BACKGROUND: Endoscopically placed duodenal stents are commonly performed procedures for palliation of obstruction due to malignancy. A relatively small number of studies highlight the potential complications of this procedure, and to date no data have been published in New Zealand specifically addressing this issue. Duodenal ulcer is a common disease and is considered acid-peptic disease. Duodenal ulcer patients are known to have higher incidence of antral gastritis and duodentitis than general population. Thus one can expect some alteration in the enzyme products of mucosal cells of antrum and duodenum in this condition. A mutagenicity of gastric and duodenal contents in regard to gastric carcinogenesis was estimated in animals with duodenogastric reflex. Male Wistar rats were subjected to one of following four types of operative procedures: 1) Reflux through the pylorus (DGR), b) DGR + truncal vagotomy (DGR + V), 2) Control group; a) sham operation treated with gastrotomy (SO), b) SO + truncal vagotomy (SO + V). No carcinogens were given to animals, and they were killed 50 weeks after surgery. No cancers were detected in any of 25 animals in the control group, but nine (41%) of 22 animals in the reflux group

Bleeding posterior duodenal ulcers is a critical problem especially in patients of advanced age. Surgical treatment should obtain efficient and stable hemostasis and avoid digestive fistulas due to leakage of the duodenal stump. The main difficulty lies in the closure of the duodenal stump. A procedure using intramural dissection of the duodenum and closure by means of a stapler is reported for a series of 40 patients without recurrent bleeding or duodenal fistula. An infant who presented with signs of duodenal atresia had proximal duodenal obstruction by upper gastrointestinal barium study, but had a small amount of air in the distal bowel. Exploration showed an annular pancreas and microperforation of a duodenal diaphragm. Prior to concluding that pancreaticobiliary duct anomalies are the path of air into the distal bowel in patients with duodenal atresia, microperforation of a duodenal diaphragm must be excluded.

Herpes simplex virus (HSV) esophagitis and duodenitis are well-known features of herpes virus reactivation in immunosuppressed patients. We discuss the case of HSV esophagitis concomitant with duodenitis in an immunocompetent individual. A 77-year-old male patient, previously healthy, was found to have on upper endoscopy several ulcers in the esophagus and second portion of the duodenum. Acute ischemic pancreatitis and duodenitis are uncommon diagnoses that can affect patients with shock. As respiratory, cardiac, and neurological manifestations of shock are prioritized, resultant gastrointestinal (GI) related pathologies can be overlooked, leading to underdiagnosis of these conditions. In this report, we highlight the importance of recognizing acute ischemic pancreatitis and duodenitis in our description of a unique patient presenting with GI related complaints with underlying cardiogenic shock.

The effects of acid, infectious (Helicobacter pylori) and neuroendocrine factors on the course and outcome of chronic duodenitis were studied in 57 patients aged 15-35 years.
