Extended replacement of the thoracic aorta.

OBJECTIVES: We present our experience of total aortic arch replacement. METHODS: Twenty-nine patients (21 males and 8 females; mean age 63.3 ± 13.3 years) with extended thoracic aortic aneurysms underwent graft replacement. The pathology of the diseased aorta was non-dissecting aneurysm in 11 patients, including one aortitis and aortic dissection in 18 patients (acute type A: one, chronic type A: 11, chronic type B: six). Five patients had Marfan syndrome. In their previous operation, two patients had undergone the Bentall procedure, three had endovascular stenting, one had aortic root replac

Cardiac tamponade during transesophageal echocardiography in the patient of circumferential aortic dissection.

A 43-year-old woman, whose physical findings were consistent with Marfan's syndrome, presented with acute chest pain. Transthoracic two-dimensional echocardiography demonstrated dilated ascending aorta with a circular shape intimal flap at the root level. Subsequently, the patient required transesophageal echocardiography (TEE), but during esophageal intubation, the patient developed acute pericardial tamponade which resulted in death in spite of cardiopulmonary resuscitation. Although, some investigators recommend TEE as the first choice of diagnostic method of aortic dissection, hemodynamic

Endovascular stent-graft treatment for Stanford type A aortic dissection.

OBJECTIVE: The aim of the study is to summarise our experience of endovascular stent grafting for Stanford type A aortic dissection. DESIGN: Retrospective analysis at single centre. METHODS: From January 2001 to January 2009, we treated 45 cases of Stanford type A aortic dissection with endovascular stent grafting. The entry tear was located at the ascending aorta in 10 cases (DeBakey type I), the aortic arch in 14 cases and the distal aortic arch or proximal descending aorta in 21 cases in which the ascending aorta was also involved by the dissection. RESULTS: The surgical success rate was

Repair type I aortic dissection in a patient with human immunodeficiency virus infection.

A 57-year-old man with human immunodeficiency virus (HIV) infection was evaluated in October 1997 with complaints of weakness of the right lower extremity. A chest computerized axial tomographic scan revealed a type I aortic dissection. He underwent surgical repair including resuspension of the aortic valve and placement of a 32-mm interposition graft between the aortic root and the transverse arch. Postoperatively he required abdominal aorta fenestration and stenting for ischemia of the left lower extremity. Follow-up magnetic resonance imaging 3(1)/(2) years postoperatively showed a normal-s

Successful Endovascular Treatment of an Anastomotic Pseudoaneurysm in the Ascending Aorta.

We present a case of a 72-year-old-male with an ascending aortic anastomotic pseudoaneurysm following Dacron graft repair of an iatrogenic ascending aortic dissection that occurred during an open mitral valve repair, which was successfully treated with endovascular graft placement. In conclusion, this uncommon finding demonstrates a complex pathology and additional clinical examples of the versatility of endovascular stent grafts within the ascending aorta.

Treatment of delayed retrograde dissection after endovascular stenting of thoracoabdominal aortic aneurysm: graft-to-endograft anastomosis.

INTRODUCTION: Endovascular treatment of thoracic aortic aneurysms may be associated with procedure-related early and late complications. REPORT: We describe the case of a 70-year-old man who developed acute retrograde dissection of the ascending aorta 30 days after endovascular stent grafting of a thoracoabdominal aortic aneurysm and prior revascularization of the left subclavian artery. The ascending aorta and aortic arch were successfully replaced with a Dacron graft; the distal part of the graft was partially anastomosed to the previously placed endograft. DISCUSSION: Surgical repair of d

[Acute myocardial infarction due to an acute aortic dissection].

Myocardial ischemia secondary to dissection of the ascending aorta remains a relatively rare complication. A 76-year old man with no prior history developed sudden chest pain. The electrocardiogram showed a ST-segment elevation in leads II and III suggesting an inferior wall acute myocardial infarction. Upon arrival, he received anticoagulation and antiplatelet aggregation intravenously and orally respectively. Coronary angiography showed an anomalous origin of the right coronary and extrinsic stenosis by a false lumen. Computed tomography confirmed the diagnosis of type A aortic dissection wi

Endarterectomy and external prosthetic grafting of the ascending and transverse aorta under hypothermic circulatory arrest.

