Can chronic neuroleptic treatment promote sleep disturbances in elderly schizophrenic patients?

It has been proposed that sleep disturbances, especially reduced delta sleep, are related to a poor outcome in schizophrenia. To determine whether long-term treatment with neuroleptics can promote sleep disturbances by increasing the risk of a nocturnal myoclonus syndrome (NMS) (=periodic movements in sleep) related insomnia, we performed all-night polysomnography in 10 chronically ill schizophrenic patients who had been under neuroleptic therapy for a mean of 27 years. NMS-related insomnia was detected in all 10 patients. Potential pathophysiological relationships between long-term neurolepti

Automatic detection of K-complexes: validation in normals and dysthymic patients.

A model-based automatic K-complex (Kc) detector was applied to all-night single-channel sleep electroencephalographic (EEG) recordings from normal and dysthymic patients. The performance of the detector was analyzed in the two groups, and the differences obtained were discussed. The results showed that the detection rate of Kc in the normal group was around 92% through all stages of non-rapid eye movement (NREM) sleep, but with high numbers of "false" positives in stage 4 NREM, which reached 57%. In the dysthymic patients "true" detection included 85% of the Kc, but the percentage of "false" p

Auditory neural entrainment: frequency-specific functional roles and emerging applications in neurological and psychiatric disorders.

Among sensory inputs, auditory information plays a significant role. Neural entrainment may establish a bridge for the brain's perception of rhythmic sound signals. It refers to the synchronization of ongoing neuronal activity with internal and external rhythmic stimuli, demonstrating the brain's plasticity and adaptive dynamics. Across canonical EEG bands, delta activity is associated with temporal prediction and hierarchical auditory processing over longer time scales; theta activity with speech parsing, audiovisual integration, and memory-related timing; alpha activity with cross-modal info

A Personalized Closed-Loop Brain Stimulation Protocol for Difficulty Falling Asleep.

Insomnia, a common mental health issue, is characterized by brain hyperarousal and difficulties in transitioning between sleep stages. Pharmacological interventions often come with adverse effects and variable efficacy, while non-invasive brain stimulation holds potential but lacks in providing tailored, dynamic, and precisely targeted solutions. A novel online closed-loop individualized frequency transcranial alternating current stimulation protocol (IF-tACS) was developed to dynamically adjust the stimulation sites and parameters in sync with sleep states. Specifically, the frequency of the

[Comparison of continuous wave Doppler and heart catheterization in the determination of the mean transvalvular gradient in mitral stenosis. A simultaneous beat to beat analysis].

Continuous wave Doppler echocardiography (CWD) is widely used in the assessment of pressure gradients in patients with valvular heart disease, utilizing the simplified Bernoulli equation. However determination of non-simultaneous mean pressure gradient (MG) in mitral stenosis (MS) from CWD recordings has often been described as being unsatisfactory. Therefore, the purpose of this study was to compare the estimates of trans-mitral MG derived from CWD with gradients measured simultaneously at cardiac catheterization (beat to beat analysis). We studied 3 patients (pts) with MS (1 man and 2 women,

Delta oscillations are a robust biomarker of dopamine depletion severity and motor dysfunction in awake mice.

Delta oscillations (0.5-4 Hz) are a robust feature of basal ganglia pathophysiology in patients with Parkinson's disease (PD) in relationship to tremor, but their relationship to other parkinsonian symptoms has not been investigated. While delta oscillations have been observed in mouse models of PD, they have only been investigated in anesthetized animals, suggesting that the oscillations may be an anesthesia artifact and limiting the ability to relate them to motor symptoms. Here, we establish a novel approach to detect spike oscillations embedded in noise to provide the first study of delta

Safranal enhances non-rapid eye movement sleep in pentobarbital-treated mice.

AIMS: Safranal (2,6,6-trimethyl-1,3-cyclohexadiene-1-carboxaldehyde, C(10) H(14) O) is an active ingredient in the saffron, which is used in traditional medicine. It has been reported to have sedative and anti-epileptic effects, but its hypnotic effects remain uncertain. The aim of this study was to evaluate effects of safranal on sleep-wake cycle. METHODS: We established hypnotic-model mice treated with a low dose of pentobarbital 20 mg/kg, and administered different doses of safranal, vehicle, or diazepam. The change of sleep-wake cycle was assessed by sleep recording and c-Fos expression i

Intracortical electroencephalography in acute brain injury.

OBJECTIVE: Continuous electroencephalography (EEG) is used in patients with neurological injury to detect electrographic seizures and clinically important changes in brain function. Scalp EEG has poor spatial resolution, is often contaminated by artifact, and frequently demonstrates activity that is suspicious for but not diagnostic of ictal activity. We hypothesized that bedside placement of an intracortical multicontact electrode would allow for improved monitoring of cortical potentials in critically ill neurological patients. METHODS: Sixteen individuals with brain injury, requiring invas

Electroencephalographic and respiratory activities during acute intermittent hypoxia in anesthetized rats.

