ventilation in premature i

Propofol Increases Host Susceptibility to Microbial Infection by Reducing Subpopulations of Mature Immune Effector Cells at Sites of Infection.

Anesthetics are known to modulate host immune responses, but separating the variables of surgery from anesthesia when analyzing hospital acquired infections is often difficult. Here, the bacterial pathogen Listeria monocytogenes (Lm) was used to assess the impact of the common anesthetic propofol on host susceptibility to infection. Brief sedation of mice with physiologically relevant concentrations of propofol increased bacterial burdens in target organs by more than 10,000-fold relative to infected control animals. The adverse effects of propofol sedation on immune clearance of Lm persisted

Effect of sedation using Ketamine for primary closure of pediatric facial laceration.

Pediatric lacerations are frequently encountered by plastic surgeons in the emergency room. Since pediatric patients cannot cooperate due to the anxiety and pain occurring during the suture procedure, sedation is induced. Since commonly used drugs inducing shallow sedation such as chloral hydrates are insufficient to perform procedures, the need or deep sedation has been increased. In our experience, inducing sedation with ketamine is safe and allows for accurate procedures. A total of 106 pediatric patients aged between 3 months to 5 years who visited the emergency room between August 2020 an…
