Knowledge on age-relationship to irritant contact dermatitis (ICD) remains of interest. We searched for articles and textbooks on age-relationship to ICD and evaluated relevant data. Irritant response may be enhanced in children and decline with increasing age. In general, older skin reacts more slowly and with less intensity to irritants when compared with young skin. Such age-related changes may depend: (i) on differences in percutaneous penetration in old and young skin, and/or on (ii) differences in the microcirculatory efficiency, which serves as the route by which inflammatory cells make

Allergic contact dermatitis (ACD) is a common skin disease that has significant socio-economic impact. ACD is mediated by a T-cell mediated inflammatory reaction. Langerhans cells (LCs) are an epidermal DCs subset specialized in antigen presentation. After hapten exposure, LCs play a major role as in induction adaptive immune response against allergens. LCs recognize, take up and process haptens and migrate to the local draining lymph nodes. However, LCs specific functions and the LCs migration to local draining lymph nodes are not yet clearly defined. Recent advance in the knowledge of LCs fu

Dimethyl fumarate is a fumaric acid ester. It been used for some years to treat psoriasis and also as a preservative in desiccant sachets in the transport of furniture and footwear. Its irritant properties and sensitizing potential in contact with the skin were recently highlighted when it was implicated as the causative agent in 2 epidemics of severe acute eczema: sofa dermatitis in northern Europe and shoe dermatitis in Spain. The present article aims to guide dermatologists in the diagnosis and management of patients allergic to dimethyl fumarate. We review the clinical manifestations, resu

A deterioration in chronic inflammatory ear disease despite topical medication, or failure of the condition to improve with such treatment, should raise the possibility of allergic contact dermatitis. Allergen testing (patch testing) is the 'gold standard' method of identifying an agent causing allergic contact dermatitis. We describe an ENT patch test series applied by our department as a screening device for this condition.

Inhaled and intranasal corticosteroids (CS) are used to treat various inflammatory or allergic diseases of the upper and lower respiratory tract. Despite their wide use, only a few reports about their sensitization potential have been published. We determined the frequency, clinical signs and molecules responsible for skin allergic reactions following inhalation of corticosteroids amongst patients with identified and investigated "contact allergy" to corticosteroids. We reviewed clinical data, patch test results and sensitization sources in patients who reacted positively to corticosteroids te

Allergic contact dermatitis to topical medicaments occurs often. Common medicaments associated with allergic contact dermatitis include topically applied antibiotics, antihistamines, nonsteroidal antiinflammatory drugs, anesthetics, and corticosteroids. In cases of suspected allergic contact dermatitis, medicaments that the patient is using should be included when patch testing. Body sites susceptible to allergic contact dermatitis to topical medicaments include scalp, face, anogenital area, and lower extremities.

The management of patients who have developed a contact dermatitis due to topical drugs requires to stop the suspected drug application, to treat the patient with corticosteroid ointments, to perform dermatoallergological investigations e.g. drug patch tests in case of contact eczema and diluted prick tests in case of contact urticaria in order to determine whether the adverse drug reaction was due to excipients or to the drug itself. Patients with chronic leg ulcers have a high risk of contact dermatitis to topical drugs and to antiseptics. It is necessary to advise the patient concerning the

BACKGROUND: Buprenorphine is a low-molecular-weight, lipophilic, opioid analgesic. The transdermal delivery system (TDS) containing it has skin irritation potential, but at least two cases of contact allergy to the active principal have been described previously.
OBJECTIVE: To confirm allergic contact dermatitis from transdermal buprenorphine (TDB) in five older patients suffering from chronic pain and who developed persistent, pruritic erythematous plaques at the contact sites, with two of them also presenting with a generalized skin eruption.
METHODS: Besides the baseline patch test series

Heracleum plants occur in numerous species worldwide and may cause phototoxic reactions due to its content of various furocoumarins. In this case report, a widespread photoallergic contact dermatitis after exposure to Heracleum giganteum (giant bear claw) is described. A photopatch test with extracts from the stem, leaves and seeds of the giant bear claw revealed a positive papulovesicular reaction that already appeared at 24 h and peaked at 72 h after irradiation with 5 J/cm(2) UVA. The unirradiated controls remained negative. We conclude that in rare cases Heracleum plants may cause severe p

PURPOSE OF REVIEW: To give selected new information on contact allergy and allergic contact dermatitis with focus on diagnostic procedures and pitfalls.
RECENT FINDINGS: Recent studies dealing with common contact allergens have improved our understanding of the relationship between positive patch tests and the clinical interpretation and consequences for the patient.
SUMMARY: Nickel allergy is still the most common contact allergy in Europe in spite of full implementation of the EU Nickel Directive in 2001. Contact allergens in cosmetics and topical drugs are another common cause of allergic
