OBJECTIVE: To determine the incidence of catheter-related surgical emergencies during pediatric interventional catheterization procedures.
STUDY DESIGN: We reviewed all interventional catheter procedures (n = 578) over a 4-year period (April 1996 to April 2000) to determine any complication during interventional catheterization that required surgery within 24 hours after catheterization.
RESULTS: The overall incidence of surgical emergencies was 1.9% (70% confidence limits, 1.5% to 2.7%). Complications that required surgical intervention occurred with balloon dilation (valvuloplasty, angiopl…

The aim of the study was to demonstrate the importance of early laparoscopic cholecystectomy for acute cholecystitis, without "conservative" treatment (intravenous fluids and antibiotics for 48-72 hours) to reduce inflammation. Early laparoscopic cholecystectomy reduces bile duct injury and the percentage of conversion to open cholecystectomy. Thirty-five patients with acute cholecystitis were submitted to early laparoscopic cholecystectomy, equally divided according to sex. All patients were submitted to US scans preoperatively and operated on by surgeons skilled in emergency laparoscopic ope

True emergencies of the urinary tract center on three major issues, including uncontrolled renal hemorrhage, accumulation of urine within the peritoneal cavity or retroperitoneal space, and obstruction to urine outflow. Successful management of urinary tract emergencies in small animal patients is based not only on the severity of the injury or obstruction but on the condition of the patient at the time of diagnosis and the patient's response to medical stabilization. When most urinary tract emergencies are initially recognized, patients are metabolically and hemodynamically unstable. Therefor…

Urologic emergencies are common in the cancer patient and relate mainly to complications of bladder hemorrhage, upper or lower urinary tract obstruction, urinary tract infection, and priapism. Hemorrhagic cystitis is commonly due to bladder injury from radiation therapy, viral infection, or metabolites of chemotherapeutic agents. Treatments aimed at ameliorating the effects of theses metabolites, such as mesna and intravenous (IV) hydration, coupled with cystoscopy, evacuation of clots, and formalin instillation, have given clinicians an effective means of avoiding exsanguinating hemorrhage fr…

Infectious disease emergencies can be described as infectious processes that, if not recognized and treated immediately, can lead to significant morbidity or mortality. These emergencies can present as common or benign infections, fooling the primary care provider into using more conservative treatment strategies than are required. This review discusses the pathophysiology, history and physical findings, diagnostic criteria, and treatment strategies for the following infectious disease emergencies: acute bacterial meningitis, ehrlichiosis, Rocky Mountain spotted fever, meningococcemia, necroti…

In this paper we discuss the actual role of imaging in surgical management of urgency in colon-rectal pathology. We have considered four different kinds of pathologic: obstruction; occlusion; inflammatory and infections pathology; vascular pathology: In the first group an accurate radiologic evaluation in different decubitus is necessary. In negative or doubt cases, but with clinic highly suggestive for the presence of pathology, a CT examination is mandatory. In the second group, an accurate preliminary radiologic evaluation with plain radiographs of the abdomen is also necessary. With the us

The results of electrical transcutaneous cardiac stimulation (ETCCS) were evaluated in 38 emergency situations seen at the Emergency Service … electrocardiographic monitor-defibrillator-ETCCS units for the complete early electrical support of emergencies associated with disorders of the cardiac rhythm. This support may be given in emergency areas with minimal training requirements.

A variety of acute supraventricular tachycardias may be encountered. In many instances the therapies for these rhythm disturbances overlap, but a rational approach to individual disturbances should be based on an understanding of the anatomy and physiology involved in the individual dysrhythmia. Numerous investigative approaches are underway at present, especially with regard to interruption of arrhythmia pathways by electroshock therapy or surgical therapy. In addition, pacing overdrive may be very effective, especially in patients with reentrant arrhythmias.

As antitumor therapy becomes more effective and less toxic, it is imperative to recognize potentially life-threatening or permanently disabling complications that may be prevented or reversed by prompt action. Accurate diagnosis and treatment of oncologic emergencies may both improve the quality of life and increase the length of survival, allowing more cancer patients to receive an adequate trial of definitive therapy designed to eradicate or palliate their disease.

Ultimate survival of the neonate with a surgical emergency depends upon early recognition by the family physician, who may refer the infant to a pediatric surgical center. The family physician must prevent infectious and metabolic insults to the critically ill neonate while preparing him for safe and rapid transport to a specialty institution. In many cases, the final outcome is as often determined by the initial care received at the referring hospital and during transport as it is by the surgical procedures and postoperative management.
