Biophysical testing in premature rupture of the membranes.

The management of pregnancies complicated by premature rupture of the membranes (PROM) should include some type of surveillance for the development of infectious complications. This is a necessity because neonatal sepsis is not heralded by maternal chorioamnionitis in the majority of cases. Fetal biophysical assessment holds promise as a method to assess fetal well-being and predict the development of infectious complications. The frequent use of fetal biophysical profiles in patients with PROM helps to distinguish healthy fetuses from those infected or at high risk of infection. This distinct

Labor admission test.

The Labor Admission Test permits the clinician to reallocate risk patients based on their admission fetal heart rate pattern. This means that obstetric patients normally considered high risk on admission to labor and delivery can be reallocated to low risk when the initial fetal heart rate pattern is reactive.

Intrapartum fetal pulse oximetry in the presence of fetal cardiac arrhythmia.

We report our experience with a reflectance pulse oximeter in intrapartum monitoring of a fetus with atrial flutter and heart block. During labor fetal arterial oxygen saturation ranged between 50% and 85%. Spontaneous vaginal delivery occurred with good maternal and neonatal outcome.

[Fetal heart rate patterns in response to acoustic stimulation].

Fetal heart rate response to fetal acoustic stimulation (FAST) during the antepartum period after 25 weeks of gestational age was studied in 157 cases of normal pregnancy, 22 cases of fetal distress, 12 cases of SFD, 10 cases of fetal malformation, 9 cases of placental dysfunction, 4 cases of oligohydramnios, 12 cases of toxemia of pregnancy, and 15 cases of SLE. Five different types of response were observed: Type I through type V. 1. FAST was available to evaluate fetal well-being from 27 weeks of gestational age, although a nonstress test (NST) has been used after 31 weeks of gestational ag

[Critical analysis of fetal risk recognition by serum estriol determination].

In 901 pregnancies between 1978-1981 a total of 5281 serum estriol determinations were performed. 596 gravidas were high-risk patients (diabetes mell., toxemia, IUGR, premature labour, prolonged pregnancy, Rh incompatibility). In 142 of these high-risk pregnancies the estriol values were below the 10th percentile or the estriol decrease extended 40%. These cases were analysed individually, in order to answer the question, how often serum estriol values were decisive for active obstetrical management. Our data show that a drop of estriol over 40% was very rare (0.53%). For the newborn such an e

Towards Noninvasive Accurate Detection of Intrapartum Fetal Hypoxic Distress.

Current intrapartum fetal well-being assessment is performed using electronic fetal monitoring (EFM), technically referred to as cardiotocography (CTG), which transabdominally monitors fetal heart rate (FHR) in relationship to maternal uterine contractions. Sometimes the deceleration in FHR following a uterine contraction can be sign of fetal hypoxic distress, but it may also be a normal physiological response. Multiple studies have shown that EFM has a high false positive rate for detecting fetal hypoxia. This has caused a rise in emergency Cesarean section (C-section) deliveries performed in…

Fetal bradycardia and acidosis during maternal parenteral iron: Case reports and literature review.

Iron deficiency anemia is an important problem among pregnant women, and intravenous (IV) iron infusions have been increasingly used. Whether fetal monitoring is required during infusion has been debated, with a recent guideline by Hematologists recommending against such. We report two cases of fetal bradycardia after iron isomaltoside (IIM), in women with otherwise good maternal and fetal health. Both developed dyspnea with desaturation minutes from infusion, followed by persistent fetal bradycardia. Both underwent category 1 CS, with cord arterial pH of 7.08 and 6.94 respectively. Upon liter…

Mathematical relationships between uterine contractions, cervical dilatation, descent and rotation in spontaneous vertex deliveries.

OBJECTIVE: To determine the mathematical relationships between the strength and duration of the uterine contractions, the descent and rotation of the fetal head and the degree of cervical dilatation in 50 multiparous women with spontaneous vaginal deliveries using a simple device applied to the fetal vertex. METHOD: A simple device for monitoring the progress of labor was applied to the fetal vertex. The device allows the continuous monitoring of descent and rotation of the fetal head. The amount of descent and the degree of rotation were also determined by repeated vaginal examinations as we

Anesthetic management of cerebral aneurysm clipping during pregnancy: a case report.

A 35-week pregnant female presented emergently for clipping of a cerebral aneurysm under general anesthesia. The patient was neurologically stable with an active fetus. Anesthetic goals for this patient included maintenance of uteroplacental perfusion, fetal well-being, and maternal well-being. Maternal monitoring consisted of invasive arterial blood pressure, central venous pressure, and urine output, in addition to the standard monitors for anesthesia. Fetal monitoring consisted of fetal heart rate by external Doppler and uterine activity by external tocometer. Anesthesia care was directed a…

ACR Appropriateness Criteria® growth disturbances - risk of intrauterine growth restriction.

Fetal growth disturbances include fetuses at risk for intrauterine growth restriction. These fetuses may have an estimated fetal weight at less than the 10% or demonstrate a plateau of fetal growth with an estimated fetal growth greater than the 10%. Uteroplacental insufficiency may play a major role in the etiology of intrauterine growth restriction. Fetuses at risk for intrauterine fetal growth restriction are susceptible to the potential hostility of the intrauterine environment leading to fetal hypoxia and fetal acidosis. Fetal well-being can be assessed using biophysical profile, Doppler

[Fetal pulse oximetry. Intrapartum foetal hypoxia evaluation. Comparative study with invasive techniques concerning foetal welfare].

UNLABELLED: Pulse oximetry is a technique that continuously measures the blood oxygen saturation (%SpO2); it has become a standard monitoring method in Anaesthesia, Intensive Care, Neonatology, etc; and it can be helpful in Obstetrics during delivery to monitor foetal welfare. Its advantages are that it is minimally invasive and easy to use. Moreover, SpO2 is a good parameter for measuring oxygen carriage. A saturation below 30% during a time period over 10 minutes is correlated with an acid-base equilibrium (ABE) of foetal scalp blood sample below 7.20, and it is predictive of worse perinatal

An evaluation of the importance of formal, maternal fetal movement counting as a measure of fetal well-being.

Maternal perception of fetal movements has, over the years, become recognised as a valuable tool for early detection of fetal compromise. Several studies published in the mid-1970s have demonstrated that a reduction or cessation in maternally perceived fetal movements may precede antepartum late fetal death by a day or longer. From these findings formal fetal movement counting emerged as a valuable, non-invasive method of assessing fetal well-being; the belief being that clinical actions taken on the basis of reduced fetal movement counting may prevent antepartum death or morbidity. Conversely
