Three etiologic facets of dandruff and seborrheic dermatitis: Malassezia fungi, sebaceous lipids, and individual sensitivity.

Application of new molecular and biochemical tools has greatly increased our understanding of the organisms, mechanisms, and treatments of dandruff and seborrheic dermatitis. Dandruff results from at least three etiologic factors: Malassezia fungi, sebaceous secretions, and individual sensitivity. While Malassezia (formerly P. ovale) has long been a suspected cause, implicated by its presence on skin and lipophylic nature, lack of correlation between Malassezia number and the presence and severity of dandruff has remained perplexing. We have previously identified the Malassezia species correla

Beyond Efficacy: 0.6% Selenium Disulfide Shampoo Matches 2% Ketoconazole for Seborrheic Dermatitis with Superior Cosmesis.

BACKGROUND: Although most dandruff shampoos are effective, many leave hair dry and unmanageable, potentially impacting patient compliance and treatment outcomes. OBJECTIVE: This study compared the efficacy, tolerability, and cosmetic benefits of 0.6% micronized selenium disulfide shampoo (SeS2) to 2% ketoconazole shampoo. METHODS: Eighty-seven male and female subjects with moderate to severe dandruff participated in a single-center, 6-week, randomized, double-blind study comparing the benefits and attributes of the shampoos. RESULTS: After 6 weeks of use, Total Scale Score (TSS) decreased b

Ketoconazole beyond antifungal activity: Bioinformatics-based hypothesis on lipid metabolism in dandruff and seborrheic dermatitis.

The role of Malassezia yeasts in dandruff and seborrheic dermatitis is unclear; however, antifungal therapy with ketoconazole is commonly used. We propose that ketoconazole shifts skin lipid profile, affects Malassezia lipid metabolism and favours biotin-producing bacteria. Biotin regulates inflammatory response and cell proliferation, contributing to symptom improvement.

Clinical and biochemical assessment of maintenance treatment in chronic recurrent seborrheic dermatitis: randomized controlled study.

INTRODUCTION: Few studies have investigated the long-term effects of a maintenance regimen in the prevention of relapses in scalp seborrheic dermatitis (SD), in particular following biomarker changes. MATERIALS AND METHODS: A new shampoo containing beta-glycyrrhetinic acid (18βGA) in addition to cyclopiroxolamine (CPO) and zinc pyrithione (ZP) was tested in 67 subjects suffering from SD with moderate to severe erythema and itching in a biphasic study. After a first common intensive treatment phase (investigational product thrice a week × 2 weeks), subjects randomly received the investigationa

Physical activity as a potential nonpharmacologic strategy to prevent seborrheic dermatitis: a cohort study in the UK biobank.

BACKGROUND: Seborrheic dermatitis (SD) is a chronic inflammatory skin disease with limited long-term treatment options. The association between physical activity and incident SD, as well as the potential mediating role of depressive symptoms, remains unclear. OBJECTIVE: To investigate the association between physical activity and incident SD and to assess whether depressive symptoms mediate this relationship. METHODS: Data from 396,903 participants in the UK Biobank were analyzed. Cox proportional hazards models were used to examine the association between physical activity and incident SD.

Clinical Efficacy and Scalp Microbiome Changes Induced by AMPamide-Containing Shampoo in Patients With Seborrheic Dermatitis.

BACKGROUND: Seborrheic dermatitis (SD) is a chronic inflammatory scalp disorder associated with Malassezia dysbiosis and increased sebum production. AMPamide has been suggested to have anti-inflammatory and sebum-regulating effects, but its clinical efficacy and microbiome-modulating effects in SD remain unclear. OBJECTIVE: To evaluate the clinical efficacy and scalp microbiome changes following 4 weeks of use of an AMPamide-containing shampoo in patients with SD. METHODS: In this observational study, 30 patients with SD applied an AMPamide-containing shampoo for 4 consecutive weeks. Clinica

Association of Serum IgE and Peripheral Eosinophilia with Seborrheic Dermatitis: Evidence from a Case-Control Study.

