BACKGROUND AND OBJECTIVES: Pre-donation deferral can negatively affect donor retention and blood supply sustainability. Comprehensive analysis of national deferral rates and reasons can inform evidence-based adjustments to donor eligibility criteria and improve the specificity of pre-donation screening.
MATERIALS AND METHODS: Data on deferral rates and the top three deferral causes were collected from 47 blood establishments (BEs) (23 provincial-level, 24 prefectural-level) across China between 2016 and 2024. Deferral reasons were classified into 16 standardized categories. Chi-square and Spe

BACKGROUND: Blood services worldwide continue to face persistent challenges in recruiting and retaining voluntary, non-remunerated blood donors, particularly among younger populations. Over the past decade, communication strategies have shifted from traditional mass media to digital environments, where social media platforms, influencers, and online communities increasingly shape social norms, identity formation, and prosocial behaviour. These digital tools are now widely used to mobilise blood donors; however, the existing evidence base remains fragmented across disciplines, regions, and stud

The demand for donated kidneys is steadily increasing, making it critical that the transplant community maximizes the use of organs procured from deceased donors. When a donated kidney becomes available, a transplant program evaluates the suitability of the organ for their patients and will choose whether to decline or accept. A pre-implantation biopsy may be performed if the transplant center is unsure about the suitability of the organ. More than half of deceased donor kidneys are biopsied prior to implantation, and "biopsy findings" is the most commonly cited reason for a kidney not being a

BACKGROUND AND OBJECTIVES: Concern over variant Creutzfeldt-Jakob disease (vCJD) led to the deferral of donors who had resided in the United Kingdom since January 1980. This deferral was implemented in 1999 and subsequently modified to include other countries. Some deferrals were removed in February 2022; deferrals for the United Kingdom, Ireland and France were removed on 22 November 2023. In this study, we describe efforts made to encourage donation from newly eligible people and the resulting donation gain.
MATERIALS AND METHODS: Actions targeted individual donors deferred after 1 January

BACKGROUND AND OBJECTIVES: In Québec (Canada), the donation deferral for men who have sex with men (MSM) has recently been shortened to 3 months. Whether this change impacted compliance with pre-donation screening is unknown. We assessed compliance with the disclosure of male-to-male sex and other behavioural risk factors for HIV amid this change.
MATERIALS AND METHODS: Québec residents who donated from 14 July 2020 to 30 November 2020 were invited to participate in an online survey. Donors were informed that the survey was optional and anonymous. Survey questions were those used for routine

BACKGROUND: Due to the increasing demand for kidney transplants, sometimes donors with underlying medical conditions can be considered for living kidney donor transplant. Thalassemia is amongst the most common inherited disorders of hemoglobin globally, which is not restricted as an exclusion criterion. However, there is currently no study examine the safety and characteristics of kidney donors with thalassemia minor.
METHODS: All eligible live kidney donors between 2016 and 2019 with thalassemia minor at a tertiary hospital were recruited. Baseline characteristics, clinical and laboratory ou

We surveyed US transplant centers to assess practices regarding the evaluation and selection of living kidney donors based on metabolic, cardiovascular, and substance use risk factors. Our companion article describes renal aspects of the evaluation. Response rate was 31%. Compared with 2005, programs have become more accepting of hypertensive candidates: 65% in 2017% vs 41% in 2005 consider candidates with hypertension well controlled with 1 medication. One notable exception is black hypertensive candidates, who are frequently excluded regardless of severity. The most common body mass index (B

BACKGROUND: Determination of blood donor hemoglobin (Hb) levels is a pre-requisite to ensure donor safety and blood product quality. We aimed to identify Hb measurement practices across blood donation services and to what extent differences associate with low-Hb deferral rates.
METHODS: An online survey was performed among Biomedical Excellence for Safer Transfusion (BEST) Collaborative members, extended with published data. Multivariable negative-binomial regression models were built to estimate adjusted associations of minimum donation intervals, Hb cut-offs (high, ≥13.5 g/dL in men or ≥ 12

