Intralabyrinthine Cholesterol Granuloma.

Cholesterol granulomas are typically identified by their histological features, including cholesterol crystals, giant cells, fibrosis, and inflammation. They occur predominantly in the middle ear, petrous apex, and orbital region, with rare occurrences in the labyrinth. Diagnosis of these lesions is challenging due to their imaging similarities with endolymphatic sac tumors, particularly in preoperative differentiation. In the present case, a 60-year-old woman diagnosed with an endolymphatic sac tumor through preoperative magnetic resonance imaging underwent a transmastoid surgical procedure, …

Surgical treatment of vertigo with retrolabyrinthine vestibular neurectomy.

Results for control of vertigo and preservation of hearing in patients who have had a retrolabyrinthine vestibular neurectomy (RVN) by our g … re's disease, hearing results were not statistically different from our results with surgery of the endolymphatic sac. Control of vertigo was much more successful with the RVN than endolymphatic sac surgery.

Otosclerosis involving the vestibular aqueduct and Menière's disease.

The coexistence of otosclerosis and endolymphatic hydrops in the temporal bone have been described; however, the mechanism for the development of endolymphatic hydrops in otosclerosis remains unknown. Among 128 temporal bones with otosclerosis, involvement of the vestibular aqueduct by otosclerosis was observed in four temporal bones from two patients. In all four, the vestibular aqueduct was filled with active otosclerotic foci; the lumen of the endolymphatic duct and sac was narrowed as a result of fibrosis, and endolymphatic hydrops, more severe in the pars inferior than the pars superior,

An update on the surgical treatment of Ménière's diseases.

Various surgical procedures have evolved for the treatment of Ménière's disease patients who are refractory to medical therapy. These can be in the form of conservative surgeries as in endolymphatic sac surgery or in the form of destructive procedures. The latter include labyrinthectomy and vestibular nerve section, which result in deafferentation of the vestibular end organ. In the past decade, intratympanic therapy with gentamycin and steroids has evolved as new methods for the treatment of Ménière's disease. The literature witnesses controversies in the effectiveness of endolymphatic sac su…

[Long-term results of Meniere's disease treatment].

The authors present long-term results of Meniere's disease treatment: 2, 5, 10, 15, 20 and more years after drainage and exposure of the endolymphatic sac, chordoplexustomy (50 patients), conservative treatment (20 patients). Acoustic and vestibular functions in these patients are compared. Choice of Meniere's disease effective treatment depends on accurate diagnosis of the disease for which it is important to stage the disease exactly, to assess functional condition of the internal ear (cochlear secretion, lesion of the vestibular analyser, in particular). The most effective and safe treatmen…

Delayed vertigo and profound sensorineural hearing loss.

Episodic vertigo, similar to that of Meniere's disease, can occasionally develop years to decades after an ear has suffered a profound sensorineural hearing loss. Although the pathogenesis of this process is unknown, it is theorized that delayed endolymphatic hydrops develops. An inflammatory reaction could cause obstruction of the endolymphatic duct or interfere with venous drainage from the region of the endolymphatic sac, leading to hydrops. This condition is reviewed, and two adult patients with severe episodic vertigo and a unilateral profound sensorineural hearing loss which occurred in …

Hydrops due to syphilitic endolymphatic duct obliteration.

Three pairs of temporal bones with otologic syphilis were studied histopathologically to detect obliteration of the endolymphatic duct and the associated hydrops. All cases showed infiltration and obliteration of the endolymphatic duct by round cells, giant cells, and fibrous tissue. These findings may be related to failure of medical treatment and sac surgery to arrest or alleviate symptoms of advanced syphilis.

Transforming growth factor beta expression during an inner ear immune response.

OBJECTIVES: The involvement of transforming growth factor beta (TGF-beta), a strong mediator of fibrogenesis, during cochlear immune responses was investigated. METHODS: An inner ear adaptive immune response to antigen was created in mice that were painlessly sacrificed 3 to 48 hours and 7 days after initiation of the immune response. The cochleas were assayed by immunocytochemistry for TGF-beta and latency-associated peptide (LAP). RESULTS: We found LAP expressed in normal cochleas and the endolymphatic sac, in the small round cells in the cochlear scalae and the mesothelial cells under the…

Surgical treatment of vertigo.

Surgical therapy for disabling vertigo usually comes in at a late stage in the course of treatment for this patient group. The surgical procedures can be divided into two categories: non-destructive and destructive surgery. The former constitutes primarily procedures directed against the endolymphatic sac such as endolymphatic sac shunt procedures or decompression procedures. Peri-endolymphatic shunt procedures have also been proposed by some surgeons. None of these latter procedures have, however, gained widespread clinical acceptance. Destructive surgery is aimed against the end-organ itself…

[The role of the glycerol test for diagnosis and treatment of Menière disease (author's transl)].

