OBJECTIVE: Peripheral neuropathy (PN) is a complication of type 2 diabetes (T2D). Whether T2D subgroups differ in PN risk and modify the effect of intensive lifestyle intervention (ILI) on PN prevention remains unknown. We assessed 1) whether T2D subgroups were associated with the incidence of PN and 2) whether the impact of ILI compared with control on risk for PN differed by T2D subgroups.
RESEARCH DESIGN AND METHODS: We included 2,891 Look AHEAD (Action for Health in Diabetes) participants with T2D and overweight or obesity randomly assigned to ILI or diabetes support and education (DSE).

Diabetic peripheral neuropathy (DPN) is a major complication of diabetes, characterized by progressive nerve damage and debilitating pain. Neuroinflammation plays a critical role in its pathogenesis, but therapeutic options remain limited. A disintegrin and metalloprotease 17 (ADAM17) regulates inflammatory signaling, but its ubiquitous expression makes it a difficult target. This study examined the role of inactive rhomboid protein 2 (iRhom2), a cofactor essential for ADAM17 activation, in the development of DPN. Diabetes was induced in wild-type (WT) and iRhom2 knockout (KO) mice using strep

Background/Objectives: Diabetic neuropathy (DN), a common complication of type 2 diabetes mellitus, manifests as peripheral nerve dysfunction with symptoms such as fatigue. Although exercise effectively reduces fatigue in neuropathy patients, precise detection methods are crucial to elucidate the role of rehabilitation. Accordingly, this study aimed to evaluate fatigue in DN patients using a multimodal approach (clinical and instrumental) and to compare the efficacy of aerobic versus resistance training on fatigue parameters. Methods: Eligible DN inpatients admitted for rehabilitation at the N

BACKGROUND: Diabetic neuropathy has typically been viewed as a peripheral nerve disorder, most commonly presenting as distal symmetrical polyneuropathy (DSPN). However, accumulating evidence suggests that diabetes affects not only peripheral somatic and autonomic fibers but also the central nervous system, indicating more widespread neurodegenerative processes.
AIM: This narrative review aims to synthesize current knowledge on how diabetes affects the nervous system across the neuroaxis, integrating peripheral, autonomic, and central mechanisms, and to provide an overview of clinical manifest

Diabetes mellitus is frequently accompanied by gastrointestinal symptoms, but these symptoms are not fully explained by extrinsic autonomic neuropathy and should not be equated with a single enteric lesion. Human biopsy and motility studies, complemented by experimental models, indicate that the enteric nervous system (ENS) and its neuromuscular niche can be injured in segment- and cell-specific ways. Human evidence supports gastric changes in interstitial cells of Cajal (ICC) and macrophages, and colonic enteric neuronal loss associated with oxidative stress; plexus-resolved and repair mechan

Diabetes mellitus affects both the peripheral and central nervous systems, giving rise to a spectrum of neurological complications that extend far beyond the traditional focus on diabetic peripheral neuropathy (DPN). Chronic hyperglycemia disrupts cellular homeostasis through increased formation of advanced glycation end-products, activation of inflammatory pathways, mitochondrial dysfunction, oxidative stress, and impaired neurovascular regulation. These disturbances converge to drive axonal degeneration, demyelination, synaptic injury, and progressive cognitive decline. Emerging evidence als

PURPOSE: Distal symmetric polyneuropathy (DSP) is the most common diabetic neuropathy (75% of all diabetic neuropathies), potentially leading to plantar ulcers and lower limb amputation. Early detection of at-risk individuals is essential for timely intervention. This study aimed to evaluate different surface EMG (sEMG) parameters from isometric and dynamic exercises as biomarkers for early DSP detection and monitoring.
METHODS: The study involved 61 participants (34 controls and 27 cases with low, moderate and high levels of severity). sEMG signals were recorded from four lower-limb muscles …

AIM: To investigate the possible neuroprotective effect of low-intensity ultrasound (US) application against diabetic neuropathy. METHODS: Adult male wistar rats were randomly divided into three groups as control, untreated diabetic and US-treated diabetic. Diabetes was induced by a single intraperitoneal injection of streptozotocin (50 mg/kg) to the rats and verified by the presence of hyperglycemia (> 250 mg/dl). US was applied to sciatic nerve at a power density of 1.5 W/cm2 and a frequency of 1 MHz for 3 min/day for 5 weeks over a 5 cm2 skin surface. Blood glucose, insulin and HbA1c levels

Diabetes mellitus is a common chronic condition frequently complicated by distal sensorimotor polyneuropathy (DSPN), a major contributor to pain, foot ulceration, and limb loss. Despite its impact, DSPN often remains undetected until there is significant loss of protective sensation. Routine screening practices tend to identify more advanced stages of neuropathy, offering limited sensitivity for early disease. Earlier diagnosis could substantially improve outcomes by enabling timely intervention and optimised glycaemic control. A range of diagnostic modalities has been investigated for the ear

As a leading ocular disorder caused by diabetes mellitus, diabetic corneal neuropathy (DCN) is characterized by decreased corneal sensation, ocular irritation, delayed corneal wound healing and corneal neuropathic pain. Several researchers including us previously evidenced that DCN is regulated by several transcriptional and/or epigenetic factors. However, the role of N6-methyladenosine (m6A) modification, the most common internal modification in eukaryotic RNAs, in the pathogenesis of DCN remains largely unexplored. In the current work, we aimed to elucidate the pathological role of m6A modif

Diabetic neuropathy is one of the commonest complications of diabetes mellitus. Its most frequent form is diabetic peripheral neuropathy (DPN). Currently, there is no established and widely used biomarker for diagnosis and clinical staging of DPN. There is accumulating evidence that low-grade systemic inflammation is a key element in its pathogenesis. In this context, several clinical studies have so far identified potential biomarkers of DPN. These studies have enrolled both subjects with type 1 diabetes mellitus (T1DM) and subjects with type 2 diabetes mellitus (T2DM), including children wit

There is an increasing prevalence of diabetes mellitus (DM), particularly type 2 DM (T2DM), and its associated complications. T2DM is linked to insulin resistance, chronic inflammation, and oxidative stress, which can lead to both macrovascular and microvascular complications, including peripheral diabetic neuropathy (PDN). Inflammatory processes play a key role in the development and progression of T2DM and its complications, with specific markers like C-reactive protein (CRP), interleukins (ILs), and tumor necrosis factor (TNF)-α being associated with increased risk. Other key inflammatory m

Diabetic peripheral neuropathy (DPN) is a common chronic complication of diabetes mellitus. One of the most common types is distal symmetric poly-neuropathy, which begins as bilateral symmetry pain and hyperesthesia and gradually progresses into hypoesthesia with nerve fibre disorder and is frequently accompanied by depression and anxiety. Notably, more than half of patients with DPN can be asymptomatic, which tends to delay early detection. Furthermore, the study of adverse outcomes showed that DPN is a prominent risk factor for foot ulceration, gangrene and nontraumatic amputation, which dec

Diabetic peripheral neuropathy (DPN) is one of the common chronic complications of diabetes, resulting in neuropathy of spinal nerve, cranial nerve, and vegetative nerve. Diabetic distal symmetric multiple neuropathy is the most representative lesion of DPN, including symptoms of bilateral limbs pain, numbness, and paresthesia, etc. DPN is one of the main reasons causing diabetic foot ulcer (DFU). Schwann cells (SCs) are the primary glia cells of the peripheral nervous system, which play very important role in repairing after nerve injury. As the target cells of chronic hyperglycemia, SCs' fun
