Transiliac approach for exposure of lumbosacral intervertebral disk and foramen: technique description.

OBJECTIVE: To describe and evaluate a transiliac approach to the L7-S1 disk and intervertebral foramen in dogs. STUDY DESIGN: Cadaver study … k and/or foramen. CLINICAL RELEVANCE: Transiliac approach could be used for lateral corpectomy and foraminotomy in dogs with degenerative lumbosacral stenosis caused by ventral or ventrolateral disk protrusion, foramen stenosis, or OCD of the L7-S1 joint. Clinical study will be necessary to evaluate the efficacy of this approach.

Comparison between posterior lumbar fusion with pedicle screws and posterior lumbar interbody fusion with pedicle screws in adult spondylolisthesis.

The purpose of this study was to compare patients with lumbar spondylolisthesis submitted to two different surgical approaches, and evaluate … to a posterior lumbar interbody fusion procedure (PLIF) with pedicle screws. All patients underwent foraminotomy for nerve root decompression. Clinical evaluation was carried out using the Prolo Economic and Functional Scale and the Rolland-Morris and the Oswestry questionnaire. Mean age was 52.4 for Group I (PLF), and 47.6 for Group II (PLIF). The mean follow …

Objectively measured physical activity following lumbar decompression surgery: systematic review and meta-analysis.

Lumbar decompression surgery reliably improves pain and disability in lumbar disc herniation and spinal stenosis, but whether these symptomatic gains translate into greater free-living physical activity (PA) remains unclear. This systematic review and meta-analysis evaluates postoperative changes in objectively measured PA, and its association with patient-reported outcome measures (PROMs). PubMed, MEDLINE, EMBASE and Scopus databases were searched to identify studies reporting objectively measured PA after lumbar decompression, including laminectomy, laminotomy, foraminotomy and discectomy. Cervical Radiculopathy Caused by Epidural Venous Varix Associated with a Dural Arteriovenous Fistula in the Anterior Condylar Confluence: A Case Report.

OBJECTIVE: While degenerative disorders are the typical cause of cervical radiculopathy, vascular etiologies are exceedingly rare. Dural art … d to cause motor weakness. After a short observation period, the patient underwent direct posterior foraminotomy with variceal coagulation. Postoperatively, motor strength steadily improved and eventually normalized, with sustained recovery during follow-up. Anterolateral Approach for a Cervical Nerve Root Compression Syndrome Due to an Ectatic Vertebral Artery.

BACKGROUND AND IMPORTANCE: Nerve root compression by an ectatic vertebral artery is a known but easily overlooked cause of cervical radiculo … ximal right C5 nerve root. She previously underwent both nonoperative measures as well as posterior foraminotomy at this level with limited improvement in her pain. Therefore, we performed an anterolateral approach for vascular decompression of the C5 nerve root from the vertebral artery loop. Patient has been pain-free at 1 yr with full recovery of her deltoi …

Interventional Therapy and Surgical Management of Lumbar Disc Herniation in Spine Surgery: A Narrative Review.

Significant advancements in lumbar disc herniation (LDH) management have been made in interventional pain therapy, operative therapy, peri-o … ndoscopic discectomy, tubular discectomy, percutaneous laser disc decompression, and transforaminal foraminotomy. Prevention of infections during surgery is paramount and is often done via a single-dose preoperative antibiotic prophylaxis. Recurrence of LDH post-surgery is commonly observed and thus mitigative strategies for prevention have been proposed inclu …

Microsurgical jugular foraminotomy followed by balloon angioplasty and stenting significantly reduced jugular pressure gradients. Symptomatic cervical disc herniation in teenagers: two case reports.

INTRODUCTION: The development of a symptomatic herniated cervical disc before the age of 20 is extremely rare. Sporadically reported cases of patients with cervical disc herniation under the age of 20 usually have had underlying disease. CASE PRESENTATION: Case 1: A 19-year-old Asian man visited our clinic and presented with progressive pain in his upper left scapula and weakness of the left deltoid and biceps brachii muscles. C5 radiculopathy by soft disc herniation at C4-C5 without calcification was diagnosed. Microsurgical posterior foraminotomy was performed and he recovered completely ei…

Postoperative cauda equina syndrome in patients undergoing single-level lumbar microdiscectomy. Report of two cases.

Cauda equina syndrome is a well-documented complication of uneventful lumbar microdiscectomy. In the vast majority of cases, no radiological explanation can be obtained. In this paper, the authors report two cases of postoperative cauda equina syndrome in patients undergoing single-level de novo lumbar microdiscectomy in which intraoperative electrophysiological monitoring was used. In both patients, the amplitudes of cortical and subcortical intraoperative somatosensory evoked potentials (SSEPs) abruptly decreased during discectomy and foraminotomy. In the first patient, a slow, partial impro…

Bow Hunter's Syndrome: Surgical Management (Video) and Review of the Literature.

BACKGROUND: Bow Hunter's syndrome (BHS) is a rare condition characterized by vertebrobasilar insufficiency associated with rotational vertebral artery occlusion during head movement. Many existing reports describe surgical management, although no single technique has proven superior. OBJECTIVE: To review all reported cases of BHS with focus on outcomes of individual techniques as well as to present a video report of a posterior decompression without fusion performed at the authors' institution. METHODS: We searched PubMed for all relevant articles of BHS available in the English language. Pe

Anterior Cervical Discectomy and Fusion.

Anterior cervical discectomy and fusion can be performed for a variety of pathologies but is most commonly used for the treatment of cervical radiculopathy or myelopathy. The procedure involves an anterior decompression of the disc space followed by interbody grafting and fusion. Supplemental anterior plating is commonly performed, and in certain circumstances, posterior instrumentation may provide additional fixation. The procedure includes the following steps: (1) The use of an anterior approach to the cervical spine, most commonly the Smith-Robinson approach medial to the sternocleidomastoi

Cervical discectomy. A prospective analysis of three operative techniques.

BACKGROUND: Cervical disc herniation causing neurological compromise is a common affliction. Sophisticated surgical treatments have been developed throughout the twentieth century and are largely successful. Although each procedure has its supporters, it is still unclear if one surgical technique is superior. METHODS: A prospective trial was designed to evaluate the efficacy of three surgical procedures for the treatment of cervical radiculopathy caused by a unilateral acute herniated cervical disc. Patients were randomized to posterior cervical foraminotomy (FOR), and anterior cervical disce…