60 patients--25 with different hearing losses and 35 with Menière disease--underwent the glycerol test. It was demonstrated that only the group with endolymphatic hydrops showed in about 60% a positive test result. The meaning of the glycerol test for differential diagnosis of Menière disease from other diseases with attack vertigo is discussed and the role to select the patients for endolymphatic sac surgery is emphasized.

Surgery for intractable vertigo: Chiang Mai experience.

If medical therapy fails (6-12 months or more), careful consideration of surgical therapy should be followed with; the patient's hearing, severity of symptoms, age, and occupation. Cochleosacculotomy is reserved for elderly Meniere's patients with poor health, poor hearing and good vestibular function. Endolymphatic sac shunt should be considered as the first procedure for disabled Meniere's patients with aidable hearing and may also be used in those patients with bilateral Meniere's disease. Retrolabyrinthine vestibular nerve section (RLVNS) is indicated for patients with disabling vertigo an…

The effect of kanamycin on experimentally induced endolymphatic hydrops.

In guinea pigs with endolymphatic hydrops 6 months after operation the DC potential had decreased from +80 mV to +50 mV, while the sodium activity had increased. The twofold increase in Na+ activity could explain the increased degree of endolymphatic hydrops. The K+ activity and Cl- activity were unchanged. However, endolymphatic hydrops could be produced in kanamycin-induced deaf guinea pigs after obliteration of the endolymphatic sac and duct. The physiological data on DC potential, K+, and Na+ activity were similar to those for non-treated animals. When the animals were treated with kanamyc…

The effect of glycerol on short-term experimental endolymphatic hydrops.

The aim of this paper was to evaluate the effect of one month of treatment with different dosages of glycerol on experimental endolymphatic hydrops produced by obliteration of endolymphatic sac and duct through an extradural approach. Forty-two guinea pigs were used. The animals, divided into six groups, received 0.5-2 gr/kg body weight of the drug per day for one month. The activity of glycerol was determined by statistical analysis of volumetric changes of scala media. Glycerol demonstrated the effect of reducing endolymphatic hydrops. The decrease in hydrops was influenced by the dosage sug…

Experimental endolymphatic hydrops: are cochlear and vestibular symptoms caused by increased endolymphatic pressure?

The correlation between inner ear pressure and cochlear function was investigated in guinea pigs with unilaterally obliterated endolymphatic sacs and ducts. In 11 animals that developed endolymphatic hydrops, auditory thresholds as monitored by auditory evoked action potentials had increased with recruitment. Most of these animals also experienced episodes of spontaneous nystagmus. In control ears endolymphatic pressure did not differ more than 0.5 cm H2O from perilymphatic pressure. In six of 11 hydropic ears, endolymphatic pressure was more than 0.5 cm H2O higher than perilympathic pressure;…

Menière's disease - roentgenologic diagnosis.

Radiologic diagnosis in Menière's disease should not be based on the vestibular aqueducts tomographic reproducibility. Instead, valuable preoperative information should be given about the size and location of the aqueducts external aperture in relation to the posterior and lateral semicircular canals. In saccotomy the semicircular canals are landmarks for identification of the endolymphatic sac. The periaqueductal pneumatization is demonstrated to be partly decisive for the vestibular aqueducts gross morphology. In Menière's disease either a sparse or a total lack of periaqueductal pneumatizat…

Profound hypokalemia and hypochloremic metabolic alkalosis during thiazide therapy in a child with Pendred syndrome.

Pendred syndrome is a recessive autosomal disorder characterized by thyroid goiter and sensorineural hearing loss. The Pendred syndrome gene (SLC26A4) encodes a new anion exchanger named pendrin which mediates iodide transport by thyrocytes and regulates ion and fluid transport by the endolymphatic sac epithelium. Pendrin defects result in inner ear malformations, with enlargement of the endolymphatic sac and duct in association with a large vestibular aqueduct. Furthermore, patients may develop endolymphatic hydrops requiring diuretic therapy, mainly in the form of thiazides. Pendrin could al…

Aquaporin-2 regulation by vasopressin in the rat inner ear.

Our previous studies have suggested a close relationship between vasopressin and endolymphatic hydrops, or the increased volume of endolymph in the inner ear. Endolymphatic hydrops is also thought to occur in Ménière's disease patients. In the kidney collecting duct, vasopressin induces the expression of aquaporin-2 (AQP2), resulting in increased water reabsorption. We explored the possibility, using a quantitative PCR method, that vasopressin regulates the expression of AQP2 mRNA in the rat inner ear, as it does in the kidney. The levels of AQP2 mRNA in the cochlea and endolymphatic sac were …
