Laryngocele and epiglottic cyst as rare causes of obstructive sleep apnea.

INTRODUCTION: Upper airway pathologies have been reported to cause obstructive sleep apnea (OSA). CASE REPORT: We present two rare cases of OSA, one with laryngocele secondary to partial laryngectomy and the other with epiglottic cyst. CONCLUSION: In the management of cases with OSA symptoms, a detailed evaluation of upper airways, including endoscopic and radiological examinations as well as polysomnography, should be done.

Septic epiglottic chondritis with abscessation in 2 young Thoroughbred racehorses.

Septic epiglottic chondritis with abscessation diagnosed in 2 Thoroughbred racehorses. Infected cartilage removed videoendoscopically followed by systemic antibiotics. The infectious process was successfully controlled, but permanent dorsal displacement of the soft palate (DDSP) with a shortened, deformed epiglottic cartilage developed. Surgery for the DDSP using bilateral partial sternothyroidectomy or laryngeal tie-forward failed.

Foreign body in the vallecula presenting as acute epiglottitis with unilateral supraglottitis.

Life threatening inflammatory swelling of the epiglottic and supraglottic regions secondary to a foreign body in the vallecula is an unusual occurrence. Upper airway obstruction is potentially a life threatening sequela. We present a unique case of acute epiglottitis with unilateral supraglottitis secondary to foreign body in the vallecula, which gradually improved after emergency endotracheal intubation for respiratory arrest, followed by intensive conservative management with intravenous antibiotics, cortisone and intravenous fluids. This case emphasizes that a foreign body in the vallecula …

Epiglottic hematoma leading to airway obstruction after general anesthesia.

Bleeding into the upper airway can cause airway obstruction and death if not recognized promptly. Anesthesiologists are quite familiar with potential airway obstruction from acute epiglottitis, but they may be less familiar with the potential for airway obstruction from epiglottic hematoma. We report what we believe is the second case of epiglottic hematoma after anesthesia and surgery that led to an acute upper airway obstruction. Our case was unique in that there was no excessive airway trauma during tracheal intubation. Most important, this case emphasizes that patients receiving multiple a…

[Ten-year experiences of epiglottic laryngoplasty after partial laryngectomy].

In order to objectively evaluate the long-term result of epiglottic laryngoplasty (ELP), ten-year experience of ELP after partial laryngectomy in 100 patients with glottic cancer was analysed. Our program included 1. complete excision of laryngeal carcinoma; 2. correct selection of material for laryngoplasty (LP); 3. technical modification of LP. The 3, 5 and 10 year survival rates were 87%, 79.5% and 66.7% respectively. It is believed that modified ELP is better than original K-S-T technique in the aspect of preserving all laryngeal functions.

The significance of pre-epiglottic space invasion in supraglottic laryngeal carcinomas.

It is widely accepted that tumoral invasion of the pre-epiglottic space (PES) has a significant prognostic importance in supraglottic laryngeal carcinomas. The lymphatics of the supraglottic larynx drain to cervical lymph nodes via the PES. Since the supraglottic larynx is an embryological unit that contains the PES, malignant lesions of this region must be resected with en bloc surgery, including the PES. Tumors with PES invasion are already considered to be T3 in TNM staging. The purpose of this study was to review the clinical experience we have had with these tumors at Ankara University. T

The characteristics of children with epiglottitis who develop the complication of pulmonary edema.

A review was performed of 234 consecutive cases of epiglottis that occurred during a 20-year period to delineate the rate of, and clinical c … mechanisms for the development of this complication are described. Pulmonary edema associated with epiglottitis is an uncommon complication that can occur following endotracheal intubation in those patients with marked respiratory insufficiency. An artificial airway should be instituted in all cases of pediatric epiglottitis--the potential complication of pul …

Radiographic assessment of epiglottic length and pharyngeal and laryngeal diameters in the Thoroughbred.

