PURPOSE: We detail a case of cobalt toxicity with visual and systemic complications, review the pathogenic process for the optic neuropathy and retinopathy, and discuss the controversy of metallic hip prosthesis.
OBSERVATIONS: A 59-year-old female with a history of multiple left hip arthroplasties presented to our clinic with bilateral visual loss. The year prior, she had failure of the hip implant necessitating revision surgery with placement of a chrome-cobalt head. A few months after surgery, she began experiencing blurred and "white, spotty" vision in both eyes in addition to hypothyroidi

PURPOSE: The lumbar plexus, formed by the anterior rami of T12/L1 to L4/L5, provides the main neural supply to the lower abdominal wall, groin, and anterior-medial thigh. Its principal peripheral branches (the iliohypogastric, ilioinguinal, genitofemoral, lateral femoral cutaneous, femoral, and obturator nerves) are clinically relevant targets for invasive diagnostic and therapeutic procedures, including nerve blocks and neuromodulatory techniques. This manual aims to describe how to visualize these six main lumbar plexus branches using ultrasound guidance.
MATERIALS AND METHODS: For each ner

ObjectiveObturator neuropathy is a rare condition. Many neurosurgeons are unfamiliar with the obturator nerve anatomy. The purpose of this study was to define obturator nerve landmarks around the obturator foramen.MethodsFourteen cadavers were studied bilaterally to measure the distances from the nerve root to relevant anatomical landmarks near the obturator nerve, including the anterior superior iliac spine (ASIS), the pubic tubercle, the inguinal ligament, the femoral artery, and the adductor longus.ResultsThe obturator nerve exits the obturator foramen and travels infero-medially between th

INTRODUCTION: The piriformis muscle syndrome (PMS) has remained an ill-defined entity. It is a form of entrapment neuropathy involving compression of the sciatic nerve by the piriformis muscle. Bearing this in mind, a medical examination is likely to be suggestive, as a classical range of symptoms corresponds to truncal sciatica with frequently fluctuating pain, initially in the muscles of the buttocks.
PATHOPHYSIOLOGICAL HYPOTHESES: The piriformis muscle is biarticular, constituting a bridge in front of and below the sacroiliac joint and behind and above the coxo-femoral joint. It is essenti

Percutaneous electrical nerve stimulation (PENS) has gained increasing relevance as a minimally invasive neuromodulation technique for managing musculoskeletal pain and peripheral neuropathies. Its efficacy and safety largely depend on accurate needle placement near target nerves under ultrasound (US) guidance. However, comprehensive didactic resources detailing peripheral nerve visualization remain limited. This paper aimed to develop an educational US-based manual to identify major peripheral nerves of the gluteal region, integrating anatomical, clinical, and sonographic perspectives. A narr

Previous studies using the femoral nerve model in both mice and rats have shown that regenerating motor axons prefer to reinnervate the terminal nerve branch to muscle versus a terminal nerve branch to skin, a process that has been termed preferential motor reinnervation (PMR). If end organ contact with muscle and skin is prevented, this preferential motor reinnervation still occurs in the rat. To better understand the process of preferential motor reinnervation in the mouse, we examined motor neuron reinnervation of muscle and cutaneous pathways without any end organ contact as well as with o

The anterior supine approach for total hip arthroplasty (THA) offers the advantage of operating through a true intravascular and intranervous plane, but it places the lateral femoral cutaneous nerve at risk. The purpose of this study was to identify the incidence of and impairment relating to injury of the lateral femoral cutaneous nerve. We performed a retrospective chart review of 81 hips undergoing anterior supine THA from November 2005 through May 2007 to determine operative time, estimated blood loss, fluoroscopic time, type of anesthesia used, intraoperative complications, and postoperat

Peripheral nerves of the lower limb may become entrapped at various points during their anatomical course. While clinical assessment and nerve conduction studies are the mainstay of diagnosis, there are multiple imaging options, specifically ultrasound and magnetic resonance imaging (MRI), which offer important information about the potential cause and location of nerve entrapment that can help guide management. This article overviews the anatomical course of various lower limb nerves, including the sciatic nerve, tibial nerve, medial plantar nerve, lateral plantar nerve, digital nerves, commo

The descending branch of the lateral circumflex femoral artery (DBLCFA) has been suggested as an option for use in coronary artery bypass grafting (CABG). Our aim was to combine radiological examination, surgical and anatomical preparation, and histological assessment of the DBLCFA to map its variability and to assess the benefits of this conduit in cardiac surgery. The pelvic and femoral arteries were examined by CT angiography (CTA) in 100 patients (aged 68.3 ± 9.3 years) to assess the variability of the DBLCFA. Anatomical dissections were performed on 20 cadavers. In 15 patients, an autolog

