[Anticoagulant therapy for chronic cardiac diseases. Atrial fibrillation, valvular heart disease, congestive heart failure].

In cardiology, anticoagulant therapy is absolutely indicated after mechanical valve replacement, but is much more often necessary as a prophylactic measure in atrial fibrillation for prevention of embolic stroke. For more than 50 years, there has been no alternative to the oral application of vitamin K antagonists (VKA), which are known to have a very narrow therapeutic window. Despite being highly effective in preventing embolic stroke, many patients are not adequately treated with VKA, and up to 45% of the time the values lie outside the therapeutic range. The reasons for this might be diffi

Identification and purification of vitamin K-dependent proteins and peptides with monoclonal antibodies specific for gamma -carboxyglutamyl (Gla) residues.

Novel monoclonal antibodies that specifically recognize gamma-carboxyglutamyl (Gla) residues in proteins and peptides have been produced. As demonstrated by Western blot and time-resolved immunofluorescence assays the antibodies are pan-specific for most or all of the Gla-containing proteins tested (factors VII, IX, and X, prothrombin, protein C, protein S, growth arrest-specific protein 6, bone Gla protein, conantokin G from a cone snail, and factor Xa-like proteins from snake venom). Only the Gla-containing light chain of the two-chain proteins was bound. Decarboxylation destroyed the epitop

Anticoagulation Monitoring During ECMO Support: Monitor or Flip a Coin?

Should we rely on anticoagulation monitoring in ECMO patients or simply flip a coin? The increasing use of anti-factor Xa activity to monitor the effect of UFH appears appropriate, given its moderate correlation with the UFH infusion rates, and it may play a role in preventing thromboembolic events. However, to avoid bleeding complications, more sophisticated tools, and careful clinical decision-making remain essential.

Differentiation of low-molecular-weight heparins: impact on the future of the management of thrombosis.

Low-molecular-weight heparins (LMWHs) are now universally accepted as drugs of choice for postsurgical prophylaxis and treatment of deep vein thrombosis (DVT). Currently, these agents are also being developed for the treatment of various cardiovascular conditions. Because of manufacturing differences, each of the LMWHs exhibits distinct pharmacologic and biochemical profiles. The specific activity of these agents in anticoagulant assays ranges from 35 to 45 anti-IIa U/mg, whereas the activity in terms of anti-Xa units is designated as 80 to 145 U/mg. These LMWHs are also capable of producing p

The inhibition of human factor VIIa-tissue factor by antithrombin III-heparin is enhanced by factor X on a human bladder carcinoma cell line.

Previous studies have shown that antithrombin III-heparin effectively inhibited the factor VIIa-tissue factor complex. Herein, we show that the neutralization of factor VIIa in complex with the cell surface tissue factor by antithrombin III-heparin was markedly enhanced by plasma levels of factor X. Active site-mutated factor X (S376A factor X) and factor Xa previously inactivated with dansyl-Glu-Gly-Arg-chloromethyl ketone were as effective as plasma-derived factor X in this reaction, indicating that the active site serine residue of factor Xa was not involved in this mechanism. Furthermore,

Reversal strategies in patients treated with direct oral anticoagulants.

Administration of direct oral anticoagulants (DOACs) for the treatment of venous thrombotic events (VTE) or non-valvular atrial fibrillation (AF) is now standard of care and has demonstrated clinical efficacy and safety in numerous clinical studies. Usually these substances have lower overall mortality and less risk of cerebral hemorrhage, but depending on the substance and study, they are more likely to cause gastrointestinal bleeding than vitamin K antagonists (VKA), the medication that used to be standard for VTE and AF. Since DOACs have very short plasma elimination half-lives compared to

Reversal of anticoagulation in patients with intracerebral haemorrhage related to oral anticoagulants: State of the evidence.

PURPOSE: About 20%-25% of all intracerebral haemorrhages are associated with oral anticoagulation therapy. Reflecting changings prescription patterns in the general population, the spectrum of oral anticoagulation-associated intracerebral haemorrhage has substantially changed in the last decade. In many European countries, direct oral anticoagulant-associated intracerebral haemorrhage is now more frequent than vitamin K antagonist-associated intracerebral haemorrhage. Outcome in patients with anticoagulation-associated intracerebral haemorrhage is poor, likely mediated by a high incidence of h

The dual FXa/thrombin inhibitor SATI prevents fibrin and platelet deposition in hypercoagulant rats.

INTRODUCTION: Systemic hypercoagulation is often a severe complication of infective and inflammatory diseases, which overcome the hemostatic balance and lead to multiple thrombotic occlusions in the microvasculature and organ damage and is related to high mortality rates. SATI is a potent dual inhibitor of FXa and thrombin with antithrombotic efficacy in venous and arterial thrombosis models. In this study, the antithrombotic efficacy of SATI was investigated in a microthrombosis model in rats with an induced hypercoagulant state. MATERIALS AND METHODS: The hypercoagulant state was generated

[The role of PAR family receptors in activation of mast cells in the norm and in acute inflammation in rats].

Dose-dependent release of beta-hexoaminidase induced with thrombin was shown to be mediated by the PAR-1. This was further confirmed by means of agonist, antagonist and PAR desensitization. Acceleration of the mast cell mediator secretion by the Xa factor and PAR-2 agonist, was revealed. An increase in the mast cell release induced by thrombin and TRAP-6 was shown in the acute peritonitis model.

Fondaparinux: a new antithrombotic agent.