OBJECTIVES: To study the main causes of pre-donation blood donors' deferral in a Brazilian blood bank.
BACKGROUND: Blood donor selection is the most important process to maintain transfusion safeness. Pre-donation deferral aims to avoid the transmission of infectious diseases in the serological window, as well as to preserve blood donors' health.
METHODS: This was a retrospective study undertaken in a single blood centre in Curitiba, Brazil, taking into account the number of blood donations per year, the number of annual donations by gender and the total number of blood donors deferred annua…

BACKGROUND/CASE STUDIES: Despite decreased blood use, there are supply limitations. Therefore, it is important to determine the number of people potentially eligible to donate blood. This study updates an analysis from 2007 to estimate the pool of eligible blood donors in the United States.
STUDY DESIGN/METHODS: We developed a revised epidemiologic model to account for changes in donor exclusion factors based on AABB standards. Donor exclusionary factors were identified and epidemiologic databases selected to enumerate the population prevalence of the donor exclusion factors. Prevalence data

Blood donors attending a donation session may be deferred from donating blood due to a failure to meet low hemoglobin (Hb) thresholds. This costs the blood donor service and donors valuable time and resources. In addition, donors who are deferred may have more symptoms, and as a direct and/or indirect effect of their experience, return rates of donors deferred for low Hb are reduced, even in repeat donors. It is therefore vital that low Hb deferral (LHD) is minimized. The aim of this updated systematic review is to expand the evidence base for factors which affect a donor's risk of deferral du

AIMS/OBJECTIVES: To describe the impact of additional testing on the England blood supply.
BACKGROUND: The blood service for England, NHS Blood and Transplant, applies a system of deferral and testing to donors with potential exposure to Chagas disease, malaria and West Nile virus; however, testing costs must be justified. Here, we describe the donations and donors gained by testing.
METHODS: Donation testing results and demographic data on donors in England where additional testing was applied were analysed in 2012-2016. The total number and proportion of donations tested, reactive and conf

Zika virus (ZIKV) is a mosquito-borne flavivirus that is the focus of an ongoing pandemic. ZIKV is notable for its severe neurologic sequelae in babies born to infected mothers. High rates of subclinical infection, as evidenced by the finding of ZIKV RNA in asymptomatic donors, raise concerns of risk to the blood supply. To date, a total of four suspected cases of transfusion-transmitted ZIKV have been reported (all in Brazil), none of which were associated with clinical infection in the transfusion recipients. In 2016, the US Food and Drug Administration issued a guidance mandating national b

Kidney chains are a recent and novel method of increasing the number of available kidneys for transplantation and have the potential to save thousands of lives. However, because they are novel, kidney chains do not fit neatly within existing legal and ethicalframeworks, raising potential barriers to their full implementation. Kidney chains are an extension of paired kidney donation, which began in the United States in 2000. Paired kidney donations allow kidney patients with willing, but incompatible, donors to swap donors to increase the number of donor/recipient pairs and consequently, the nu

BACKGROUND: Transfusion-related acute lung injury (TRALI) is known as a life-threatening complication of transfusion. HLA and HNA antibodies have been associated with the immune pathway of TRALI. Since donors with a history of transfusion and/or pregnancy are presumed to have an increased risk of carrying such antibodies, we investigated the association of a history of transfusion or pregnancy with the occurrence of HLA alloimmunization in our donor population.

BACKGROUND AND OBJECTIVES: Inaccuracy of fingerstick haemoglobin compromises donor's health and losses blood donations. We evaluated the benefit of double haemoglobin screening with HemoCue.
STUDY DESIGN AND METHODS: Blood donors underwent fingerstick screening by HemoCue and were driven for donation if capillary haemoglobin was within the regulatory range. Those failing were drawn venous blood and donated if their venous haemoglobin determined with HemoCue was acceptable.
RESULTS: Of 276 605 donor clinic visits, 10 011 (3·6%) were assessed by two-step haemoglobin screening using HemoCue, be
