Occurrence and clinical outcome of lateral wall fractures in proximal femoral fractures whose fracture line runs from femoral basal neck to subtrochanteric area.

Treatment methods for proximal femoral fractures, when the fractures run from the femoral basal neck to the subtrochanteric area, have not yet been fully reported. Thus, we aimed to clarify osteosynthesis methods based on the fracture frequency and clinical results. We classified the proximal femoral fractures using the Area classification method based on the location (area) of the fracture line. The proximal femur has 4 areas with 3 boundaries; the center of the femoral neck, the boundary between femoral neck and trochanter, and the plane connecting the lower ends of the greater trochanter an…

Slipped capital femoral epiphysis following fracture of the femoral neck: a case report.

Reports on cases of femoral neck fracture complicated by a slipped capital femoral epiphysis associated with avascular necrosis and coxa vara deformity in children are extremely rare. In this case report, we describe a patient who had complications of a slipped capital femoral epiphysis with avascular necrosis and coxa vara deformity after a Delbet type III left femoral neck fracture. We also describe the surgical treatment to overcome these complications.

Early Postoperative Effects of Mechanical Versus Manual Femoral Canal Opening During Proximal Femoral Nail Antirotation Fixation of Pertrochanteric Femoral Fractures.

Background and Objectives: The method of femoral canal opening during intramedullary fixation may influence the biological response to surgery. The aim of this study was to compare the early biological responses associated with mechanical canal opening and manual canal opening during Proximal Femoral Nail Antirotation (PFNA) fixation in patients with pertrochanteric femoral fractures, with a particular focus on inflammation, coagulation, organ dysfunction, and bone metabolism. Materials and Methods: This single-centre prospective randomised study consisted of 60 participants (50 women) with a …

Peri-Implant Fracture Following Prior Fixation of Femoral Neck Fracture Using the Femoral Neck System: A Case Report.

INTRODUCTION: Peri-implant fractures (PIF) reported after cannulated screw and dynamic hip screw fixation historically occur distal to the implant within the subtrochanteric region secondary to the development of a stress riser. Newer implant designs for femoral neck fractures have attempted to combine the benefits of minimally invasive techniques without forgoing rigid angular stability but bring new potential complications. CASE REPORT: We present a case of an intertrochanteric PIF in the setting of a non-displaced femoral neck fracture treated with the DePuy Synthes Femoral Neck System (FN

Bisphosphonates and atypical femoral fractures: Is the location of the fracture associated with the time of consolidation?

INTRODUCTION: Bisphosphonates are commonly prescribed to mitigate the effects of osteoporosis, a condition associated with an increased risk of fracture. However, despite their success, there has been growing concern regarding the long-term use of bisphosphonates because of the increased risk of atypical subtrochanteric fractures of the femur (AFFs). The objective of this study was to describe a cohort of patients with AFFs and evaluate the risk of evolving to non-union, the time of consolidation associated with the location of the fracture, the time of use of the drug, and the implant involve

Hip Fracture Mortality: Differences Between Intertrochanteric and Femoral Neck Fractures.

The purpose of this study was to identify the specific risk factors that affect mortality in patients with hip fractures and differentiate mortality-associated factors between intertrochanteric (IT) and femoral neck (FN) fractures. A total of 1538 consecutive patients with hip fractures were treated at the authors' institution between January 2005 and October 2013. Ultimately 858 IT and 479 FN fracture patients were included on the basis of age >60 years with an isolated hip fracture. Mortality rate at 90 days was 12.1% for IT and 9.6% for FN fractures. In both IT and FN fractures, variables a

Subchondral fracture of the femoral head after acetabular fracture: a case report.

INTRODUCTION: Preventing post-traumatic osteoarthritis is a challenging problem following acetabular fracture. Progressive osteoarthritis is considered to be caused by an irregular articular surface of the acetabular roof or cartilage injury, but little is known about the pathogenesis of collapse of the femoral head after acetabular fracture. We report a case of post-traumatic osteoarthritis after acetabular fracture in which subchondral fracture of the femoral head contributed to the progressive collapse of the femoral head and osteoarthritis. To the best of our knowledge, there has been no p

Femoral fracture in children (a prospective study of two hundred and four fractures).

A PROSPECTIVE study of 204 femoral fractures in 200 children admitted continuously to a single surgical unit during a period of two years (1979-81), was carried out to determine the adverse effects on the overall outcome of treatment, due to lack of standard orthopaedic equipment to apply low friction traction. There was no case of fracture of the femoral neck, confirming its rarity in children. Left femur was fractured more than the right and middle third shaft was the commonest site. Spiral type of fracture was more common than the transverse or oblique type. Thirty percent of fractures were…

Developmental dysplasia of the hip alters fracture patterns in femoral neck fractures.

Femoral neck fractures are commonly classified using Pauwels' system, which reflects fracture-line inclination and mechanical stability. However, anatomical factors influencing fracture configuration remain unclear. This multicenter prospective cohort study investigated whether pelvic morphology, particularly developmental dysplasia of the hip (DDH), is associated with variations in Pauwels classification. Between April and December 2023, 254 surgically treated femoral neck fractures from nine hospitals were analyzed. Radiological parameters, including center-edge angle, sharp angle, acetabula

Fracture-induced pain-like behaviours in a femoral fracture mouse model.

UNLABELLED: This study is the first comprehensive characterisation of the pain phenotype after fracture using both evoked and naturalistic behaviours in adult male and ovariectomised female mice. It also shows that an anti-nerve growth factor (NGF) therapy could be considered to reduce pain after fracture surgery. INTRODUCTION: Bone fractures are common due to the ageing population and very painful even after healing. The phenotype of this pain is still poorly understood. We aimed to characterise it in a femoral fracture model in mice. METHODS: We employed both adult male, and female ovariec

Spontaneous subcapital femoral neck fracture complicating a healed intertrochanteric fracture.

A spontaneous subcapital femoral neck fracture is an uncommon complication of a healed intertrochanteric hip fracture. To determine the etiology of this complication, 274 patients who had been treated for intertrochanteric hip fractures were followed up over an 8-year period from June 1988 to June 1996. We found 7 fractures of the subcapital femoral neck without a history of a trauma. The remaining 267 patients were defined as the control group. All seven patients with fractures of the subcapital femoral neck were women, and their average age at the time of compression hip screw (CHS) fixation

Postoperative weight-bearing after a fracture of the femoral neck or an intertrochanteric fracture.

Sixty patients who had had operative treatment of a fracture of the femoral neck or an intertrochanteric fracture were allowed to bear weight as tolerated on the injured limb. The average age was seventy-seven years. Computerized gait-testing was performed at one, two, three, six, and twelve weeks postoperatively to quantify weight-bearing. For the purpose of analysis, the patients were divided into three groups according to whether they had internal fixation of a stable fracture, internal fixation of an unstable fracture, or a primary hemiarthroplasty. Thirty-two patients completed the entire

[Fractures of the femoral shaft in children. Apropos of a homogeneous series of 97 fractures].

The authors have analysed the results of 97 fractures of the femoral shaft in children. In 75 instances, the treatment was conservative-traction followed by a plaster cast after three weeks. The reduction was satisfactory in only 19 cases. The plaster cast did not prevent secondary displacement. After surgical treatment, complications were observed in one third of the cases. The review of the cases with a long follow up showed that with growth, there was good remodelling. In 95 p. 100 of the children there was no loss of function detectable by the family, and only a careful clinical and radiol