BACKGROUND: Venous thrombosis is usually triggered by a low-flow state, as in prolonged periods of bed rest after hip or knee surgery. Antithrombotic agents are the drugs of choice in such circumstances. The new factor Xa inhibitor fondaparinux has been approved by the US Food and Drug Administration for the prevention of venous thromboembolism in patients undergoing hip fracture surgery, hip replacement surgery, or knee replacement surgery. OBJECTIVE: The aim of this article was to review the clinical pharmacology of fondaparinux and summarize the data from available clinical trials of this

Comparative studies on the anticoagulant and protease generation inhibitory actions of newly developed site-directed thrombin inhibitory drugs. Efegatran, argatroban, hirulog, and hirudin.

Site-directed thrombin inhibitors are being currently assessed clinically for their antithrombotic efficacy. Although these agents are claimed to be specific and direct thrombin inhibitors, their mechanism of inhibition varies. The objective of these studies was to compare four such agents in in vitro systems and to assess their relative anticoagulant efficacy. The four agents utilized in these studies were argatroban, Efegatran, hirulog, and hirudin. While hirulog and hirudin are specific irreversible inhibitors of thrombin, argatroban and Efegatran are reversible. All four agents were found

[New anticoagulants -- their clinical significance].

Heparin and Vitamin K antagonists have been the only available anticoagulants for several decades. Their use has lead to significant achievements in all fields of medicine despite various shortcomings and bleeding complications. With the objective of an improved benefit-/risk ratio selective inhibitors of factor Xa (Fondaparinux) and factor IIa (Ximelagatran) have been developed. Ximelagatran can also be orally administered. The results obtained from various clinical trials with these compounds are extremely encouraging. Thus, a significant improvement of antithrombotic treatment may be expect

Pharmacotherapeutic aspects of unfractionated and low molecular weight heparins.

Standard unfractionated heparin is a mixture of mucopolysaccharide chains of various length that may vary from 5000 to 30,000 daltons. Heparin is only effective as an anticoagulant in the presence of a plasma protein termed antithrombin III, with which it forms a complex. High- and low-affinity heparin are 2 types that readily bind or do not bind, respectively, to antithrombin III. The pharmacokinetics of unfractionated heparin are compatible with a model based on the combination of a saturable and a linear mechanism. The primary indication for intravenous infusion of conventional heparin is t

Contemporary treatment of venous thromboembolic disease.

Venous thromboembolism (VTE) is a potentially fatal condition and includes deep vein thrombosis and pulmonary embolism. The novel oral anticoagulants, which include the direct thrombin and factor Xa inhibitors, have been shown to be safe and effective for the treatment of VTE. Additional interventions include thrombolysis and the use of inferior vena cava filters. The purpose of this article is to provide a contemporary review of the treatment of VTE.

Structural characterization, in vitro anticoagulant, and antiplatelet activities of a Distolasterias nipon dermatan sulfate-like polymer with a distinctive sulfation pattern.

A novel dermatan sulfate-like polysaccharide (DNP) was isolated from the body walls of the starfish Distolasterias nipon. Its structure was elucidated using chemical methods and 2D NMR spectroscopy, revealing a backbone of →4)-α-L-IdopA-(1→3)-β-D-GalpNAc-(1→, with the α-L-iduronic acid residues predominantly 2,3-di-O-sulfated, alongside 2-O- and 3-O-monosulfated variants, and the β-D-GalpNAc residues 4-O-sulfated. Functional assays showed that DNP prolongs thrombin time (TT) comparable to heparin and more potently than enoxaparin (Clexane®), whereas its effect on activated partial thromboplast

Antithrombotic treatment failures in antiphospholipid syndrome: the new anticoagulants?

Anticoagulation with oral vitamin K antagonists (VKA) is the mainstay of the treatment of venous and/or arterial thromboembolism in patients with antiphospholipid syndrome (APS), although the optimal intensity of anticoagulation remains controversial. The limitations of existing anticoagulants have driven a search for novel agents. Dabigatran etexilate (Pradaxa), a direct thrombin inhibitor (DTI), and rivaroxaban (Xarelto), the first in a new class of drugs, the oral direct factor Xa (FXa) inhibitors, are both fixed-dose orally administered agents. They are now licensed in the UK and Europe fo

[Treatment and prevention of venous thromboembolic events: present and future antithrombotic agents].

Two new classes of anticoagulants are actually developed which would change in the near future our strategies for the prevention and the treatment of venous thromboembolic events. These two classes are the anti-factor Xa and anti-factor IIa (direct antithrombin) agents. Among the anti factor Xa, the pentasaccharides are initiating their clinical use. Fondaparinux is a synthetic form of the natural pentasaccharide, its pharmacokinetics allows one s.c. administration/24 hours. It is active in prevention and treatment of venous thromboembolic and coronary thrombotic events. A modified form (idrap

Haemostasis-altering effects of Milos viper (Macrovipera schweizeri) venom.

Haemotoxicity is one of the primary symptoms of viperid envenomation, manifesting in cardiovascular and haemostatic disturbances such as hypotension, haemorrhage, and coagulopathy. Bites by the Milos viper (Macrovipera schweizeri) have been reported to induce symptoms affecting the blood system, including fibrinogenolysis, erythrocytopenia, and venom-induced consumption coagulopathy. Consistent with these reports, its venom contains a variety of haemotoxic components and has been observed to exert strong procoagulant activity on human plasma. However, a more comprehensive analysis of the effec
