Fetal Brain Lesions after Endoscopic Laser Surgery in Twin-to-Twin Transfusion Syndrome: An MRI Classification.

BACKGROUND AND PURPOSE: Brain injury in monochorionic pregnancies complicated by twin-to-twin transfusion syndrome (TTTS) remains incompletely understood, and reported patterns vary widely due to heterogeneous imaging protocols. This study aimed to characterize fetal brain lesions after endoscopic laser surgery for TTTS using a structured MRI-based classification. MATERIALS AND METHODS: We conducted a retrospective observational study of TTTS pregnancies treated with endoscopic equatorial laser photocoagulation between January 2012 and January 2023. Fetuses with posttreatment brain abnormalit

Fetoscopic two-layer closure for open neural tube defects: prospective study of obstetric, surgical and perinatal outcomes in the first 50 cases.

OBJECTIVE: Fetoscopic repair for open neural tube defects (ONTDs) has gained acceptance among leading groups, although it remains controversial owing to the lack of a standardized neurosurgical technique. In 2018, our group described a new fetoscopic two-layer procedure with an exteriorized uterus for ONTD reconstruction. This study aimed to report obstetric, surgical and perinatal outcomes for the first 50 cases since the implementation of this technique and to provide comparative data with open fetal surgery studies. METHODS: This was a single-center, observational, prospective study conduc

Anesthetic management for fetal surgery: lessons from a single-center experience (2019-2023).

BACKGROUND: Fetal surgery presents distinctive anesthetic challenges due to the need to balance maternal and fetal safety. This retrospective study evaluates the anesthetic management strategies used at a single center for various fetal interventions, including fetoscopic laser photocoagulation, fetal endoluminal tracheal occlusion, bipolar cord coagulation, and fetoscopic spina bifida repair. METHODS: We reviewed 195 fetal procedures performed between 2019 and 2023 at Rabin Medical Center. Data collected included maternal and fetal characteristics, anesthetic techniques (spinal, combined spi

The clinical experience of fetoscopic repair of myelomeningocele in Taiwan: The dilemma in prenatal decision-making and first successful case.

OBJECTIVE: Objective: Myelomeningocele (MMC) is the most severe type of spina bifida, with an incidence of 1.87 per 10,000 live births in Taiwan. Exposure of the lesion to amniotic fluid exacerbates neurological outcomes, while fetal surgery for MMC repair, now a routine practice, improves postnatal outcomes. However, Asian women and their families often find it difficult to accept prenatal defects, leading nearly all pregnancies with fetal MMC to opt for termination without considering fetal surgery. MATERIALS AND METHODS: In Taiwan's first approved trial of fetoscopic MMC repair, we prospec

Morbidity in children after fetoscopic endoluminal tracheal occlusion for severe congenital diaphragmatic hernia: Results from a multidisciplinary clinic.

BACKGROUND: Although fetoscopic endoluminal tracheal occlusion (FETO) was recently shown to improve survival in a multicenter, randomized trial of severe congenital diaphragmatic hernia (CDH), morbidity outcomes remain essentially unknown. The purpose of this study was to assess long-term outcomes in children with severe CDH who underwent FETO. METHODS: We conducted a prospective study of severe CDH patients undergoing FETO at an experienced North American center from 2015-2021 (NCT02710968). This group was compared to a cohort of non-FETO CDH patients with severe disease as defined by liver

Interrater agreement for sonographic stomach position classification in fetal diaphragmatic hernia across the North American Fetal Therapy Network.

OBJECTIVE: To evaluate inter-rater agreement for sonographic classification of stomach position (as a surrogate for liver herniation) in fetal left congenital diaphragmatic hernia (LCDH) among: (i) fetal medicine specialists from the North American Fetal Therapy Network (NAFTNet) centers within and without the fetal endoscopic tracheal occlusion (FETO) consortium and in comparison to an expert external reviewer (ER1); and (iii) among two expert ERs (ER1 and ER2). METHODS: Forty-eight physicians from 26 NAFTNet centers and 2 ERs were asked to assess 13 sonographic clips of isolated LCDH and cl

Spinal Dysraphia, Chiari 2 Malformation, Unified Theory, and Advances in Fetoscopic Repair.

Fetal spina bifida, the most common nonlethal birth defect of the central nervous system, results in substantial neurologic morbidity. The unified theory describes the complex relationship between local spinal lesions and development of Chiari 2 malformation, contributing to hydrocephalus. Prenatal ultrasonography reliably allows diagnosis, but fetal MR imaging is an important complement to identify additional brain abnormalities. Fetal surgery improves neurologic and motor outcomes, but various approaches, either open hysterotomy or minimally invasive to the uterus, carry substantial obstetri

Donor mesenchymal stem cell kinetics after transamniotic stem cell therapy (TRASCET) in a rodent model of gastroschisis.

PURPOSE: We sought to comprehensively scrutinize donor mesenchymal stem cell kinetics following transamniotic stem cell therapy (TRASCET) in experimental gastroschisis. METHODS: A gastroschisis was surgically created in 102 rat fetuses at gestation day 18 (term = 22 days), immediately followed by volume-matched amniotic injections of either amniotic fluid mesenchymal stem cells (afMSCs) labeled with a luciferase reporter gene (n = 58), or luciferase protein alone (n = 44). Samples from multiple anatomical sites from survivors were screened for luciferase activity via microplate luminometry at

Fetal Surgery and Delayed Cord Clamping: Neonatal Implications.

