Epidemic preparedness: Prenatal Zika virus screening during the next epidemic.

Zika virus (ZIKV) is a vectorborne infectious agent of global public health significance due to its potential to cause severe teratogenic outcomes. The question of whether health systems should consider adopting screening programmes for ZIKV infections during pregnancy warrants consideration. In this analysis, we apply the Wilson-Jungner framework to appraise the potential utility of a prenatal ZIKV screening programme, outline potential screening strategies within the case-finding pathway, and consider other epidemiological factors that may influence the planning of such a screening programme

Parameter Scaling for Epidemic Size in a Spatial Epidemic Model with Mobile Individuals.

In recent years, serious infectious diseases tend to transcend national borders and widely spread in a global scale. The incidence and prevalence of epidemics are highly influenced not only by pathogen-dependent disease characteristics such as the force of infection, the latent period, and the infectious period, but also by human mobility and contact patterns. However, the effect of heterogeneous mobility of individuals on epidemic outcomes is not fully understood. Here, we aim to elucidate how spatial mobility of individuals contributes to the final epidemic size in a spatial susceptible-expo…

Will Africa's future epidemic ride on forgotten lessons from the Ebola epidemic?

The Ebola virus disease (EVD) outbreak in the three West Africa countries of Guinea, Liberia, and Sierra Leone, was declared by the World Health Organization (WHO) as a public health event of international concern in August 2014. The disease, which has caused more than 10,000 deaths from over 25,000 cases, has thrived on a failed disease control system, national denial, and a poor and fragile healthcare delivery system. The slow and initially uncoordinated national and global response turned the outbreak into an unprecedented humanitarian disaster. Prevention and control of future outbreaks de

The developmental cycle of a drug epidemic: the cocaine smoking epidemic of 1981-1991.

Although Americans have experienced many drug epidemics, the majority of which have ended within ten years of onset, they nevertheless believed that the use of smokable cocaine, which took the popular form of crack cocaine in 1984, would grow exponentially throughout the 1990s unless it was vigorously combated. However, in 1991 it appears that crack use is in decline even in the inner-city neighborhoods where it had been most entrenched, and that the decline is due more to natural controls than to the War on Drugs. The cyclical nature of drug epidemics, as well as their progression through reg…

The Cholera Epidemic of 1907 and the Formation of Colonial Epidemic Control Systems in Korea.

It was in 1907 when Korea was annexed by Japan in the field of health care systems as the Gwangje Hospital, Uihakgyo the National Medical School and the Korean Red Cross Hospital were merged into the colonial Daehan Hospital, and massive cholera epidemic controls by the Japanese Army were enforced. However, despite their importance, the cholera epidemic of 1907 in Korea and preventive measures taken at that time have not yet been studied extensively as a single research subject. The purpose of this paper is to contribute to a more concrete and broader understanding of the Korea-Japan annexatio

[The nutritional-viral etiology of epidemic neuropathy. The Group for the Study of Epidemic Neuropathy].

Viral sequences of the cerebrospinal fluid were amplified by using oligonucleotides that are homologous to region 5' of the Enteroviruses. The results showed a great variability in the amplified sequences, suggesting the presence of enteroviral quasispecies with altered biological properties in the stressed individuals that may play an important role in the neurological damage.

[Reliability of the diagnosis of cerebrospinal meningitis during an epidemic. The epidemic in Chad in 1988].

The authors analysed the positive predictive value (PPV) of the clinical diagnosis during an epidemic of meningococcal meningitidis in Africa. This PPV was globally 73.3 +/- 11.2%. This PPV was fluctuating according to the standards of the diagnosis. Typical clinical meningitic syndrome without spinal picture: 85.7 +/- 10.3%, Non typical syndrome and macroscopical aspect of cerebrospinal fluid: 62.5 +/- 16.4%.

1987 yellow fever epidemics in Oyo State, Nigeria: a survey for yellow fever virus haemagglutination inhibiting antibody in residents of two communities before and after the epidemics.

A survey for yellow fever haemagglutination inhibiting antibody was carried out before and after the 1987 yellow fever epidemics in Nigeria in two localities in Oyo State, namely: Ibadan and Ogbomosho. A total of 129 sera were collected from the two localities before the epidemic. The overall prevalence of yellow fever HI antibody was 37%. A breakdown of positive sera showed that 30/100 and 18/29 sera tested in Ibadan and Ogbomosho respectively were positive. There was a higher prevalence of HI antibody in males (43%) than in females (32%). Of the 48 yellow fever positive sera 23(47%) containe…

Comparative genomic analysis of pre-epidemic and epidemic Zika virus strains for virological factors potentially associated with the rapidly expanding epidemic.

Less than 20 sporadic cases of human Zika virus (ZIKV) infection were reported in Africa and Asia before 2007, but large outbreaks involving up to 73% of the populations on the Pacific islands have started since 2007, and spread to the Americas in 2014. Moreover, the clinical manifestation of ZIKV infection has apparently changed, as evident by increasing reports of neurological complications, such as Guillain-Barré syndrome in adults and congenital anomalies in neonates. We comprehensively compared the genome sequences of pre-epidemic and epidemic ZIKV strains with complete genome or complete

Impact of the Opioid Epidemic.

In recent years the prescription opioid overdose epidemic has decreased, but has been more than offset by increases in overdose caused by fentanyl and fentanyl analogues. Opioid overdose patients should receive naloxone if they have significant respiratory depression and/or loss of protective airway reflexes. Patients who receive naloxone should be observed for recurrent opioid effects. Patients with opioid overdose may be admitted to the intensive care unit for naloxone infusions, treatment of noncardiogenic pulmonary edema, autonomic instability, or sequelae of hypoxia-ischemia or cardiac ar

Ebola: Anatomy of an Epidemic.

As of the end of March 2016, the West Africa epidemic of Ebola virus disease (Ebola) had resulted in a total of 28,646 cases, 11,323 of them fatal, reported to the World Health Organization. Guinea, Liberia, and Sierra Leone were most heavily affected, but Ebola cases were exported to several other African and European countries as well as the United States, with limited further transmission, including to healthcare workers. We review the descriptive epidemiology of the outbreak, novel aspects and insights concerning the unprecedented response, scientific observations, and public health implic
