Women's health 18 years after rupture of the anal sphincter during childbirth: I. Fecal incontinence.

OBJECTIVE: We studied the long-term outcome after an anal sphincter tear. STUDY DESIGN: From a cohort of 4569 women who gave birth in 1982 to 1983, we identified 445 (9.7%) who sustained a sphincter tear and 445 controls. Eighteen years after the delivery, we mailed them a questionnaire and graded fecal incontinence with the Wexner score, a summary of incontinence to flatus, liquid, or solid stools; need to wear a pad; and lifestyle alterations. We predefined severe incontinence as a score above 4 of 20. RESULTS: Five hundred forty of 890 women (61%) returned the questionnaire. Severe fecal …

Comparative Effectiveness of Biofeedback vs Dextranomer-Hyaluronate Injection for Fecal Incontinence: A Randomized Clinical Trial.

BACKGROUND AND AIMS: Anal dextranomer injection and anorectal biofeedback therapy have not been rigorously compared in patients with fecal incontinence (FI) that is refractory to medical and behavioral therapy (MBT). This study compared their effectiveness, safety, and costs. METHODS: The Fecal Incontinence Treatment (FIT) study was a clinical trial (ClinicalTrials.gov: NCT03811821) conducted at 5 US medical centers. Adults with MBT-unresponsive FI (≥4 episodes/2 weeks) were randomized to receive either injections or biofeedback. The 3 outcomes were a ≥75% reduction in FI episodes at 3 months…

Baseline Anal Sphincter Elastance May Predict Long-Term Outcomes of Sacral Neuromodulation for Fecal Incontinence.

INTRODUCTION: Anal acoustic reflectometry (AAR), a novel test of anal sphincter function, was shown to predict a successful trial phase of sacral neuromodulation (SNM) for fecal incontinence. This follow-up study aims to explore if AAR can also predict short- and long-term SNM outcomes at less than and more than 5 y, respectively. METHODS: Outcome data were reviewed from a prospectively managed database. Successful treatment was defined as >50% improvement in patient reported fecal incontinence or urgency episodes, or in a symptom severity score. RESULTS: Twenty-six female patients (median: …