A 78-year-old woman presented with acute pulmonary edema, a blood pressure of 250/160 mmHg, and a 4/6 diastolic murmur of probable aortic origin. Aortography revealed 4+ aortic regurgitation, left ventricular dysfunction, a right coronary artery with good distal run-off but complete proximal occlusion, a fusiform aneurysm of the ascending and transverse aorta (with a transverse dissection in the left anterolateral wall of the upper ascending aorta, but no evidence of intramural lumen), and milder, isolated dilatation of the descending thoracic aorta. Upon operation, on 8 September 1987, I disc

Surgical pathology of chronic ascending aortic dissections.

AIMS: A majority of aortic dissections affect the ascending aorta and are acute in nature. Chronic dissections in this region are uncommon. This report characterises the clinicopathological features of 15 non-iatrogenic chronic ascending aortic dissections. METHODS: Among 65 patients who had surgery for ascending aortic dissection over a period of 6 years, 15 showed morphological evidence of chronic dissection. Clinical data and morphological features of these were analysed. RESULTS: Twelve patients (80%) were over the age of 50 years, with a male predominance (11 patients, 73.33%). Exertion

[Dissecting aneurysms of the aorta: from causes to therapeutic indications].

In its acute phase, aortic dissection (a term that is now preferred to dissecting aneurysm of the aorta) is a medico-surgical emergency. The diagnosis is suspected on clinical data: intensive thoracic pain with recently appeared diastolic murmur and abolition of a peripheral pulse, contrasting with an electrocardiogram that is either normal or not suggestive of acute coronary thrombosis. It is confirmed by imaging methods, notably echocardiography and/or angiography. These methods, rapidly performed in all circumstances, provide the surgeon with useful information on the size and extent of the

Intimal intussusception: unusual complication of dissecting aneurysm.

A case is reported of a 65-year-old man operated on to relieve obstruction of the left common carotid and left subclavian arteries due to a dissecting aneurysm. At operation the obstruction was found to be due to detachment of the intima from the ascending aorta followed by intussusception into the distal aortic arch. Surgical correction consisted in resection of the ascending aorta and aortic arch and replacement with a Dacron graft. Certain technical aspects of the operation were considered important in achieving a successful outcome.

[A case of combined operation of aortic valve slicing and root remodeling for ascending aortic aneurysm with aortic regurgitation].

A 56-year-old female admitted with severe back pain, and her chest computed tomography demonstrated non-dissecting sclerotic aneurysm of the ascending aorta. Aortography and echocardiography showed marked dilatation of the ascending aorta and the Valsalva sinuses resulting in disappearance of the sinotubular junction. Aortic regurgitation of grade three was, also, recognized. A combined operation of aortic valve slicing of the right and the left coronary cusps and aortic root remodeling (Yacoub's method) was successfully performed. Surgical treatment of ascending aortic complications in Marfan syndrome: early and long-term outcomes.

Findings in 54 patients (mean age 39 years, range 18-66 years, 25% female) were analyzed. Of these patients, 21 had dissection of the ascending aorta (15 acute and six chronic) and 33 had aneurysm of the ascending aorta. Surgery was classified as emergency surgery in three cases, as urgent in 15, and as scheduled surgery in 36. The Bentall-De Bono procedure was performed in 39 patients, aortic valve reimplantation was carried out in nine, Cabrol's operation was performed in three, and a homograft was used in three. The mean diameter of the ascending aorta was 66.6 mm. Overall, in-hospital mort

Mechanical properties of dilated human ascending aorta.

