Clinical and Ethical Considerations in Allocation of Ventilators in an Influenza Pandemic or Other Public Health Disaster: A Comparison of the 2007 and 2015 New York State Ventilator Allocation Guidelines.

OBJECTIVES: During an influenza or coronavirus disease 2019 (COVID-19) pandemic that results in acute respiratory distress, the number of available ventilators will not meet demand. In 2007, the New York State Task Force on Life and the Law and Department of Health released draft Guidelines for ethical allocation of ventilators for adults. In 2015, updated guidelines were released to ensure that: (1) revisions reflect the public's values and (2) the triage protocol is substantiated by evidence-based clinical data. We summarize the development and content of the 2015 Guidelines compared with th

The professional guest: Ethical challenges in home-based end-of-life care among interprofessional teams.

BackgroundHome-based end-of-life palliative care presents unique ethical challenges that differ fundamentally from those in institutional settings. Healthcare professionals navigate the complex role of being both clinical experts and guests in patients' domestic environments, operating in a context where professional authority is continuously negotiated rather than institutionally established.Research aimThis study examines the ethical tensions healthcare professionals encounter in home-based end-of-life palliative care and explores the strategies they employ to balance and negotiate competing

An Innovative University and Non-Governmental Organization Partnership in Global Health: Application of the Brocher Declaration to 20 Years of Collaboration in Kenya.

Background: Global health partnerships between academic institutions and non-governmental organizations (NGOs) are increasingly recognized as essential mechanisms for strengthening health systems in low-resource settings. Despite their proliferation, few partnerships demonstrate long-term, equitable collaboration grounded in established ethical frameworks. Objectives: To describe and evaluate a 20-year partnership between Lwala Community Alliance, a community-based NGO in western Kenya, and Vanderbilt University in the United States, using the Brocher Declaration principles as an evaluative fr

Envisioning a Multidisciplinary HBV Cure Research Agenda.

PURPOSE OF REVIEW: We examine the current understanding of the multidisciplinary aspects of hepatitis B cure research, such as socio-behavioral sciences, ethics, community engagement, and translational and implementation science. RECENT FINDINGS: The peer-reviewed literature on the multi-disciplinary aspects of HBV cure research is gradually expanding, although several areas still require attention. These deficiencies include: the acceptability of HBV treatment discontinuations, HBV-related stigma, the impact of co-infections (e.g., HIV), and the translation of discoveries to resource-limited

Coverage of ethics within the artificial intelligence and machine learning academic literature: The case of disabled people.

Disabled people are often the anticipated users of scientific and technological products and processes advanced and enabled by artificial intelligence (AI) and machine learning (ML). Disabled people are also impacted by societal impacts of AI/ML. Many ethical issues are identified within AI/ML as fields and within individual applications of AI/ML. At the same time, problems have been identified in how ethics discourses engage with disabled people. The aim of our scoping review was to better understand to what extent and how the AI/ML focused academic literature engaged with the ethics of AI/ML

Use of the Vitiligo Extent Score: a protocol for a scoping review.

INTRODUCTION: Vitiligo is a chronic skin condition with no cure. Clinical assessment and treatment evaluation relies heavily on clinometry tools and expert knowledge. The Vitiligo Extent Score has been proposed as one of the most reliable and easy-to-use clinometry tools for vitiligo. METHODS AND ANALYSIS: We proposed a scoping review to identify all the available evidence on the clinical research availability around the Vitiligo Extent Score. The following databases will be searched: MEDLINE (PubMed), Embase, Open Grey, Lens and Directory of Open Access Journals. In addition, the approach pr

Integrating ethics into infectious disease graduate training: a multidimensional framework for public health practice.

Through a narrative review and synthesis of the global status of Infectious Disease Ethics (IDE) education, this paper proposes positioning IDE as a core competency in graduate training and constructs a three-dimensional integrated model of "Theory-Practice-Assessment." Drawing on the experience of the OPENING project by the European Society of Clinical Microbiology and Infectious Diseases (ESCMID), it emphasizes that the ethical framework must adapt to the paradigm shifts brought about by emerging technologies such as genomics. This model not only addresses the gaps in IDE education exposed b

Beyond treatment: Ethical decision-making and palliative care in critically ill patients with cancer.

Over recent decades, advances in cancer diagnosis and treatment have improved survival and quality of life, leading to a growing number of cancer patients being admitted to Intensive Care Units (ICUs). In this setting, clinical decision-making is challenged by prognostic uncertainty, intensive technology use, and therapeutic interventions whose proportionality may be uncertain, generating relevant ethical conflicts in clinical practice. The application of bioethics to critical oncology provides a deliberative framework based on the principles of autonomy, beneficence, nonmaleficence, and justi

Genetic testing: practical, ethical, and counseling considerations.

Genetic testing is becoming a much more common practice in medicine today. This presents a unique set of challenges for medical professionals in virtually all specialties. The practical aspects of determining which test to order, and in interpreting the result accurately in the context of the family history, can be difficult. Additionally, the ethical conundrums that frequently present themselves when genetic risk assessment and/or genetic testing is being considered can be daunting. These challenges present real concerns for medical professionals and patients alike. Included in this article i

Opioid administration for severe neuropathic pain in a patient with depression and prior heroin use.

The treatment of pain can raise ethical dilemmas. This article presents a case of a young woman with a pain crisis from severe neuropathic pain that is complicated by comorbidities of depression and prior (but not active) heroin use. Medical and ethical justifications for either withholding or providing opioids for her pain are examined. State and federal laws governing opioid prescriptions by physicians for pain management are reviewed.

Ethical issues in vesico-vaginal fistula care and research.

The plight of women in poor nonindustrialized countries who have incurred catastrophic childbirth injuries, such as vesico-vaginal and recto-vaginal fistulas, from prolonged obstructed labor is receiving increased attention from the world medical community. While the good intentions that have prompted this greater concern are not in doubt, intentions by themselves are insufficient guarantees of ethical conduct in programs developed to repair these injuries. Clinical proposals put forward to deal with the problem of fistula must undergo critical analysis to insure that basic ethical requirement

Australia: regulating genomic data sharing to promote public trust.

The regulation of genomic data sharing in Australia is a confusing mix of common law, legislation, ethical guidelines, and codes of practice. Beyond privacy laws, which only apply to genomic data that meets the definition of personal information, the key regulatory lever is the National Health and Medical Research Council (NHMRC) National Statement for Ethical Conduct in Human Research ("National Statement") (2007). Compliance with the National Statement is a requirement for institutions to apply to the NHMRC for funding, and includes-among other things-requirements for review of most genomic

Long-read sequencing for neurological disorders: opportunities, challenges, and future directions.

BACKGROUND: Many neurological disorders (NDs) have a genetic basis, yet traditional diagnostic tools such as EEGs, EMGs, and neuroimaging primarily capture downstream manifestations. Although short-read sequencing (SRS) has advanced genetic diagnostics, significant gaps remain. Large repeat expansions, complex structural variants, mitochondrial variants, transcript splicing alterations, and epigenetic changes, all common contributors to NDs, are difficult to resolve with SRS. METHODS: This review examines the capabilities of long-read sequencing (LRS) technologies in addressing these limitati
