[Clinical observation of traumatically-damaged joint after its repair with transplantation of allogenic joint].

OBJECTIVE: To evaluate the long-term function of the traumatically-damaged joint after its repair with transplantation of a fresh or a frozen allogenic joint. METHODS: From March 1977 to September 1993, 13 patients (9 males, 4 females; age, 17-55 years) with traumatically-damaged joints underwent transplantation of the fresh or the frozen allogenic joints. Five patients had 5 damaged metacarpophalangeal joints, 6 patients had 9 damaged interphalangeal joints, and 2 patients had 2 damaged elbow joints. So, the traumatic damage involved 13 patients and 16 joints. All the metacarpophalangeal joi

Tuberculosis Septic Arthritis of the Elbow: A Case Report and Literature Review.

Tuberculosis (TB) infections of the musculoskeletal system are rare. A 77-year-old female with chronic left elbow pain for five months was treated by irrigation and debridement of the elbow for a presumed diagnosis of septic arthritis. Her pain and wound condition did not improve, and she was referred to our institution. Plain radiograph and magnetic resonance imaging (MRI) revealed an osteolytic lesion with joint effusion and severe destruction of the elbow joint. We suspected an atypical infection of the elbow due to the chronicity, negative culture results and severe osteoarticular destruct

Pigmented Villonodular Synovitis of Ankle Joint - A Case Report.

INTRODUCTION: Neglected elbow dislocation is defined as a dislocation that is present for 3 weeks or longer. There is no available literature on neglected elbow dislocations, leading to ankylosis of the elbow joint. CASE REPORT: A 17-year-old female sustained a road traffic accident. Anteroposterior and lateral view radiographs of the forearm were done and deemed to be normal at another hospital. Six months after the injury, she presented to us with complaints of absent elbow movements. Examination revealed the elbow to be fixed in 30° of flexion. Radiographs of the elbow revealed an ankylosi

In vivo 3-dimensional Kinematics of Cubitus Valgus after Non-united Lateral Humeral Condyle Fracture.

BACKGROUND: Nonunion of lateral humeral condyle fracture causes cubitus valgus deformity. Although corrective osteotomy or osteosynthesis can be considered, there are controversies regarding its treatment. To evaluate elbow joint biomechanics in non-united lateral humeral condyle fractures, we analyzed the motion of elbow joint and pseudo-joint via in vivo three-dimensional (3D) kinematics, using 3D images obtained by computed tomography (CT) scan. METHODS: Eight non-united lateral humeral condyle fractures with cubitus valgus and 8 normal elbows were evaluated in this study. CT scan was perf

Arthroscopic treatment of lateral epicondylitis: clinical and cadaveric studies.

Clinical and cadaveric studies were done to investigate the role of a degenerative fringe of the radiocapitellar complex subluxating in the radiocapitellar joint in patients with lateral epicondylitis. In the clinical study, arthroscopic resection of this capsular complex was done. Thirty patients with recalcitrant symptoms of lateral epicondylitis for a minimum of 9 months had surgery. In all patients at arthroscopy, a collar-like band of radiocapitellar capsular complex was found to impinge on the radial head and subluxate into the radiocapitellar joint with manipulation under direct vision.

Elbow proprioception is normal in patients with a congenital absence of functional muscle spindles.

KEY POINTS: Individuals with hereditary sensory and autonomic neuropathy type III (HSAN III), also known as Riley-Day syndrome or familial dysautonomia, do not have functional muscle spindle afferents but do have essentially normal cutaneous mechanoreceptors. Lack of muscle spindle feedback from the legs may account for the poor proprioception at the knee and the ataxic gait typical of HSAN III. Given that functional muscle spindle afferents are also absent in the upper limb, we assessed whether proprioception at the elbow was likewise compromised. Passive joint angle matching showed that prop

Kinematics and laxity of a linked total elbow arthroplasty following computer navigated implant positioning.

Aseptic loosening in total elbow arthroplasty (TEA) remains the most common cause of long-term failure. While several different mechanisms of implant loosening have been suggested, it is likely that one important underlying cause is implant malpositioning, resulting in changes in joint kinematics and loading. Although use of computer navigation has been shown to improve component positioning in other joints, no such system currently exists for the elbow. This study used real-time computer feedback for humeral, ulnar, and radial component positioning in 11 cadaveric extremities. An elbow motion

Osteoarthritis of Proximal Radioulnar Joint: Computed Tomography-Based Assessment and Associations with Clinical Findings.

BACKGROUND: Although many studies have focused on the degenerative changes of the ulnohumeral and radiohumeral joints in osteoarthritis (OA) of the elbow, the proximal radioulnar joint (PRUJ), which facilitates the pronation and supination motions of the forearm, has not been comprehensively evaluated. PURPOSE: To assess the prevalence of PRUJ OA in patients diagnosed with OA of the elbow using computed tomography (CT) images and to establish an association between the CT findings and clinical manifestations. STUDY DESIGN: Cross-sectional study; Level of evidence, 3. METHODS: Between 2010 a

Osteomyelitis variolosa.

