Patients (n = 10) at the acute phase of farmer's lung were investigated with chest roentgenography, lung function tests, and bronchoalveolar lavage (BAL) fluid analysis (n = 9). They had diffuse interstitial lung infiltrates and a reduction of the diffusion capacity. The dominating recovered cell types during BAL were lymphocytes; and in two patients, granulocytes. A prominent increase in mast cell numbers was seen in all patients. Fourteen cases of farmer's lung, which were clinically and pathologically defined, were reported. The incidence of disease in Hokkaido was estimated to be 1-2% and its geographical distribution was uniform. It was stressed that there is a considerable number of undiagnosed patients with farmer's lung.

Three cases of acute histoplasmosis demonstrated, in addition to the usual serologic findings, strongly positive precipitins against farmer's lung antigens. Twelve additional serums from patients with acute histoplasmosis were subsequently studied. Forty percent of these patients demonstrated positive precipitins against organic dust antigens.

The presence of anti-lung antibody was evaluated in 20 patients with farmer's lung disease. Antibody was found in 14. Peripheral B lymphocytes of 36 HLA typed farmer's lung (FL) patients were tested in a microcytotoxicity assay with 37 sera from multiparous women. The sera had been previously found negative with T lymphocytes from 150 healthy persons. A quantitative estimate of the IgG subclasses in patients with farmer's lung has revealed significantly higher IgG3 levels in patients with this condition compared to an age-matched group of control farmers similarly exposed to Micropolyspora faeni (P less than 0.01) and a group of normal blood bank donors (P less than 0.001). The IgG1 and total IgG levels were significantly higher in both groups of farmers compared to blood bank controls (P less than 0.001). The possible significance of these observations is discussed in relation to susceptibility to the disease and its pathogenesis.

The numbers of mononuclear cells having receptors for sheep red blood cells (T lymphocytes) or complement (B lymphocytes) in the peripheral blood of farmer's lung patients were determined. In patients recovering from a clinical episode of farmer's lung or exposed to moldy hay or fodder, both the percentage of T lymphocytes and the T lymphocytes per mm3 were reduced, whereas the number of B lymphocytes remained within normal limits. Farmer's lung patients having no exposure to moldy hay or fodder had T and B lymphocyte numbers similar to a normal population.

Complement-fixing antibodies to Micropolyspora faeni were measured in farmers and controls. Farmers with known or suspected farmer's lung showed significantly higher titers than control groups. Variations in titer were measured and correlated with the clinical disease.
