Ozone exposure and pulmonary effects in panel and human clinical studies: Considerations for design and interpretation.

UNLABELLED: A wealth of literature exists regarding the pulmonary effects of ozone, a photochemical pollutant produced by the reaction of nitrogen oxide and volatile organic precursors in the presence of sunlight. This paper focuses on epidemiological panel studies and human clinical studies of ozone exposure, and discusses issues specific to this pollutant that may influence study design and interpretation as well as other, broader considerations relevant to ozone-health research. The issues are discussed using examples drawn from the wider literature. The recent panel and clinical literature

Single locus genotyping to track Leishmania donovani in the Indian subcontinent: Application in Nepal.

BACKGROUND: We designed a straightforward method for discriminating circulating Leishmania populations in the Indian subcontinent (ISC). Research on transmission dynamics of visceral leishmaniasis (VL, or Kala-azar) was recently identified as one of the key research priorities for elimination of the disease in the ISC. VL in Bangladesh, India, and Nepal is caused by genetically homogeneous populations of Leishmania donovani parasites, transmitted by female sandflies. Classical methods to study diversity of these protozoa in other regions of the world, such as microsatellite typing, have proven

Traditional reviews, meta-analyses and pooled analyses in epidemiology.

BACKGROUND: The use of review articles and meta-analysis has become an important part of epidemiological research, mainly for reconciling previously conducted studies that have inconsistent results. Numerous methodologic issues particularly with respect to biases and the use of meta-analysis are still controversial. METHODS: Four methods summarizing data from epidemiological studies are described. The rationale for meta-analysis and the statistical methods used are outlined. The strengths and limitations of these methods are compared particularly with respect to their ability to investigate h

The Childhood Asperger Syndrome Test (CAST): test-retest reliability in a high scoring sample.

The Childhood Asperger Syndrome Test (CAST) is a 37-item parental self-completion questionnaire designed to screen for high-functioning autism spectrum conditions in epidemiological research. The CAST has previously demonstrated good accuracy for use as a screening test, with high sensitivity in studies with primary school aged children in mainstream schools. This study aimed to investigate test-retest reliability of the CAST in a high scoring sample. To this end, 73 parents filled in the second CAST (CAST-2) within approximately 2 months of the first administration of the CAST (CAST-1). Agree

Relationship between dysfunctional beliefs, self-esteem, extreme appraisals, and symptoms of mania and depression over time in bipolar disorder.

OBJECTIVES: Bipolar disorder (BD) is a mental health problem characterized by episodes of mania and depression which can lead to significant difficulties impairing one's daily functioning. Cross-sectional research has highlighted self-esteem and dysfunctional beliefs in those with this diagnosis, but there has been little research into how self-esteem and dysfunctional beliefs relate to symptoms of mania and depression over time. DESIGN: A secondary data analysis of a prospective cohort study was used. METHODS: Forty patients with BD attending a community adult mental health service complete

The role of the Mobile Mental Health Units in mental healthcare delivery in rural areas in Greece: current challenges and prospects.

The present paper aims to highlight the importance of Mobile Mental Health Units (MMHUs) in the delivery of mental health services in remote areas in rural Greece. Since the foundation of the first MMHUs in the eighties till nowadays, those services have expanded in many rural areas and there is some evidence that they are effective in the management of patients with severe mental disorders. The care of those patients seems to contribute to a significant reduction in voluntary and involuntary acute admissions and in length of hospital stay. Cost/effectiveness analysis studies have also shown f

Transgender People and HIV Prevention: What We Know and What We Need to Know, a Call to Action.

Transgender people have been disproportionally affected by HIV, particularly transgender women. Their increased vulnerability to HIV is due to multiple issues, including biological (eg, increased efficiency of HIV transmission through receptive anal sex), epidemiological (eg, increased likelihood of having HIV-infected partners), structural (eg, social stigma limiting employment options), and individual factors (eg, internalized stigma leading to depression and substance use and risk-taking behaviors). There have been limited culturally appropriate HIV prevention interventions for transgender

Resolving the enigma of the mesoamerican nephropathy: a research workshop summary.

The First International Research Workshop on Mesoamerican Nephropathy (MeN) met in Costa Rica in November 2012 to discuss how to establish the extent and degree of MeN, examine relevant causal hypotheses, and focus efforts to control or eliminate the disease burden. MeN describes a devastating epidemic of chronic kidney disease of unknown origin predominantly observed among young male sugarcane cutters. The cause of MeN remains uncertain; however, the strongest hypothesis pursued to date is repeated episodes of occupational heat stress and water and solute loss, probably in combination with ot

Hearing and age: research challenges and the National Institute on Aging.

