Spatial organization of types I and II collagen in the canine meniscus.

The meniscus of the knee joint is a fibrocartilage mainly composed of type I collagen and smaller amounts of type II collagen. The distribution of type II collagen in the canine meniscus and its spatial relationship to type I collagen was examined by immunohistochemistry and confocal microscopy. Dorsal and coronal slices of the mid-section of medial and lateral menisci from the knee joints of skeletally mature dogs were predigested with Streptomyces hyaluronate lyase and bacterial Protease enzyme XXIV. Monoclonal antibodies against type I collagen (CP17L) and type II collagen (II-II6B3) and an…

Bone Morphogenetic Protein-2 and Cabozantinib for Temporomandibular Joint Cartilage Repair: Murine and Porcine Injury Models.

Temporomandibular joint osteoarthritis (TMJ OA) is characterized by progressive degeneration of the fibrocartilaginous articular surface. We previously demonstrated that combined bone morphogenetic protein-2 (BMP-2) and vascular endothelial growth factor (VEGF) inhibition promotes endogenous hyaline cartilage regeneration in appendicular joints. Whether this regenerative strategy can be extended to the specialized fibrocartilage of the TMJ remains unknown. In this study, we evaluated the regenerative potential of BMP-2 combined with cabozantinib, an FDA-approved multikinase inhibitor with acti…

Surgical treatment of acute distal radioulnar joint instability associated with distal radius fractures.

This study investigates the question of whether open repair of acute distal radioulnar joint instability at the time of volar plating of distal radius fractures would enable early mobilization of the wrist without the risk of distal radioulnar joint instability. We evaluated 29 patients of mean age 53 years with a distal radius fracture and acute distal radioulnar joint instability who underwent volar plating of the radius combined with surgical repair of the triangular fibrocartilage complex or an ulnar styloid base fracture, followed by active motion exercise of the wrist at 1 week after sur…

Pathophysiology of longitudinal forearm instability (Essex-Lopresti syndrome) and implications for treatment.

Longitudinal forearm instability, or Essex-Lopresti syndrome, associates radial head fracture and rupture of the structures uniting the 2 bones, mainly the interosseous membrane and triangular fibrocartilage complex adjacent to the distal radioulnar joint. It is often overlooked at first, and should be screened for in case of comminuted radial head fracture without elbow dislocation or instability. Treatment should be prompt, within 4 weeks of trauma, to avoid soft-tissue retraction and hopefully allow healing. This interval is anecdotal, without firm evidence, but matches observations regular…

Arthrotomography of the wrist: an experimental and preliminary clinical study.

Experimental arthrotomography including posteroanterior and lateral projections was completed in 18 fresh-frozen wrists from cadavers with an average age of 65 years at death. Arthrotomographic findings were correlated with anatomic dissections. Three types of defects of the triangular fibrocartilage complex were clearly defined in 38% of wrists. Chondromalacia was present in 24% of wrists, in all cases on the ulnar half of the proximal surface of the lunate bone. The palmar radiocarpal ligaments (radiocapitate and radiotriquetral) were visible in all specimens. Experimental arthrotomography a…

Correspondence. Reply to the article "Cornelis S, Mufty S, Peters S. Isolated distal radioulnar joint septic arthritis, the intact triangular fibrocartilaginous complex serving as an anatomical barrier. Hand Surg Rehabil, January 2025.

Cornelis et al. reported an isolated DRUJ infection treated with open surgery to prevent spread to the radiocarpal joint. We suggest that arthroscopy, proven effective in other joint infections and technically feasible for the DRUJ without damaging the TFCC,could have been a less invasive and effective alternative.

Predictors of Secondary Ulnar Shortening and Reoperation After Arthroscopic TFCC Debridement.

BACKGROUND: Predicting which patients will do well with arthroscopic triangular fibrocartilage complex (TFCC) debridement alone or which patients may benefit from arthroscopic debridement and ulnar shortening at the same time can be challenging. In this retrospective cohort study, we aimed to assess the rate and type of complications and reoperations after arthroscopic TFCC debridement. Furthermore, we aimed to identify factors associated with reoperation and specifically ulnar shortening osteotomy (USO) after this procedure. METHODS: We included 163 patients who underwent arthroscopic TFCC d…

Editorial Commentary: Lesions of the Superior Labrum From Anterior to Posterior (SLAP) Are a Slap in the Face to the Traditional Trinity of History, Examination, and Imaging.

Superior labrum from anterior to posterior (SLAP) lesions are a slap in the face to the revered trinity of history, physical examination, and imaging. SLAP lesions are difficult to diagnose, and arthroscopy is not only the gold standard, but the current method on which expert arthroscopic shoulder subspecialists rely.

Arthroscopic management of distal radius fractures.

Arthroscopy has the advantage of providing a direct and accurate assessment of the articular surfaces and detecting the presence of injuries associated with distal radius fractures. Current indications, although numerous and potentially expanding, also are controversial. This report presents a global view of the current status of arthroscopy in the management of distal radius fractures. The rationale of arthroscopic treatment, the available evidence, and finally the diagnosis and treatment are discussed.

FSTL1 Orchestrates Metabolic-Epigenetic Crosstalk: Glycolysis-Dependent H3K18 Lactylation Drives Cartilage Fibrosis in Osteoarthritis.

The progression of osteoarthritis (OA) is fundamentally characterized by the aberrant transformation of chondrocytes into a fibrotic phenotype, although the precise molecular mechanisms involved remain inadequately understood. In this study, the interplay between epigenetic modifications and metabolic reprogramming during the activation of fibrocartilage cells in osteoarthritis was investigated. The findings demonstrate that FSTL1 markedly upregulates key glycolytic enzymes, including LDHA, HK2, and PKM, in chondrocytes, triggering a "glycolytic burst" that results in elevated intracellular la…

Histological and molecular characterization of the femoral attachment of the human ligamentum capitis femoris.

The ligamentum capitis femoris (LCF) has increased in clinical significance through the development of hip arthroscopy. The histological pathologies and molecular composition of the femoral attachment of the LCF and the degeneration caused by LCF disruption were investigated in the human hip joint. Twenty-four LCFs were retrieved at surgery for femoral neck fracture (age range: 63-87 years). In the "intact" (i.e., intact throughout its length, n = 12) group, the attachment consisted of rich fibrocartilage. Fibrocartilage cells were present in the midsubstance. In contrast, the construction of …