We present a patient with osteomyelitis variolosa in both elbow joints with short middle fingers-sequelae of smallpox. Both elbow joints showed sclerosis of bones around elbow joint, enlargement of olecranon process of ulna, and head of radius with intact joint space. Hands showed shortening of both middle fingers following dactylitis leading to short metacarpals. The purpose of this report is to remind those who seldom see one of the most important and easily recognized complications of smallpox.

Elbow stiffness: Interview with professor Bernard Morrey.

Stiff elbow is a complex condition whose diagnosis and management are sometimes quite a challenge. Compared to the other joints, the elbow is disproportionately affected by loss of motion following trauma or surgery. It is unclear why the elbow tends to develop stiffness; its anatomical complexity, namely the presence of three highly congruent joints in the same capsule and synovial space, the tautness of the lateral and medial collateral ligaments through the whole range of motion, and the very close relationship among tendons, muscles, and skin 2 may account for this characteristic. In a sti

[Arthroscopic capsular release of the elbow].

Limited mobility of the elbow joint can be due to various factors, the most common of which is trauma to the elbow. In addition, limited elbow movement can be a direct cause of decreased functionality and interfere with everyday life activities. The majority of the patients can recover a functional range of motion if physical therapy is combined with a splinting regimen. However, some patients will experience continued elbow stiffness. Traditionally, open techniques have been successful in releasing the elbow joint capsule and regaining functional range of motion. Early results of arthroscopic

Subacute dislocation of the elbow following Galeazzi fracture-dislocation of the radius: A case report.

INTRODUCTION: The Galeazzi fracture-dislocation was originally described by Sir Astley Cooper in 1822 but was named after Italian surgeon Ricardo Galeazzi in 1934. It is an injury classified as a radial shaft fracture with associated dislocation of the distal radioulnar joint and disruption of the forearm axis joint. The associated distal radioulnar joint injury may be purely ligamentous in nature, tearing the triangular fibrocartilaginous complex, or involve bony tissue (that is, ulnar styloid avulsions) or both. We report this case because of the rare association of posterior dislocation of

Exploring elbow kinematics in the central bearded dragon (Pogona vitticeps) using XROMM: Implications for the role of forearm long-axis rotation in non-avian reptile posture and mobility.

The functional morphology and kinematics of the elbow joint remain relatively understudied in squamates. Previous investigations of lizard elbow morphology and kinematics suggest long-axis rotation (LAR) of the radius and ulna during stance allows the manus to remain pronated during forelimb retraction. Using XROMM (X-ray Reconstruction of Moving Morphology), we explored the range of 3D movements and kinematics of the humerus, radius, and ulna in three adult male Central bearded dragons (Pogona vitticeps) during trackway walking. Our data indicate that the elbow joint of P. vitticeps experienc

[Essex-Lopresti lesion is often undiagnosed].

In this review, the diagnostics of the Essex-Lopresti lesion is discussed. The lesion is a combination of fracture of the head of radius, rupture of the interosseous mebrane (IOM) and dislocation of the distal radioulnar joint. The lesion is often overlooked in the primary clinical settings, and it is correlated to severe instability, osteoarthritis and pain if untreated. The tools for diagnosing the condition are physical examination of relevant anatomical structures, radiographs of elbow and wrist and ultrasonic evaluation of IOM as well as peroperative tests of stability. The treatment is p

Increased contact pressure at the radiocapitellar joint with anterior and posterior osteochondral defects in the sagittal section of the humeral capitellum: a cadaveric study.

BACKGROUND: In osteochondritis dissecans of the humeral capitellum, defect location and size were important in surgical planning. Recently, it was reported that posterior or large osteochondral defects of the humeral capitellum on pre-operative sagittal computed tomography affected outcomes of arthroscopic débridement for osteochondritis dissecans in adolescent baseball players. Here, we investigated radiocapitellar joint contact pressure for anterior and posterior osteochondral defect positions in the sagittal section of the humeral capitellum. METHODS: An anterior or posterior osteochondral

Improved Understanding of Traumatic Complex Elbow Instability.

Recent advancements in surgical treatment have improved clinical results in complex traumatic elbow injury. There is increasing recognition that conservative treatment and inadequate surgical fixation carry high risk of substantial morbidity in many of these cases. Recent literature displays improved outcomes in complex elbow instability, in part, because of a more complete comprehension of the injury patterns and fixation methods. Prompt surgical management with stable internal fixation, which permits immediate postoperative mobilization, has been a consistent variable across the reports lead