INTRODUCTION: It is thought that some shared mechanisms may play a role in the pathophysiology of seborrheic dermatitis (SD) and atopic conditions, both of which are chronic, inflammatory, and recurrent in nature. OBJECTIVE: This study aimed to investigate the presence of atopic diseases in SD patients without atopic dermatitis or other inflammatory skin disorders and to assess their serum IgE levels and peripheral eosinophil values as a marker of atopy. METHODS: This prospective cross-sectional study included 83 SD patients and 68 age- and sex-matched healthy controls. RESULTS: There were

Cutaneous fungal microbiome: Malassezia yeasts in seborrheic dermatitis scalp in a randomized, comparative and therapeutic trial.

Malassezia spp in skin microbiome scalp has been implicated in seborrheic dermatitis pathogenesis. Thus, treatment based in antifungal combined to topical keratolitic agents have been indicated as well as oral isotretinoin as it reduces the sebum production, glandular's size and possesses anti-inflammatory properties. This randomized, comparative and therapeutic trial aimed toper form the genotypic identification of Malassezia species before and after low-dose oral isotretinoin or topical antifungal treatments for moderate to severe seborrhea and/or seborrheic dermatitis on scalp. Scales and s

Experience with repetitive use of pimecrolimus: exploring the effective and safe use in the treatment of relapsing seborrheic dermatitis.

BACKGROUND: Seborrheic dermatitis (SD) tends to reoccur frequently, so the therapeutic agent must have a high benefit versus adverse effect ratio for long-term and repetitive uses. OBJECTIVE: To assess the efficacy and safety of retreatment with pimecrolimus in SD patients. METHODS: A total of 45 patients who had been treated previously with pimecrolimus 1% cream were retreated with the same agent using the same treatment protocol after the disease reoccurred. The first and the second treatments were compared with regard to disease severity before treatment, complete cure times, remission ti

Quality of life in patients suffering from seborrheic dermatitis: influence of age, gender and education level.

Seborrheic dermatitis is a common inflammatory skin condition occurring mostly on the face, scalp and chest. Despite its high frequency, the impact of seborrheic dermatitis on patients' quality of life (QoL) has not been studied well so far. The objectives of this study were to analyse how seborrheic dermatitis affects the patients' QoL and which socio-economic factors could modulate QoL in these patients. A total of 3000 patients with seborrheic dermatitis and/or dandruff were enrolled into the study. All participants were divided into subgroups according to gender, age and education level. A

Association of Pityrosporum orbiculare (Malassezia furfur) with seborrheic dermatitis in patients with acquired immunodeficiency syndrome (AIDS).

The possible causative role of the yeastlike fungus Pityrosporum (Malassezia) orbiculare in the pathogenesis of seborrheic dermatitis in patients with and without acquired immunodeficiency syndrome (AIDS) has been discussed but not resolved. Ten patients with AIDS-related seborrheic dermatitis were studied for the presence of Pityrosporum organisms. On the basis of a quantitative correlation between numbers of yeast cells adherent to and extruded from keratinocytes and the clinical severity of seborrheic dermatitis, an association, if not a causative role, for Pityrosporum is strongly suggeste

Genetic, Lipid, Fungal Microbiome and Neuroinflammatory Links Between Seborrheic Dermatitis and Parkinson's Disease: A Narrative Review.

Background: Parkinson's disease (PD) is a progressive neurodegenerative disorder with a well- documented prodromal phase during which non-motor symptoms appear years before motor onset. Seborrheic dermatitis (SD) is a chronic inflammatory skin disease that is significantly more prevalent in PD patients than in the general population. The biological mechanisms underlying this association have not been integrated into a single framework. Objectives: This narrative review aims to synthesise the available evidence on the molecular, genetic, and pathophysiological mechanisms linking SD and PD. Meth
