A recent analysis of a group of patients with central pontine myelinolysis (CPM) disclosed that a rapid rise in serum sodium from a hyponatremic baseline preceded the clinical onset of the disorder. To test the view that electrolyte derangements may be crucial in the pathogenesis of CPM, rats were given hypertonic saline following a three-day period of hyponatremia. Symmetrical, predominantly demyelinative lesions were found the neocortex, claustrum corpus striatum, external capsule, anterior commissure, hippocampus and its fimbria, thalamus, brainstem, tegmentum, and superior vermis of the ce

We report a patient with tuberculosis treated with a five-drug regimen who experienced severe acid-base and electrolyte abnormalities including hypomagnesemia, hypokalemia, hypocalcemia, and a hypochloremic metabolic alkalosis. These disturbances are believed to be due to treatment with capreomycin, which produced renal magnesium wasting and possible tubular damage. Therefore, we recommend frequent determinations of serum electrolytes, magnesium, and calcium in patients treated with capreomycin.

A defect in colonic absorption of electrolytes and water was demonstrated in patients with tropical sprue by perfusing the colon with normal saline containing a non-absorbable marker. Colonic water absorption correlated negatively with stool weight and was abnormal in patients with steatorrhoea. The possible mechanisms producing this defect are discussed. This defect may be related to colonocyte damage produced by unabsorbed unsaturated fatty acids in patients with steatorrhoea.

The functional capacity of the kidney decreases with advancing age. We not only find a reduced glomerular filtration rate but also disturbances in the functioning of the tubules which influence the capacity to dilute and concentrate the urine and the renal electrolyte excretion. In addition, the plasma-renin activity shows decreased response to stimulation; the aldosterone and vasopressin secretions are also decreased in old age. Disturbances of the water-, electrolyte and acid-base equilibrium often cause secondary renal failure. On the other hand, as the renal capacity is limited, renal diso

Results of examination of 257 patients suffering from diabetes mellitus demonstrated a reduction of sodium, potassium and magnesium content in their plasma. Erythrocytes also displayed a reduction of potassium and magnesium, and some increase of sodium content. Reduction of electrolyte level in the plasma and erythrocytes was more pronounced in winter; the erythrocyte sodium content was less in summer than in winter. The 24-hour sodium potassium and magnesium excretion is increased, during winter in particular. The efficacy of treatment of diabetic patients with panangin and potassium chloride

Eighty-eight cases with water and electrolyte disturbances, including polyuria, hyper- or hyponatremia, were found in 1,000 cases of surgically treated cerebral aneurysms and 80 nonoperated cases. In this paper, the clinical courses of the 88 cases were studied and an investigation was made of the hypothalamic lesions seen in ten autopsy cases. Water and electrolyte disturbances were most numerous in cases of anterior communicating aneurysms, and the prognosis was poor. Those with hypernatremia had a poor prognosis, with a 42% mortality rate during hospitalization. In contrast, the mortality r

In two patients with severe hypertension and moderately severe renal insufficiency, metolazone and furosemide were used in combination with propranolol, methyldopa, and hydralazine to augment control of blood pressure. This combination of diuretics also was used in five patients with refractory congestive heart failure. The patients developed severe electrolyte disturbances with a general pattern of hyponatremia. disproportionate hypochloremia, alkalosis, and phyokalemia. These abnormalities were transient in the patients with severe hypertension and moderately severe renal insufficiency. Effe

Brief outlines of these problems serve to suggest some etiologic factors in their production. The patient seldom presents with a single or simple cause or solution. The body reacts with compensations or side effects, and suffers complications and mixed disorders -- all of which probabilities must be investigated and appreciated before rational therapy can be initiated.
