Long-term results of implant-supported over-dentures retained by double crowns: a practice-based retrospective study after minimally 10 years follow-up.

BACKGROUND: Different concepts regarding the number of implants and attachment systems for the preparation of implant-supported over-dentures (IODs) have been discussed. Nonetheless, long-term results for double-crown-retained IODs with an observational period of more than 10 years are still rare in the literature. OBJECTIVE: The aim of this practice-based study was to retrospectively evaluate the long-term clinical outcome (success/survival rates, technical/biological complications) of IODs retained by double crowns. MATERIAL AND METHODS: In a private practice, 36 non-smoking edentulous pat

Evaluation and comparison of retention and patient satisfaction with milled polyetheretherketone versus metal maxillary obturators.

STATEMENT OF PROBLEM: Metal and polyetheretherketone (PEEK) have been used to rehabilitate maxillary defects with obturators, each having advantages and disadvantages. However, patient satisfaction with the obturator retention of each material is unclear. PURPOSE: The purpose of this crossover clinical study was to compare retention and patient satisfaction between a milled cobalt chromium and a milled PEEK framework, each with a hollow bulb extension in polymethyl methacrylate (PMMA). MATERIAL AND METHODS: Twenty patients who had received unilateral total maxillectomy and were treated in th

Oral implants in dependent elderly persons: blessing or burden?

BACKGROUND: Implant-supported (partial) dentures may raise problems in patients who have become dependent on others for daily oral health care. Dental hygienists and general dental practitioners, as well as care providers, volunteer aiders and even health care insurance companies, should anticipate the growing demand for specific oral health care for patients provided with implant-supported (partial) dentures. OBJECTIVE: To report three cases of dependent patients and to present recommendations to prevent or resolve implant-related oral problems. MATERIALS AND METHODS: The three case reports

Comparison of obturator design for acquired maxillary defect in completely edentulous patients.

The most challenging and appreciated area in the field of Prosthodontics is the rehabilitation of maxillary defects. Tumors of the head and neck are the common cause for acquired maxillofacial defects. Surgical consequences predispose the patient to hypernasal speech, fluid leakage into nasal cavity, impaired masticatory function, and cosmetic deformity. The Prosthodontists play a significant role in the intervention and improve the quality of life of such patients. The current article describes two clinical case reports of completely edentulous patients with acquired maxillary defects.

Patient satisfaction and dissatisfaction with mandibular two-implant overdentures using different attachment systems: 5-year outcomes.

BACKGROUND: Different attachment systems for mandibular two-implant overdentures could influence levels of patient satisfaction. Positive consensus of the majority of patients being satisfied does not preclude the possibility of dissatisfaction for some. PURPOSE: To evaluate patient satisfaction and dissatisfaction before and after 5 years of wearing mandibular two implant overdentures using different attachment systems ; to determine the prognostic relevance of pretreatment complete denture complaints to treatment outcomes. Effect of prolonged ultrasonic instrumentation on the retention of cemented cast crowns.

Occasionally the need arises to remove restorations that have been cemented. Several ultrasonic tips have been developed to aid in removing cemented restorations. Earlier studies of the effects of ultrasonic vibration reveal detrimental effects on amalgam surface and marginal integrity and, at short instrumentation periods (2.5 minutes) in vitro, no significant effect on the retention of cemented cast crowns. Twenty identically machined metal dies had nonprecious crowns waxed, cast, and fitted. Each casting was cemented twice with glass ionomer and zinc phosphate cement. Each cement was tested

Treatment of an edentulous patient with a dry mouth.

Dental health professionals are being asked to care for a growing number and range of medically compromised patients living with chronic health problems. Although tooth loss overall has declined in the United States, millions of persons, particularly those of more advanced age, still require treatment for the edentulous condition. Particular challenges are faced when this oral state is combined with a complex medical history. The primary learning objective for this case is to increase your general knowledge of and skills in the dental management of the complete denture patient with a dry mouth

Dental rehabilitation using an implant-carrying plate system in a severely resorbed edentulous maxilla: a case report.

This clinical article describes a case of dental rehabilitation using an implant-carrying plate system (EPITEC) for a patient with severely resorbed edentulous maxilla and microstomia. In this case, the presence of microstomia prevented bone augmentation procedures through an intraoral approach. Treatment using 2 endosseous implants inserted in the canine regions and an implant-supported overdenture was planned. However, endosseous implants were not feasible on the right side because of insufficient available bone volume. An implant-carrying plate system was then utilized on the right side. Fo

Chemical etching solutions for creating micromechanical retention in resin-bonded retainers.

This study introduced three chemical etching solutions capable of producing micromechanical retention in nickel-chromium and nickel-chromium-beryllium alloys used for resin-bonded retainers. The effectiveness of the chemical etching solutions was evaluated with tensile strength tests and photographs at various magnifications with a scanning electron microscope. Chemical etching with the CG-Etch solution produced suitable and uniform microretention whereas the other solutions were not effective on all metal alloys. Significant differences (p < 0.05) relating to tensile bond strengths were noted

Evaluation of esthetics, pain scores, and prognosis of modified paralleling technique for high frenal attachment: A case series.

A frenum is a thin fold of mucous membrane that attaches the lips to the alveolar mucosa and the periosteum. The frenum provides stability and supports the lips and the tongue. Normally, the terminal end of the frenum is seen till the alveolar mucosa or the gingiva. However, in some individuals, the frenum extends to or beyond the interdental papilla. Such an aberrant or high frenular attachment compromises plaque control, orthodontic treatment, denture retention, and esthetics. Therefore, surgical correction of the high frenal attachment is recommended either by completely removing the frenum…

Modified Mandibular Advancement Device Is Effective in Patients With Insufficient Teeth: A Systematic Review.

BACKGROUND: Mandibular advancement devices (MADs) are established alternatives to continuous positive airway pressure (CPAP) for obstructive sleep apnea syndrome (OSAS), particularly valued for better patient compliance. Traditional contraindications include insufficient healthy teeth for retention, affecting a significant portion of patients due to global edentulism rates of up to 37.8% in ageing populations. Despite emerging reports challenging this, no synthesis exists on MAD efficacy in edentulous or partially edentulous patients. METHODS: This PRISMA-compliant systematic review (PROSPERO

Comparative Evaluation of Stress Acting on Abutment, Bone, and Connector of Different Designs of Acid-Etched Resin-Bonded Fixed Partial Dentures: Finite Element Analysis.

Background: Finite element analysis (FEA) is one of the best methods for evaluating the stress distribution of restorations, such as fixed partial dentures. The development of resin cement has transformed prosthesis bonding and retention properties. Resin-bonded fixed partial dentures (RBFPD) have been considered minimally invasive treatment options for the prosthetic rehabilitation of single missing teeth. Objectives: The aim of this study was to evaluate the stress load and distribution in four different designs of acid-etched RBFPDs using FEA. Materials and Methods: The designs included sta