Case History: Dairy cows recumbent following calving on farms in South Gippsland, Australia were examined during two 3-month seasonal calving periods in 2011 and 2012 as part of a larger study of 218 recumbent cows. A cohort of 104 cows diagnosed with calving paralysis following dystocia was derived from the larger group, which were examined with 3 days of becoming recumbent. A thorough medical and musculoskeletal clinical examination was performed on each cow including flexor-withdrawal and patellar reflex tests, and postural assessment in the lifted position, unless the facilities were not a

INTRODUCTION: People with type 1 diabetes are at risk of complications, including impaired bone health. Hyperglycaemia and accumulation of advanced glycation end products (AGEs) are involved in the development of those. Bone measurements such as trabecular bone score (TBS), high-resolution periphery quantitative computed tomography (HR-pQCT), and impact microindentation may detect impaired bone health better than bone mineral density (BMD). Clinical measures reflecting risk factors such as AGE accumulation or prevalence of cardiovascular autonomic neuropathy may also predict low bone quality.

Mutations in the GJB1 gene, encoding the gap junction (GJ) protein connexin32 (Cx32), cause X-linked Charcot-Marie-Tooth disease (CMT1X), an inherited demyelinating neuropathy. We developed a gene therapy approach for CMT1X using an AAV9 vector to deliver the GJB1/Cx32 gene under the myelin protein zero (Mpz) promoter for targeted expression in Schwann cells. Lumbar intrathecal injection of the AAV9-Mpz.GJB1 resulted in widespread biodistribution in the peripheral nervous system including lumbar roots, sciatic and femoral nerves, as well as in Cx32 expression in the paranodal non-compact myeli

HISTORY AND CLINICAL FINDINGS: Two patients with circumscribed alopecia on the lateral aspect of the thigh underwent a neurological investigation after medical and dermatological examinations had failed to establish the cause. Patient 1 also had neuralgia of the genitofemoral nerve after osteotomy of the iliac crest; patient 2 had insulin-dependent diabetes mellitus. Within the affected part of the skin both patients had sensory dysfunctions over the area of distribution of the cutaneous lateral femoral nerve. Patient 2 additionally had sensory dysfunctions in other areas of innervation.

Background: Regional anesthesia is often used to minimize postoperative pain, enable early mobilization, and improve patient/family satisfaction after ACL reconstruction. Little is known about the effects of nerve blocks on patient-reported outcomes and functional outcomes in pediatric ACL reconstruction. This study was performed to compare analgesic efficacy, isokinetic strength testing, and Patient-Reported Outcomes Measurement Information System (PROMIS) scores between single-shot femoral nerve block (SSNB) and femoral nerve catheter (FNC). Methods: Pediatric patients (<18 years old) who un

Obesity and a prolonged surgical duration are reported risk factors for meralgia paresthetica (MP) after prone position surgery; however, this fails to explain why MP seldom occurs after prone position craniotomy. We reviewed the incidence of MP after spinal surgery and craniotomy in the prone position and investigated whether unidentified factors are involved in the mechanism of postoperative MP. Between January 2014 and March 2020, we performed 556 prone position surgeries. We excluded patients aged ≤16 years and those who were comatose or who required redo-surgery, and reviewed 446 eligible

We present an unusual case of sartorius muscle tear presenting as acute meralgia paresthetica. A healthy 67-year old male was referred to our department with a one-week history of pain, numbness and bruising over his hip and anterolateral thigh. Extended ultrasound assessment revealed an intramuscular tear of sartorius, with acute hematoma surrounding the adjacent lateral femoral cutaneous nerve. Meralgia paresthetica from acute trauma is rare, with only three published cases relating to fractures of the anterior superior iliac spine. To our knowledge, this is the first case caused by muscular

INTRODUCTION: Meralgia paresthetica can be difficult to diagnose, as neurophysiological studies are often hard to interpret due to excess fatty tissue and the varying anatomy of the lateral femoral cutaneous nerve.
METHODS: We retrospectively analyzed the use of high-resolution ultrasound (HRU) for confirming clinical meralgia paresthetica and compared results with nerve conduction studies.
RESULTS: In all 6 patients evaluated, HRUs showed significantly enlarged nerve diameter and in 3 enlarged cross-sectional area, 4 had absent nerve potentials, and in 2 the potentials could not be recorded

An 83-year-old man presented with severe left anterolateral thigh and knee pain for several days. He was completely unable to ambulate owing to the pain. The patient reported no history of acute back pain prior to presentation. His history initially confounded diagnosis. Subsequent investigations demonstrated a chronic contained rupture of an infrarenal aortic aneurysm with significant vertebral body erosion and obliteration of the ipsilateral psoas muscle. Proposed symptomatology was compression of the lateral femoral cutaneous nerve and its roots. The patient had multiple medical comorbiditi