Cortical activity and respiratory responses to intermittent hypoxia were studied in anesthetized, paralyzed, vagotomized, and artificially ventilated rats. Respiratory responses to hypoxic exposure consisted of stimulation of phrenic and hypoglossal activity and a subsequent decline of the activity up to apnea. The respiratory response to hypoxia was accompanied by a gradual decrement of the total power of EEG. Relative EEG power increased in the delta frequency range and decreased in the remaining frequency ranges. During hypoxic bradypnea or apnea, the total power of EEG strongly diminished

The role of alertness in organic brain syndromes.

We examined 35 patients, aged from 37 to 76 years, with impairment of intellectual functions due to normotensive hydrocephalus of varied origin. Axial tomography showed an enlarged ventricular system, with or without cortical atrophy. As a rule, the patients having no cortical atrophy displayed a significantly lower alertness level than the patients with cortical atrophy, who were more alert. Because the clinical picture was similar in both subgroups, it was postulated that the impairment of cortical function due to organic lesions resulted in symptoms similar to those due to chronic decrease

Impact of Heart Rate and Rhythm on Corrected QT Interval During Paroxysmal Atrial Fibrillation.

Current knowledge on the dynamic changes of corrected QT (QTc) before, during, and after an atrial fibrillation (AF) episode is limited. It remains controversial which of the presently available formulas performs the best in calculating QTc during AF. This study was designed to explore whether an AF attack would affect QTc and to determine the performance of 6 available formulas in correcting QT before, during, and after AF. A total of 101 patients with Holter-documented paroxysmal AF were enrolled. QT interval before, during, and after AF was measured and corrected to heart rate (HR) by using

A genetic algorithm for self-supervised models of oscillatory neurodynamics.

Predictive processing theories propose that the brain builds internal models of its environment by reducing the discrepancy between internally generated predictions and external sensory signals. Prior work has linked these processes to oscillatory activity in gamma (40-100 Hz) and alpha/beta (10-30 Hz) frequency ranges. Current computational approaches face a trade-off: abstract predictive-processing models can implement self-supervised computations but often omit oscillatory spiking dynamics, whereas biophysically constrained spiking models can generate neural rhythms but often require extens

Neuronal desynchronization as marker of an impaired brain network.

Synchronization is a central key feature of neural information processing and communication between different brain areas. Disturbance of oscillatory brain rhythms and decreased synchronization have been associated with different disorders including schizophrenia. The aim of this study was to investigate whether synchronization (in relaxed conditions with no stimuli) between different brain areas within the delta, theta, alpha (alpha1, alpha2), beta (beta1, beta2), and gamma bands is altered in patients with a neurological disorder in order to generate significant cortical enhancements. The functional relationship between yawning and vigilance.

BACKGROUND: Although yawning is a ubiquitous and phylogenetically old phenomenon, its origin and purpose remain unclear. The study aimed at testing the widely held hypothesis that yawning is triggered by drowsiness and brings about a reversal or suspension of the process of falling asleep. METHODS: Subjects complaining of excessive sleepiness were spontaneously yawning while trying to stay awake in a quiet and darkened room. Changes in their electroencephalogram (EEG) and heart rate variability (HRV) associated with yawning were compared to changes associated with isolated voluntary body move

[Transesophageal pacing in the diagnosis of accelerated atrioventricular conduction].

The term "enhanced atrioventricular nodal conduction" (EAVN) is used to indicate an electrophysiologic condition characterized by subnormal conduction delay with reduced decremental properties in the AV node, which can be responsible for rapid ventricular rates in the event of fast atrial rhythms. Although identification of such an entity usually requires definition of the AV conduction intervals, some authors have suggested that EAVN can be diagnosed, by means of atrial pacing only, when 1:1 conduction with narrow QRS complexes occurs during atrial pacing at rate higher than 200 bpm. The use

Impact of transisthmus linear ablation of typical atrial flutter on coronary sinus activation time.

Complete or incomplete bidirectional isthmus conduction block after linear ablation of atrial flutter is difficult to interpret without detailed multiple electrodes mapping along the tricuspid annulus and the low right atrial isthmus area. The influence of isthmus block on the intraatrial septal and coronary sinus activation has not been assessed by endocardial mapping. This study was designed to analyze the intraartial and interatrial activation times in a retrospective fashion to investigate (1) whether isthmus conduction block can change the coronary sinus activation sequence during low lat

A high-density electroencephalography study reveals abnormal sleep homeostasis in patients with rapid eye movement sleep behavior disorder.

Rapid eye movement (REM) sleep behavior disorder (RBD) is characterized by disrupting motor enactments during REM sleep, but also cognitive impairments across several domains. In addition to REM sleep abnormalities, we hypothesized that RBD patients may also display EEG abnormalities during NREM sleep. We collected all-night recordings with 256-channel high-density EEG in nine RBD patients, predominantly early-onset medicated individuals, nine sex- and age- matched healthy controls, and nine additional controls with matched medications and comorbidities.
