Implementation of a standardized electronic medical record-integrated alternating hyperosmolar therapy protocol in neurocritical care: a single-center retrospective feasibility study.

BACKGROUND: Hyperosmolar therapy is a cornerstone of the medical management of cerebral edema and suspected intracranial hypertension. Despite widespread use of hypertonic saline and mannitol, substantial variability exists in agent selection, dosing, laboratory monitoring, treatment duration, and discontinuation practices. Standardized institutional protocols may improve consistency of care; however, reports describing their implementation in routine neurocritical care remain limited. OBJECTIVE: To evaluate implementation of a standardized electronic medical record (EMR)-integrated alternati

Association Between Electronic Patient Portal Enrollment and HIV Care Outcomes Among People Living with HIV at an Urban Academic Medical Center.

Electronic patient portals are associated with improved health outcomes in primary care settings. We assessed the relationship between patient portal enrollment and retention in care and HIV viral suppression in an urban HIV care clinic. In multivariable models, people with HIV with an active portal account had higher odds of HIV viral suppression (Odds Ratio [OR]: 2.02, 95% CI [1.3-3.7]) and higher odds of retention in care (OR: 2.2 95% CI[1.4-3.4]) than those without an active portal account. Our findings suggest that portal enrollment is associated with improved HIV care outcomes among peop

Unveiling the Enigma: Exploring Risk Factors and Mechanisms for Psychotic Symptoms in Alzheimer's Disease through Electronic Medical Records with Deep Learning Models.

Around 50% of patients with Alzheimer's disease (AD) may experience psychotic symptoms after onset, resulting in a subtype of AD known as psychosis in AD (AD + P). This subtype is characterized by more rapid cognitive decline compared to AD patients without psychosis. Therefore, there is a great need to identify risk factors for the development of AD + P and explore potential treatment options. In this study, we enhanced our deep learning model, DeepBiomarker, to predict the onset of psychosis in AD utilizing data from electronic medical records (EMRs). The model demonstrated superior predicti

Analysis of HLA Variants and Graves' Disease and Its Comorbidities Using a High Resolution Imputation System to Examine Electronic Medical Health Records.

Hyperthyroidism is a prevalent endocrine disorder, and genetics play a major role in the development of thyroid-associated diseases. In particular, the inheritance of HLA has been demonstrated to induce the highest susceptibility to Graves' disease (GD). However, thus far, no studies have reported the contribution of HLA to the development of GD and the complications that follow. Thus, in the present study, to the best of our knowledge, for the first time, a powerful imputation method, HIBAG, was used to predict the HLA subtypes among populations with available genome-wide SNP array data from

Variation of treatment decision-making for laryngeal cancer in Japan a retrospective observational study using a claim- and electronic medical record-based database.

BACKGROUND: Primary curative treatment of resectable laryngeal cancer includes surgery and chemoradiotherapy. Other treatment options include radiotherapy, chemoradiotherapy, partial laryngectomy, and total laryngectomy. Larynx-preserving treatments (partial laryngectomy, radiotherapy, and chemoradiotherapy) are recommended in early stage cases, although total laryngectomy is needed in advanced cases. Here, we evaluated trends in treatment strategies and identified the factors that are important in decision making. METHODS: We performed a retrospective analysis of data acquired from electroni
