To assess the prevalence of erectile dysfunction among a sample of patients attending a psychiatric clinic, we administered the International Index of Erectile Function on 108 male patients attending the clinic. We also obtained sociodemographic and clinical variables from the patients. The mean age of the patients studied was 39.6 (s.d.=11.6), with a mean duration of illness of 10.24 (s.d.=8.2) years. The administration and suitability of intracavernous PGE-1 in men with neuropathic erectile dysfunction is reported herein. Twenty-seven men with neuropathic erectile dysfunction (SCI, 14; multiple sclerosis, 7; discogenic disease, 6) were evaluated and treated with intracavernous PGE-1. To evaluate the use of papaverine-induced erection as a diagnostic test in men with erectile dysfunction, 24 patients had papaverine injected into the corpus cavernosum. The results of the papaverine test in combination with the tentative diagnosis, based upon patient history, clinical examination and peno-brachial pressure index, gave in most cases sufficient information about the function of the penile vessels to decide in which patients a selective arteriography or a cavernosography was indicated. Two patients developed priapism after the papaverine injection.

Based on 216 dynamic cavernosographies, performed in 25 patients with normal erectile capacity and in 191 patients with erectile dysfunction, an evaluation of this method in regard to its diagnostic validity in erectile dysfunction was possible. An additional comparison of the results of intracavernous application of vasoactive drugs with the outcome of dynamic cavernosography in 141 patients showed that a negative or clearly positive test-outcome indicated in over 90 per cent of the evaluated patients the presence or lack of venous insufficiency during dynamic cavernosography.

In 25-30% of the patients with erectile dysfunction, venous insufficiency is the (additional) reason for the erectile failure. Surgical procedures and prognosis depend largely on the precise localisation of the pathological drainage. The venous leakage is proven and exactly localized by a multiprojectional cavernosography, measurement of the maintainance flow and intracavernous pressure monitoring. The most reliable screening test for venous erectile dysfunction is the intracavernous application of a standardised vasoactive drug combination.