Dilation of the ascending aorta, associated with Marfan Syndrome, bicuspid aortic valve, or advanced age, may lead to aortic dissection and rupture. Mathematical models can be used to assess the relative importance of increased wall stresses and decreased strength in these mechanical failures. To obtain needed inputs for such models, mechanical properties of dilated human ascending aorta were measured in vitro. Specimens for opening angle, biaxial elastic, and uniaxial circumferential strength tests were cut from excised tissue obtained from 54 patients (age 18-81 years) undergoing elective ao

Long-term follow-up after separate replacement of the aortic valve and ascending aorta.

Between May 1974 and November 1991, 28 patients underwent a separate replacement of the aortic valve and the ascending aorta (20 male, eight female; 32 to 71 years old, x = 52 years). 23 patients were operated for ascending aortic aneurysm, three for chronic and two for acute aortic dissection type A. 17/18 patients living at the beginning of this study were re-investigated after a mean follow-up interval of 8.5 years postoperatively with DSA, thoraco-abdominal CT and echocardiography. 1/8 biological aortic valves and 1/20 mechanical valves had to be replaced (four years and two months postope

Mycotic Aortic Pseudoaneurysm in Zone Zero-Zero After Hybrid Therapy.

BACKGROUND: Ascending aorta mycotic pseudoaneurysms are an uncommon condition. The best treatment is controversial, including surgical treatment, endovascular treatment, or a combination of both. CASE SUMMARY: A 44-year-old man was admitted to our hospital with history of a type B aortic dissection 1 year ago. He was successfully treated with hybrid therapy, aortic cerclage, and carotid bridge plus endoprosthesis implantation. During his follow-up, an ascending aorta mycotic pseudoaneurysm was documented by computed tomography scan. Two-stage hybrid therapy with brachiocephalic trunk debranch

Ascending aorta dilatation for pulmonary atresia with ventricular septal defect: a report of three adult cases.

BACKGROUND: Patients with pulmonary atresia and ventricular septal defect (PA/VSD) are prone to progressive aortic dilation. However, there are relatively few reports of progressive development of aortic aneurysm or aortic dissection in adult patients who missed early corrective surgery. PRESENTATION OF CASES: Case 1: A 38-year-old man with PA/VSD and a bicuspid aortic valve (BAV), underwent VSD repair, aortic valve replacement, and PA correction at age 21. Seventeen years after surgery, an aortic dissection occurred in the ascending aorta, which subsequently underwent the Bentall procedure.

Abnormal aortic hemodynamics are associated with risk factors for aortic complications in patients with marfan syndrome.

BACKGROUND: It is difficult to assess the risk for aortic dissection beyond the aortic root in patients with Marfan syndrome (MFS). To aid risk assessment in these patients, we investigated aortic flow and wall shear stress (WSS) by 4D flow magnetic resonance imaging (MRI) in patients with MFS and compared the results with healthy volunteers. We hypothesized that MFS patients with a high-risk profile for aortic dissection would show abnormal hemodynamics in aortic regions associated with aortic dissection. METHODS: MFS patients (n = 55) and healthy subjects (n = 25), matched for age and sex,

Late results of operation for acute aortic dissection producing aortic insufficiency.

Since July, 1970, 8 patients with acute dissecting aneurysms of the ascending aorta producing aortic insufficiency have undergone immediate operation employing a technique in which the origin of the dissection is resected, the dissected base of the aorta is lined with nondistensible prosthetic cloth for support, and the native aortic valve is resuspended. Aortic continuity is then reestablished with end-to-end anastomosis or use of a short segment of woven graft (not necessarily circumferential). There were no operative deaths. Mean follow-up has been 60 months (range, 25 to 82 months). One la

[Therapeutic strategies for acute type A aortic dissection].

We herein present the early and mid-term outcomes of therapeutic strategies for acute type A aortic dissection in our department. Subjects were 75 patients who were admitted to our department from January 2001 to October 2006. A total of 33 patients had thrombosed dissection: emergent surgery was indicated for cases in which the maximal ascending aortic diameter was > or =50 mm or when ulcer-like projection (ULP) was observed in the ascending aorta. Only 1 case of rupture-related death was observed, in a patient who had a maximal ascending aortic diameter of 52 mm and refused surgery. Although
