Complications after oesophagectomy with possible contribution of neoadjuvant therapy including an EGFR-antibody to a fatal outcome.

BACKGROUND: Different molecular therapies like the EGFR-inhibiting antibody cetuximab have come into clinical practice. Cetuximab is EMEA-approved for metastatic colorectal cancer and advanced squamous-cell head and neck cancer. Administration is said to be safe and well tolerated with common, usually mild dermatologic side effects. CASE PRESENTATION: We present the case of a patient with fatal complications after oesophagectomy and neoadjuvant chemotherapy including cetuximab for squamous-cell esophageal cancer. A transthoracic en-bloc oesophagectomy was performed. Few days later the patient

Coexistence of light chain disease and chronic lymphocytic leukaemia, a complex karyotype with a rapid fatal outcome.

We report on a 48-year-old man with concomitantly diagnosed kappa expressing chronic lymphocytic leukaemia (CLL) and lambda light chain disease with highly complex chromosomal aberrations. The clinical course of the disease was very aggressive with survival of only 1 month. We demonstrate the distinct clonal origin by cytogenetic data and immunoglobulin rearrangement studies. To our knowledge this is the first report of a light chain disease associated with CLL.

Non-fatal heroin overdose, treatment exposure and client characteristics: findings from the Australian treatment outcome study (ATOS).

The relationship between treatment exposure, drug use, psychosocial variables and non-fatal heroin overdose was examined among a cohort of 495 heroin users, re-interviewed at 12 months. The 12-month overdose rate declined from 24% to 12%, and the proportion administered naloxone declined from 15% to 7%. There were significant reductions in overdose among those who entered maintenance therapies (22% to 4%) and residential rehabilitation (33% vs. 19%) at baseline, but not among those who entered detoxification or were not entering treatment. The total number of treatment days received over the f

Pulmonary embolism associated with combined oral contraceptives: reporting incidences and potential risk factors for a fatal outcome.

BACKGROUND: It is established that combined oral contraceptive (COC) treatment increases the risk of a pulmonary embolism (PE), but specific risk factors for a fatal outcome from a PE remain to be determined. This study aimed to identify such risk factors, and to calculate the reporting rates of fatal and non-fatal PE. METHODS: Cases of suspected PE during treatment with COCs reported to the Swedish Adverse Drug Reactions Advisory Committee (SADRAC) between 1965 and 2001 were included. Medical records were scrutinized for potential risk factors for a venous thromboembolism (VTE). Annual sales…

[Amniotic fluid embolism as an obstetric emergency--description of the syndrome and a case with fatal outcome].

Amnotic fluid embolism is an incompletely understood obstetric emergency presenting with the acute onset of hypoxia, hypotension and severe coagulopathy. Up to 86% of the cases are fatal, maternal death occurs within the first hour of symptom onset in 50% of the patients. This report describes the case of a 29-year-old, HIV-positive patient undergoing elective cesarean section who died despite immediate onset of cardiopulmonary resuscitation. The epidemiology of amniotic fluid embolism, clinical presentation, differential diagnosis, pathology and possible treatment are discussed.

Can intravascular lymphomatosis mimic sinus thrombosis? A case report with 8 months' follow-up and fatal outcome.

We report a case of intravascular lymphomatosis of the brain with 8 months' follow-up and fatal outcome. Several MRI investigations revealed variegated, rapidly changing infarct-like lesions and invasion of the walls of the superior sagittal sinus and deep veins. When disturbances of the venous outflow are detected with multifocal infarct-like lesions, intravascular lymphomatosis should be considered in the differential diagnosis. Brain biopsy may ensure the proper diagnosis ante mortem, but failure of biopsy is frequent, as in our case.

[Olivo-ponto-cerebral hypoplasia--case report of a neurodegenerative disease manifesting at birth with a fatal outcome].

We report on a boy with pontocerebellar hypoplasia Type II according to the classification of Barth. The clinical signs were noted at birth and consisted of muscular hypertonus, central hypopnoe requiring artificial ventilation, chorea, hyperthermia above 40 degrees C and myoclonic seizures resistant to all therapeutic modalities. At birth, the boy was noted to have macrocephaly, but was microcephalic by 3 months of age. He failed to develop any mental or motor facilities. After cessation of all therapy the child died at the age of 3 1/2 months. Postmortem examination of the brain revealed los

Th polyagglutination with fatal outcome in a patient with massive intravascular hemolysis and perforated tumor of colon.

Polyagglutination is a rare disorder which has been associated with intravascular hemolysis (Levene et al.: Transfus Med Rev 2:176-185, 1988). In this condition cryptantigens exposed on the red blood cell membrane agglutinate with compatible sera and with lectins. A 76 year-old man with an acute abdomen due to a perforated tumor of the colon and severe fatal intravascular hemolysis is described. Th polyagglutination of the red blood cells was found.

Fatal outcome of acute thrombocytopenic purpura associated with a viral infection in an 11-year-old child.

A case of fatal intracranial hemorrhage is reported in an eleven year old girl with acute idiopathic thrombocytopenic purpura following a viral infection. The patient was randomized to the IgG-arm of the ITP therapy study. Immunoglobulin administration was not followed by a raise of the thrombocyte count. Neither the IgG therapy nor intensive therapeutic measurements were able to prevent the fatal course of cerebral hemorrhage in this case. Pathological and immunological findings indicate that our patient suffered from a fulminant ITP which must be considered as a part of a still active viral

Case-Fatality and Functional Outcome after Subarachnoid Hemorrhage (SAH) in INternational STRoke oUtComes sTudy (INSTRUCT).