Advances made in the last several decades in the care of the fetus and newborn have had a significant impact on morbidity and mortality. Delayed umbilical cord clamping in the preterm newborn results in fewer transfusions for anemia, decreased intraventricular hemorrhage, and decreased necrotizing enterocolitis. Because of advances made in fetal ultrasound diagnosis and technological advances, fetal surgeries to treat congenital diaphragmatic hernia, myelomeningocele, twin-to-twin transfusion syndrome, fetal lower urinary tract obstructions, amniotic band syndrome, and congenital cystic adenoi

Due to the responsibility for the mother and the unborn child, fetal surgery represents a challenging task for the anesthesiologist. Maternal changes during pregnancy have to be considered as well as the fetal physiology and the surgeon's needs. Main principles of the anesthesiological management of fetal surgery include the stabilization of the mean arterial pressure in order to preserve a sufficient placental blood flow, sustainment of an adequate oxygenation and ventilation as well as thorough temperature surveillance. Since the mothers are at risk for perioperative pulmonary edema, extende

Twin-to-Twin Transfusion Syndrome: From Observational Evidence to Randomized Controlled Trials.

Fetoscopic surgery is widely accepted as the preferred first-line treatment for twin-twin transfusion syndrome (TTTS). Nonetheless, the broad diffusion of this technique relies on a single multicentric-randomized trial. We hereby question this trial in a post-hoc Bayesian analysis, submitting its results to several scenarios comprising the alternative published non-randomized literature and pessimistic opinions regarding this surgery. Furthermore, we also discuss further refinements in indications, questioning potential alternatives in early stages of the disease.

Twin-to-twin transfusion syndrome--30 years at the front.

Thirty years ago, twin-to-twin transfusion syndrome (TTTS) was one of the most challenging problems of modern obstetrics. The associated mortality and morbidity was staggering, and a sense of hopelessness prevailed among clinicians and pediatric pathologists. Technical advances in ultrasound, endoscopy, video recording, and medical lasers formed the basis for diagnosis and in utero therapy of placental disorders. We review some historical events which gave rise to a new treatment modality for TTTS, fetoscopic laser occlusion of chorioangiopagous vessels, now the most common operation performed

Fetal bronchoscopy: its successful use in a case of extralobar pulmonary sequestration.

OBJECTIVE: To report the performance of fetal bronchoscopy in a case of pulmonary sequestration. MATERIALS AND METHODS: A 24 year-old female, Gravida 2, Para 1, was referred at 27.5 weeks with a large fetal left lung mass with marked right mediastinal shift and no visible normal left lung. Differential diagnosis included possible bronchial atresia. RESULTS: The patient underwent fetal laryngoscopy and fetal bronchoscopy at 31.5 weeks. The right lung and a portion of the left lung expanded during surgery as a result of bronchial lavage. Bronchial atresia or bronchogenic cyst were not found. P

Twin-twin transfusion syndrome.

Twin-twin transfusion syndrome (TTTS) is a serious condition that affects 10% to 15% of twin pregnancies with monochorionic diamniotic placentation. The pathophysiology of TTTS is not completely understood; however, the presence of unbalanced placental vascular communications within a shared circulation has been implicated in its development. The presentation of TTTS is highly variable, and it does not always progress in a predictable manner. Monochorionic twin gestations should, therefore, be monitored for signs of TTTS with serial sonograms starting in the second trimester. Early TTTS can be

Laser therapy for twin-to-twin transfusion syndrome (TTTS).

Monochorionic twins are subjected to specific complications which originate in either imbalance or abnormality of the single placenta serving two twins including twin-to-twin transfusion syndrome. The diagnosis is well established in overt clinical forms with the association of polyuric polyhydramnios and oliguric oligohydramnios. The best treatment of cases presenting before 26 weeks of gestion is fetoscopic laser ablation of the intertwin anastomoses on the chorionic plate. Although subjected to subtle variations, the core technique follows robust guidelines which could help understanding an

Laser ablation of placental vessels in twin-to-twin transfusion syndrome: a paradigm for endoscopic fetal surgery.

Endoscopic fetal surgery is most commonly used for the treatment of twin-to-twin transfusion syndrome (TTTS), but the surgical techniques can be applied to other forms of fetal surgery. We present our experience with endoscopic fetal surgery over the past 10 years. From 2000 to 2010, 70 endoscopic laser ablations of placental vessels for TTTS were performed. Median number of placental vessels ablated was four. The incidence of preterm rupture of membranes (PROM) was 6%. Overall survival was 70%, with at least one twin surviving in 82%. Tocolysis was used in 73% of patients for a median of 12 h

Update on twin-to-twin transfusion syndrome.

Fetal and perinatal loss rates are much higher in monochorionic than in dichorionic pregnancies. This is likely to be at least in part a consequence of chorio-angiopagus and severe early onset of twin-to-twin transfusion syndrome (TTTS). TTTS occurs in approximately 15% of all monochorionic pregnancies and is defined sonographically by the combination of polyhydramnios in one sac and oligohydramnios in the other. Intertwin anastomoses on the chorionic plate are responsible for the development of a net imbalance in blood flow and subsequent development of TTTS. All cases of monochorionic pregna
