The differential diagnosis between the usual acute otitis externa and acute suppurative otitis media is presented. Other important and sometimes frequently seen entities that present as infections in the ear are also discussed. It is emphasized that a patient complaining of ear pain with normal findings on examination must be more thoroughly investigated because of the possibility of a more ominous condition such as carcinoma of the pharynx.

Surgical trauma inflicted upon the membranous labyrinth is an uncommon, but often catastrophic, complication of chronic ear surgery. Areas most vulnerable to injury are the oval window and the lateral semicircular canal. Methods of injury include subluxation of the footplate, fracture of the lateral canal by drill or chisel, and accidental opening of a fistula. Middle ear fluid is a common problem in children. Eustachian tube dysfunction has been considered the key to the pathogenesis of this disorder, but new studies show that other factors may be operative. Inflammation appears to play a significant role in high-viscosity effusion, and these effusions may be related to acute suppurative otitis media. Since persistent mild hearing loss may adversely affect verbal development, new emphasis on early treatment of middle ear effusion is warranted.