A lateral radiograph of the pharyngeal region provides a technique for identifying pharyngeal disorders, including entrapment of the epiglottic cartilage and dorsal displacement of the soft palate. Epiglottic cartilage length, predicted from radiographs by measurement from the body of the thyroid cartilage to the tip of the epiglottis (thyroepiglottic length), was well correlated to actual length at postmortem examination (r2 = 0.98). Thyroepiglottic length, corrected for magnification, was 8.76 +/- 0.44 cm in 24 healthy Thoroughbreds. Nine Thoroughbreds with entrapment of the epiglottic carti

Epiglottic enlargement following radiation treatment of head and neck tumors.

This article describes delayed noninfectious epiglottic enlargement in two children, one with rhabdomyosarcoma of nasopharynx and the other with undifferentiated anaplastic carcinoma of the neck. The pathogenesis is assumed to be delayed laryngeal injury following radiotherapy alone or combined with chemotherapy. The radiographic findings in these cases were identical to those of acute epiglottitis.

The safety of intubation in croup and epiglottitis: an eight-year follow-up.

A series of 815 infectious croup (i.e., laryngotracheobronchitis) cases and 55 epiglottitis cases, encompassing an eight-year interval, is reviewed to determine the incidence of adverse effects of nasotracheal intubation used to manage upper airway obstruction. The racial, sex, and age distributions, in addition to modalities of treatment, are presented. The intubated cases (86 patients), representing 6.5 percent of all croup cases and 60 percent of all epiglottitis cases, are described in more detail with respect to presenting symptoms and physical findings. The average duration of intubation…

Epiglottic retroversion as a cause of upper airway obstruction: A case report.

RATIONALE: Epiglottic retroversion is the abnormal movement of the epiglottis to the rima glottis, resulting in blockage of inspiratory airflow. Acute upper airway obstruction caused by epiglottic retroversion can lead to sudden respiratory failure. Epiglottic retroversion has occasionally been reported in horses and dogs; however it is extremely rare in humans. Herein, we report a case of epiglottic retroversion causing recurrent upper airway obstruction in human. PATIENT CONCERNS: We present the case of a 74-year-old man who was diagnosed with epiglottic retroversion without evidence of epi

Low-Temperature Plasma Radiofrequency Ablation for Congenital Epiglottic Cysts.

Objectives: Congenital epiglottic cysts are rare disorders of the larynx with symptoms such as laryngeal stridor and inspiratory dyspnea and are life-threatening in severe cases. This study aimed to investigate the usefulness of low-temperature plasma radiofrequency ablation for congenital epiglottic cysts and provide a reference for clinicians to develop treatment options. Methods: The clinical data of children (n = 7, 4 males and 3 females) with congenital epiglottic cysts, who were admitted to the Second Affiliated Hospital of Wenzhou Medical University and Yuying Children's Hospital from M

Epiglottic retraction is useful in diagnosis of exercise-induced laryngeal obstruction.

BACKGROUND: Exercise-induced laryngeal obstruction (EILO) is distinguished as upper airway obstruction accompanied by inspiratory dyspnoea and stridor during highly intensive exercises. Epiglottic retraction in the diagnosis of EILO has not been sufficiently explored. AIMS/OBJECTIVES: We highlight the importance of epiglottic retraction in patients with EILO by evaluation by several diagnostic methods for EILO. Consideration of epiglottic retraction may be important for accurate diagnosis of EILO. RESULTS: Epiglottic retraction could be observed in three patients by laryngoscopy during heavy

Epiglottic granular cell tumour: a case report and literature review.

Granular cell tumour is a rare, benign tumour that may arise in the soft tissues of various locations. It is often asymptomatic; hence, it gets misdiagnosed as other conditions. This is a case of an asymptomatic 50-year-old male with an incidental finding of a supraglottic lesion during routine oesophagogastroscopy. Examination under general anaesthetic showed a whitish non-ulcerating exophytic lesion on the laryngeal surface of the epiglottis, sparing anterior commissure and vocal cords. Surgical excision was performed with an open supraglottic laryngectomy approach. Histopathology confirmed