BACKGROUND: There are few large population-based studies of outcomes after subarachnoid hemorrhage (SAH) than other stroke types. METHODS: We pooled data from 13 population-based stroke incidence studies (10 studies from the INternational STRroke oUtComes sTudy (INSTRUCT) and 3 new studies; N=657). Primary outcomes were case-fatality and functional outcome (modified Rankin scale score 3-5 [poor] vs. 0-2 [good]). Harmonized patient-level factors included age, sex, health behaviours (e.g. current smoking at baseline), comorbidities (e.g.history of hypertension), baseline stroke severity (e.g. N

Fatal and non-fatal health outcomes of malaria among reproductive age women in Eastern Sub-Saharan Africa, from 1990 to 2023.

BACKGROUND: Malaria is a life-threatening disease that is transmitted by the bites of infected female Anopheles mosquitoes. Women of reproductive age (WRA) are a more vulnerable group to malaria, which increases the risk of severe anemia, maternal death, and unfavorable birth outcomes. However, only limited comprehensive studies are available in the Eastern Sub-Saharan African (ESSA) region. The Global Burden of Diseases (GBD) 2023 estimates were to assess the fatal and non-fatal health outcomes of malaria among WRA in ESSA region from 1990 to 2023. METHOD: The Cause of Death Ensemble Model (

Immune checkpoint TIM-3 defines hyperactivated NK cells and predicts fatal outcome in severe fever with thrombocytopenia syndrome.

BACKGROUND: Severe fever with thrombocytopenia syndrome (SFTS) is a viral hemorrhagic fever with high mortality, primarily driven by immune … tly reduced IFN-γ and TNF-α production. Elevated sTIM-3 and sGalectin-9 levels were correlated with fatal outcomes. CONCLUSIONS: Activated TIM-3 ⁺ NK cells were associated with fatal outcomes in SFTS. TIM-3 ⁺ NK cell proportion, sTIM-3, and sGalectin-9 may serve as novel prognostic biomarkers and therapeutic targets.

Severe Japanese Spotted Fever With Disseminated Intravascular Coagulation and a Fatal Outcome: A Case Series of Five Patients.

Japanese spotted fever (JSF), caused by Rickettsia japonica, is an endemic tick-borne disease in Japan that usually responds well to tetracycline therapy. However, delayed diagnosis and treatment can result in serious complications such as disseminated intravascular coagulation (DIC) and death. To identify early markers of severe disease, a case series of five JSF patients (one male patient, four female patients; aged 61-78) treated between 2019 and 2024 was analyzed. All patients presented with fever and erythema; two exhibited eschar. Although empirical tetracycline therapy was administered

Surgical complication of SMA and celiac artery ligation during radical nephrectomy for large renal tumor: A fatal outcome.

Iatrogenic ligation of extrarenal vessels during left nephrectomy is a rarely reported yet life-threatening complication, typically occurring due to anatomical vascular variation, distorted anatomy or large lymphadenectomy. Preoperative assessment of anatomy through radiological imaging and careful identification of each vessel intraoperatively is important to prevent this complication. Such complications often result in fatal tissue ischemia and multi-organ failure if not diagnosed and managed in time. We present a case of a 35-year-old female who presented with a large left renal tumor along

Hydrocephalus and Cerebral Small Vessel Disease in an Adult With Meningococcal Meningitis and Dilated Cardiomyopathy: A Fatal Outcome.

Meningococcal meningitis can lead to serious neurological complications such as hydrocephalus directly and contribute to cerebral small vessel disease indirectly, especially in patients with pre-existing conditions like dilated cardiomyopathy. Timely identification and aggressive treatment are essential, although the prognosis is often poor when numerous complications are present.

Cerebellar Hemangioblastoma with Leptomeningeal Spread and a Fatal Outcome: A Rare Case Report with MDM2 and EGFR Alterations.

Hemangioblastoma (HB) is a Central Nervous System (CNS) tumor with a generally favorable behavior and prognosis, classified as WHO grade 1. Sporadic HB is not related to any inherited disease, and it usually appears in a single location. Sporadic or VHL-related HBs show variable patterns of growth velocity. Cases of growing HB can cause mild symptoms such as headache, but some cases develop serious complications such as accumulation of cerebrospinal fluid in the brain with secondary neurological damage sometimes being irreversible when early treatment is not started. Our case showed some clini

FATAL ACUTE ALIMINIUM PHOSPHIDE POISONING - CASE REPORT AND LITERATURE REVIEW WITH REFERENCE TO CURRENT TREATMENT PROTOCOLS AND OUTCOME.

Aluminum phosphide (AlP) has been known for more than 80 years as an effective pesticide for grain protection, but also as a suicidal agent used for human self-poisoning. Phosphine gas released in contact with stomach acid after oral ingestion of AlP is responsible for its toxicity. The poison affects all systems, so the mortality rate is quite high, especially after deliberate ingestions. We report the first case of severe AlP poisoning seen in our institution with a fatal outcome and furthermore, we present literature review on existing and newer treatment options. A 35-year-old woman with a…
