The aim of this work was to estimate excessive somnolence and frequency of episodes of dozing off during driving a car in patients with obstructive sleep apnea. Result of questionnaire and polysomnographic investigations were analysed in a group of 503 patients. Mean age for the whole group was 49.2 +/- 9.7 of years, BMI 32.1 +/- 1.4 kg/m2 and the value for apnea and hypopnea index 45.4 +/- 24.8. PURPOSE/METHODS: To investigate whether quantitative head and eye movement recordings can distinguish patients with spasmus nutans from patients with retinal diseases mimicking spasmus nutans. Three cases of rebound positional nystagmus (RPN) are discussed in the present study. In all cases, spontaneous nystagmus was absent in the primary position, but positional nystagmus appeared in the side down position, and then nystagmus in the reversed direction appeared when the patient was returned to the primary position. We report two cases of convergence-retraction nystagmus due to cerebral infarction and hemorrhage. The electro-oculogram in 52 patients with a suspected malignant melanoma of the choroid or ciliary body was plotted in a diagram constructed for the differential diagnosis of malignant melanoma, metastasis, naevus and retinal detachment. Severe deficits of foveal smooth pursuit and optokinetic nystagmus in all directions were electro-oculographically recorded in an 80 year old woman. Magnetic resonance imaging (MRI) showed an infarct involving the postero-inferior part of the vermis (lobules VI to X) and a portion of the left cerebellar hemisphere, with apparent preservation of the flocculus and the brainstem. The role of the vermal lesion in these pursuit eye movement disorders is discussed.

Two out of 20 patients treated for metastatic or non-metastatic breast cancer with tamoxifen (20 mg 2 dd) developed bilateral secondary tapetoretinal degeneration. In one patient in whom the medication was stopped there was still slight progression of degeneration afterwards. In the other patient tamoxifen was continued under strict ophthalmological control. The total doses have been 7.3 g and 43.8 g respectively.

The dynamics of vestibular nystagmus were measured in 42 military student pilots (age 19 to 21). Their responses were compared with 40 non-flying subjects of similar age who were also fulfilling requirements for the mandatory Swiss military service. The following differences emerged: the time constant of the nystagmus response after vestibular stimulation was shorter (p less than 0.001; t-test) in student pilots, whereas the gain tended to be higher (p less than 0.025). The syndrome characterized by acquired micrognathia, hypersomnia and periodic apneas during sleep is a rare consequence of mandibular underdevelopment. Sleep of 6 depressed patients with hypersomnia was studied during their depressed phase and their remitted phase using 24-h polygraphic recording. Nine normal subjects were studied as the controls.
