Mechanically Stable Intraspinal Microstimulation Implants for Human Translation.

The goal of this study was to develop stable intraspinal microstimulation (ISMS) implants for use in humans to restore standing and walking after spinal cord injury. ISMS electrically activates locomotor networks within the lumbar region of the spinal cord. In animals, ISMS produced better functional outcomes than those obtained by other interventions, and recent efforts have focused on translating this approach to humans. This study used domestic pigs to: (1) quantify the movements and length changes of the implant region of the spinal cord during spine flexion and extension movements; and (2

Intraoperative Stereotactic Magnetic Resonance Imaging for Deep Brain Stimulation Electrode Planning in Patients with Movement Disorders.

OBJECTIVE: Implantation of deep brain stimulation (DBS) electrodes requires stereotactic imaging. Stereotactic magnetic resonance imaging (MRI) for DBS surgery has become more popular and intraoperative MRI scanners have become more available. We report on our cohort of movement disorder patients who underwent intraoperative stereotactic MRI-only DBS electrode implantation. METHODS: A review of our DBS database for eligible patients over a study period of 8 years was performed. Stereotactic accuracy was calculated as a directional error and the Euclidean distance between planned and controlle

Infections in Deep Brain Stimulator Surgery.

INTRODUCTION: Deep brain stimulation has emerged as an effective treatment for movement disorders such as Parkinson's disease, dystonia, and essential tremor with estimates of >100,000 deep brain stimulators (DBSs) implanted worldwide since 1980s. Infections rates vary widely in the literature with rates as high as 25%. Traditional management of infection after deep brain stimulation is systemic antibiotic therapy with wound incision and debridement (I&D) and removal of implanted DBS hardware. The aim of this study is to evaluate the infections occurring after DBS placement and implantable gen

Percutaneous retroperitoneal stimulation of the sacral plexus. Initial report and technical note.

A new percutaneous approach to stimulate the sacral plexus for the treatment of pain is described. The percutaneous electrode is inserted under fluoroscopy through the chosen dorsal and ventral sacral foramen and is advanced into the retroperitoneal layer where the plexus lies. The electrode, can be advanced several centimeters and lies in close proximity to the branches of the sacral plexus. Both motor and sensory responses can be elicited with electrical stimulation. Our initial experience with 4 implanted patients is presented and discussed. Several areas of the nervous system have been tar

Auditory performance of children with cochlear ossification and partial implant insertion.

The management of the profoundly deaf child with a cochlear implant poses a special challenge, particularly when total ossification of the cochlea is present. In this setting, insertion of an electrode array into a child's cochlea is often difficult. Our experience supports the feasibility of partial insertion of a multichannel implant into the basal turn of an ossified cochlea. Five children with ossified cochleae who had undergone partial implantation of a multichannel electrode were compared with the performance of matched controls who had full insertion of multichannel implants. No dramati

Gracilis muscle as neoanal sphincter for faecal incontinence.

Faecal incontinence is a debilitating chronic clinical condition which may affect the patient and care givers. Modality of treatment is based on severity of the symptoms as well as the anatomical defect itself, availability of resources and expertise. We describe a modified technique of dynamic graciloplasty as neoanal sphincter for the treatment severe faecal incontinence who has failed previous over lapping sphincteroplasty. In our modified version, instead of using implanted intramuscular electrodes and subcutaneous neurostimulator to provide continuous stimulation, the patient will undergo

Transmission of Creutzfeldt-Jakob disease to a chimpanzee by electrodes contaminated during neurosurgery.

Stereotactic multicontact electrodes used to probe the cerebral cortex of a middle aged woman with progressive dementia were previously implicated in the accidental transmission of Creutzfeldt-Jakob disease (CJD) to two younger patients. The diagnoses of CJD have been confirmed for all three cases. More than two years after their last use in humans, after three cleanings and repeated sterilisation in ethanol and formaldehyde vapour, the electrodes were implanted in the cortex of a chimpanzee. Eighteen months later the animal became ill with CJD. This finding serves to re-emphasise the potentia

[Implantation technics for long-term recording of the compound action potentials in the guinea pig, Cavia porcellus. A comparison of methods].

Long-term recordings of the compound action potential of the acoustic nerve of young, pigmented guinea pigs were carried out by two different techniques of implantation. The electrodes were inserted either by way of the mastoid process into the bony recess above the round window or they were introduced into the facial canal via the stylomastoid foramen. Before and after each implantation, special tests (Preyer-reflex, otoscopy, impedance audiometry) were performed for preliminary selection of the animals and to discover postoperative induced disturbances of sound conduction in the middle ear.

