Gartland type IV supracondylar humerus fractures in children are characterized by complete periosteal disruption and multidirectional instability, frequently resulting in failure of standard closed reduction maneuvers. Persistent rotational instability is the primary factor leading to loss of reduction and the subsequent need for open reduction. Although open reduction is effective, it is associated with increased soft-tissue dissection, longer operative times, and a higher risk of postoperative stiffness and neurovascular complications. We describe a temporary lateral cortical pin-assisted cl

BACKGROUND: Painful procedures such as closed reduction of forearm fractures are a major source of procedural pain and fear in children. Virtual reality (VR) has gained attention as a non-pharmacological distraction adjunct to standard care. This study determined the effectiveness of VR in reducing pain and fear in children undergoing closed reduction of forearm fractures.
METHOD: A quasi-experimental pre-post test unmatched group design was used in the orthopedics and emergency departments of a training and research hospital between June 2023 and November 2025. Fifty-one children aged 6-12 y

Introduction Subtrochanteric femur fractures are challenging injuries due to high mechanical stress and compromised vascularization, resulting in higher complication rates compared with other proximal femur fractures. While intramedullary nailing is the treatment of choice, controversy persists regarding whether open or closed reduction provides superior outcomes. Some evidence suggests that open reduction may achieve better alignment, whereas others highlight risks of impaired biology, blood loss, and infection. This study aimed to compare open versus closed reduction in subtrochanteric fract

BACKGROUND: Femoral nail bending is a rare complication of intramedullary (IM) fixation of femoral diaphyseal fractures. Published literature regarding this injury pattern has thus far been limited to case reports or case series, thus no universally accepted surgical treatment strategy has been developed.
METHODS: A systematic review was conducted using the Pubmed/MEDLINE and Scopus/EMBASE databases. A standardized template was used to extract data including author, year of publication, patient demographics, degree of angulation, mechanism of injury, time since initial procedure to reinjury,

BACKGROUND: Displaced ankle fractures are initially closed reduced and splinted with the goal of restoring gross ankle alignment. The benefits of an exact closed reduction are unclear and possibly detrimental and unnecessary if multiple attempts are made. The purpose of this study was to determine whether the quality of preoperative closed reduction in patients with operative ankle fractures affects post-operative wound complications.
METHODS: A retrospective analysis was performed of patients with isolated, closed, operative ankle fractures treated at two level 1 trauma centers who had an in

Vertical femoral neck fractures (Pauwels type III classification) in young adults generally occur as a consequence of high-energy trauma and are frequently seen in association with multiple injuries. Considering the controversies regarding the optimal fixation for this fracture, our aim was to evaluate the clinical outcome of a closed fixation strategy for vertical femoral neck fractures in young adults using two parallel and one transverse cancellous lag screws. This was a single-surgeon, prospective study including 20 young adults with average age of 38.75 years (range 18-59 years) with a hi

INTRODUCTION: The treatment of fractures of the mandibular process remains controversial, although there is a trend towards open reduction and internal fixation. This study compared open and closed treatments and assessed the results with a meta-analysis.
MATERIALS AND METHODS: A literature search of PubMed found eight studies that met the search criteria and were included in the meta-analysis.
RESULTS: The studies increasingly suggest better results for open treatment, in terms of mouth opening, protrusion, laterotrusion, pain, and malocclusion. In the meta-analysis, the outcome was signifi

BACKGROUND: In treating pediatric lateral condyle fractures (LCFs) of the humerus, closed reduction and percutaneous pinning (CRPP) is an attractive alternative to open reduction and internal fixation (ORIF) because of the potential decrease in tissue injury, shorter surgical times, and faster functional recovery. However, there is limited information available regarding its outcome.
METHODS: The data on 191 pediatric LCFs [163 fractures (85%) treated with ORIF (group 1) and 28 fractures (15%) treated with CRPP (group 2)], who were followed for over 12 weeks, was included in the present analy