Just as the ear trumpet once symbolized the elderly, so do contemporary approaches to hearing loss in the aged reflect many of the major themes in geriatrics and gerontology today. This paper begins by describing the National Institute on Aging (NIA) with particular emphasis on activities relevant to hearing in the elderly. Next, several areas of research interest, including the typology of presbycusis and related conditions, the epidemiology of auditory impairment in old age, the design of testing and research, and the rehabilitation of the hearing-impaired elderly, are addressed. The NIA and

Updates to the Running Shoe and Injury Considerations.

Elite athletes continue to break new records while running shoe design evolves, diverges, and incorporates new technologies. Shoe design is grossly split between minimalist, traditional, and maximalist shoes. Each shoe has a unique design with characteristics defined by the stack height, heel height, heel-to-toe drop, arch support, cushion, weight, and resulting biomechanics. Current epidemiology of running-related injuries correlates to experience level, weekly volume, and training progression, but the literature currently suggests there are limited effects from footwear. Although biomechanic

Navigating Blastocystis research - challenges in the next-generation sequencing era.

Blastocystis research has grown rapidly with molecular tools, especially next-generation sequencing, increasing subtype discovery but raising methodological and taxonomic challenges. We assess subtype designation criteria, caution against lowering divergence thresholds without biological evidence, and propose minimum standards requiring near full-length small subunit ribosomal RNA data and replication to ensure nomenclatural stability and epidemiological comparability.

"Essential Tremor Plus": A Problematic Concept: Implications for Clinical and Epidemiological Studies of Essential Tremor.

BACKGROUND: An issue of central importance for most neurological disorders is that of case definition. In the field of neuroepidemiology, case definition informs the design, conduct, and interpretation of research studies that range from observational to interventional studies, from case-control to cohort studies, and from descriptive to analytical studies. For essential tremor (ET), one of the most common neurological disorders, the issue of case definition is particularly important. Though traditionally viewed as a monochromatic disorder, recent years have seen an expansion of the clinical p

Challenges to effective research in acute trauma resuscitation: consent and endpoints.

Selection of study endpoints is one of the most important decisions in the design of effective clinical trials. Late mortality (e.g., 28 days) is an unambiguous endpoint, accepted by regulatory agencies, but it is viewed as problematic among researchers in the study of resuscitation for acute trauma injury with hemorrhagic shock. In February 2008, physicians, ethicists, statisticians, and research scientists from the military, academia, industry, the Federal Drug Administration, and the National Heart Lung and Blood Institute gathered to discuss the obstacles confronting the trauma community i

Does light cause internal cancers? The problem and challenge of an ubiquitous exposure.

Visible light of sufficient intensity and duration inhibits melatonin biosynthesis, and experimental studies suggest that melatonin may protect against cancer. From a public health point of view it is important to verify or falsify the hypothesis that artificial light--or even sunlight itself--suppresses melatonin production sufficiently to increase the risk of developing cancers of internal organs in man. Epidemiology is a discipline that can contribute to in-vivo verification of experimental findings. But when attempting to study the effects of light on man, epidemiologists are faced with a

The Italian national tuberculosis project.

This paper considers the global impact of tuberculosis (TB; one third of the world's population is infected by Mycobacterium tuberculosis, and 30,000,000 TB deaths are estimated to occur in the next ten years) to specifically examine the situation in Italy and the reasons for the resurgence of the disease in our country. These considerations led to the proposal of a nation-wide research project on tuberculosis, coordinated by the Istituto Superiore di Sanità (ISS) (The National Institute of Health), in order to meet public health needs in this field. The project consists of subprojects focussi

Key indicators for the monitoring and evaluation of control programmes of human African trypanosomiasis due to Trypanosoma brucei gambiense.

Very little research has been devoted to the design of epidemiological tools for the monitoring and evaluation of National Human African Trypanosomiasis (HAT) Control Programmes and daily management decisions are made in the absence of accurate knowledge of the situation. This paper identifies key indicators necessary to make decisions in the field and constantly adjust control activities to changing situations. Examples are derived from the Médecins Sans Frontières (MSF) HAT Control Programme in Adjumani, Uganda. Based on the principles of quality assurance, the focus is placed on process ind

Cohort Profile: The Framingham Heart Study (FHS): overview of milestones in cardiovascular epidemiology.

The Framingham Heart Study (FHS) has conducted seminal research defining cardiovascular disease (CVD) risk factors and fundamentally shaping public health guidelines for CVD prevention over the past five decades. The success of the Original Cohort, initiated in 1948, paved the way for further epidemiological research in preventive cardiology. Due to the keen observations suggesting the role of shared familial factors in the development of CVD, in 1971 the FHS began enroling the second generation cohort, comprising the children of the Original Cohort and the spouses of the children. In 2002, th