Decoding phantom limb movements from intraneural recordings.

Limb loss causes severe sensorimotor deficits and often necessitates prosthetic devices, particularly in lower-limb amputees. Although direct neural recording from residual nerves offers a biomimetic route for prosthetic control, low signal amplitudes and challenges in nerve interfacing have limited adoption. Intraneural multichannel electrodes provide a potential solution by enabling access to motor signals from muscles lost after amputation. Here, we report intraneural recordings from two transfemoral amputees using transversal intrafascicular multichannel electrodes implanted in distal bran

Completed FDA feasibility trial of surgically placed temporary diaphragm pacing electrodes: A promising option to prevent and treat respiratory failure.

BACKGROUND: Etiologies contributing to failure to wean from mechanical ventilation (MV) are multiple, resulting in morbid and costly outcomes. Diaphragm pacing (DP) is used in spinal cord injury to replace MV. Temporary DP could be utilized in acute respiratory failure to reduce MV. METHODS: A prospective FDA feasibility trial evaluated temporary DP electrodes implanted in each hemi-diaphragm during a subject's primary procedure. Objectives included: ability to provide ventilation, stability analysis with diaphragm electromyography, and adverse event monitoring. RESULTS: Twelve patients unde

Combining ENG and EEG integrated analysis for better sensitivity and specificity of neuroprosthesis operations.

Combining non-invasive monitoring of action-related brain signals with the invasive recordings of the nerve motor output could provide robust natural and bidirectional multimodal Brain-Machine interfaces. One 26 years old, right-handed male who had suffered traumatic trans-radial amputation of the left arm was connected in a bidirectional way with a robotic hand prostheses. Cortical signals related with movement programming, execution, and feed-back were recorded by non-invasive scalp electrodes to detect high-level information (i.e. onset of movement intention), while the efferent neural acti

Steroid-eluting epicardial pacing electrodes: six year experience of pacing thresholds in a growing pediatric population.

Indications for pacemaker implantation in the pediatric population often include sinus or atrioventricular node dysfunction following surgery for congenital heart defects. However, patient size, cardiac defects, and vascular and valvular concerns may limit transvenous lead utilization. Since the epicardial surface of these patients often exhibits variable degrees of fibrosis from scar tissue formation or pericardial adhesions, chronic low output (2.5/1.6 V, 0.3 ms) epicardial pacing from implant is not currently recommended in children due to frequent threshold changes and electrode exit block

Vestibular Implant Surgery: How to Deal With Obstructed Semicircular Canals-A Diagnostic and Surgical Guide.

BACKGROUND: A vestibular implant can partially restore vestibular function by providing motion information through implanted electrodes. During vestibular implantation, various obstructions of the semicircular canals, such as protein deposits, fibrosis, and ossification, can be encountered. The objective was to explore the relationship between preoperative imaging and intraoperative findings of semicircular canal obstruction and to develop surgical strategies for dealing with obstructions of the semicircular canal(s) in patients eligible for vestibular implantation. METHODS: Patients undergoi

Decoding four hand gestures with a single bipolar pair of electrocorticography electrodes.

Objective.Electrocorticography (ECoG) based brain-computer interfaces (BCIs) can be used to restore communication in individuals with locked-in syndrome. In motor-based BCIs, the number of degrees-of-freedom, and thus the speed of the BCI, directly depends on the number of classes that can be discriminated from the neural activity in the sensorimotor cortex. When considering minimally invasive BCI implants, the size of the subdural ECoG implant must be minimized without compromising the number of degrees-of-freedom.Approach.Here we investigated if four hand gestures could be decoded using a si

Evidence of transcranial direct current stimulation-generated electric fields at subthalamic level in human brain in vivo.

BACKGROUND: Transcranial direct current stimulation (tDCS) is a promising brain modulation technique for several disease conditions. With this technique, some portion of the current penetrates through the scalp to the cortex and modulates cortical excitability, but a recent human cadaver study questions the amount. This insufficient intracerebral penetration of currents may partially explain the inconsistent and mixed results in tDCS studies to date. Experimental validation of a transcranial alternating current stimulation-generated electric field (EF) in vivo has been performed on the cortica
